Preventive interventions for tendinopathy: A systematic review.

OBJECTIVES: Tendinopathy, the most prevalent tendon disorder which is considered as the clinical diagnosis of pain and dysfunction, is common in sports and its prevalence is ever-increasing. Despite the lack of clarity about risk factors, various preventive interventions for tendinopathy have been investigated. The main objective of this study is to review current preventive interventions for tendinopathy in the major regions: ankle, knee, hip, groin, shoulder and elbow. DESIGN: A systematic literature search was conducted. METHODS: The PubMed and Embase databases were explored to identify a

Dry needling as a method of tendinopathy treatment.

Tendinopathy is a broad concept that describes any painful condition that occurs in or around a tendon. The ideal treatment for tendinopathy is still nebulous. Dry needling is a treatment method in which a special needle is placed into the focus of tendinosis. The aim of this procedure is to form fenestrations which may initiateadvantageous bleeding and thus bring about the influx of growth factors (activating healing and regeneration). Relevant clinical studies have often combineddry needling with autologous blood injection therapy. Results from these studies are encouraging. This review of En

Tendinopathy of the tendon of the long head of the biceps.

Pathologies of tendon of the long head of the biceps (LHB) are an important cause of shoulder pain. They include tendinopathy, rupture, superior labrum anterior and posterior lesions, pulley tears, and tendon instability. Conservative management of symptomatic LHB tendinopathy is commonly accepted as the first-line treatment. It consists of rest, nonsteroidal anti-inflammatory drugs, corticosteroid injections, and physical therapy. Biceps tenotomy and tenodesis are the most common surgical procedures to manage both isolated LHB pathology and biceps-glenoid complex tears combined with rotator c

"Tenomalacia": a new sonographic sign of tendinopathy?

The purpose of this study was to assess tendon compressibility with sonography in extensor tendinopathy and in asymptomatic extensor tendons of the elbow. Sonography of both elbows was performed in eight patients with a clinical diagnosis of unilateral lateral epicondylitis. Tendons were assessed for compressibility by measuring their thickness before and after compression with the transducer. The same manoeuvre was performed while tendon vascularity was assessed with colour Doppler. All eight cases showed increased compressibility of the common extensor tendon on the painful side compared to

Hypoxic alterations of tenocytes in degenerative tendinopathy.

The authors examined 34 tendons under the electron microscope, excised within 48 h after rupture of the tendons. The tendons did not exhibit any signs of inflammation or lipomatosis, but fine-structurally marked hypoxic alterations in the tenocytes could be seen. The degenerative (hypoxic tendinopathy appears in three phases, and the hypoxic lesions of the tendon occur at multiple foci or diffusely, and the various phases can exist simultaneously.

Percutaneous Zadek Osteotomy for Insertional Achilles Tendinopathy.

The gold standard intervention for insertional Achilles tendinopathy with or without Haglund's deformity has been the open midline Achilles tendon splitting approach for many years. While this is a successful and highly vetted technique, this open hindfoot procedure has been associated with several complications and an extensive post-operative recovery. In comparison, the percutaneous Zadek osteotomy (ZO) has demonstrated safe and effective healing in patients with insertional Achilles tendinopathy with or without Haglund's deformity, with very minimal complications. Additionally, patients who

Nonsurgical Treatment Options for Insertional Achilles Tendinopathy.

Most nonoperative treatments for insertional Achilles tendinopathy (IAT) have insufficient evidence to support treatment recommendations. Exercise has the highest level of evidence supporting the ability of this treatment option to reduce IAT pain. The effects of exercise may be enhanced by a wide variety of other treatments, including soft tissue treatment, nutritional supplements, iontophoresis, education, stretching, and heel lifts. When exercise is unsuccessful, extracorporeal shock wave therapy seems to be the next best nonoperative treatment option to reduce IAT pain. After other nonoper

Revision Surgery for Failed Patellar Tendinopathy Exploration.

Chronic patellar tendinopathy affects athletes, with a higher incidence in high-impact sporting activities. It can seriously impair sports activities. Many patients respond well to conservative treatment, but about 10% of them are refractory to conservative treatment. In these cases, surgery is indicated. Multiple surgical techniques have been reported, including both open and arthroscopic procedures with good results. However, in a small percentage of patients surgery is unsuccessful. This group of patients presents a major challenge, as options are limited. We describe our surgical approach

Lateral elbow tendinopathy: Evidence of physiotherapy management.

Lateral elbow tendinopathy (LET) is a common musculoskeletal/sports injury. A plethora of physiotherapy techniques has been proposed in the management of LET. The exercise programme is the most common treatment in the management of LET. The optimal protocol of exercise programme is still unknown. The effectiveness of the exercise programme is low when it is applied as monotherapy. Therefore, exercise programme is combined with other physiotherapy modalities such as soft tissue techniques, external support, acupuncture, manual therapy and electrotherapy, in the treatment of LET. Future research

Noninsertional Achilles tendinopathy treated with gastrocnemius lengthening.

BACKGROUND: Surgery is frequently considered an option for refractory, symptomatic noninsertional Achilles tendinopathy. Gastrocnemius equinus can result in mechanical overload of the Achilles tendon and may be a factor in its etiology. Our hypothesis was that reducing load transmission to the Achilles tendon by gastrocnemius lengthening (Strayer procedure) may be an effective treatment. MATERIALS AND METHODS: A prospective case series of all patients with a minimum 1-year symptomatic noninsertional Achilles tendinopathy who underwent gastrocnemius lengthening was evaluated before surgery, an

Prospective randomized surgical treatments for calcifying tendinopathy.

Thirty-eight patients with chronic calcifying tendinopathy of the shoulder were randomized prospectively into two groups: 19 patients had endoscopic decompression and 19 had open decompression. Thirty-three patients (19 with open and 14 with endoscopic decompression) were available for followup. Clinical and ultrasonographic investigations were done at followup. The purpose of the current study was to evaluate prospectively the clinical and sonographic results after open decompression and after endoscopic decompression. Apportionment of age and gender was similar in both groups. The followup w

Ultrasound imaging/guidance for insertional Achilles tendinopathy.

OBJECTIVES: This observational study aimed to define a standardized sonographic approach for evaluating the elementary lesions of the tendon-bone junction (TBJ) in insertional Achilles tendinopathy (IAT). METHODS: Using high-frequency transducers, we matched the histological microarchitecture and the anatomical features of the TBJ of the Achilles tendon in patients with a clinical diagnosis of IAT. Colour/power Doppler assessments have been performed as well. RESULTS: Fifty-eight patients, with a mean age of 54 years (54.50 ± 11.72) and a gender distribution of 32 males (55.17%) and 26 femal

Insertional Achilles Tendinopathy: A Minimally Invasive Approach.

Insertional Achilles tendinopathy is a common overuse injury of the lower extremity classically treated with open debridement and tendon repair. However, wound complications and prolonged non-weightbearing are limiting factors for this treatment. Recently, a minimally invasive endoscopic debridement with double-row suture repair has emerged as an alternative, soft-tissue friendly option for operative management. The purpose of this study is to examine the benefits of and provide a technique for this procedure using the Arthrex MIS Achilles Speedbridge System (Naples, FL).

Tropical Achilles Tendinopathy: Sea Urchin Spine Injury.

Sea urchin spine injuries have been reported in the hand and foot, but there are no published cases in the Achilles tendon. We report an unusual case of Achilles tendinopathy secondary to sea urchin spine injury. The patient had Achilles tendon pain that increased over time and was worsened with weightbearing activity. His left ankle plantarflexion was limited by pain. He had received medical care 3 months earlier to remove sea urchin spines after stepping on a long-spined sea urchin. Bedside ultrasound and imaging studies revealed that there were foreign bodies related to sea urchin spines on
