Tendon transfer surgery to restore fundamental wrist and hand function is made possible by the redundancy that exists among the actions of our upper-extremity musculature. Potential donors for transfer are those muscles with adequate power to motor the recipient tendon, similar tendon excursion to the recipient, and function in phase with the recipient. Resolution of wound healing, union of fractures, and mobilization of stiff joints are prerequisites for a functioning tendon transfer. Injuries to the radial, median, and ulnar nerves occur above (high nerve injury) and below the elbow (low ner

BACKGROUND: Flexor tendon injuries are difficult to treat and the postoperative results have traditionally been poor.
MATERIAL AND METHODS: The article presents postoperative results (11-22 months) after surgical treatment of injured flexor tendons at Stavanger University Hospital. 32 injuries in 30 patients met the inclusion criteria; 23 of these patients responded to letters and telephone contacts. A physician and a hand therapist followed up 20 of the patients with a clinical examination and three were just interviewed by telephone. The results were evaluated according to joint movement, g

Although the anatomy of the biceps tendon and the restraining structures within the rotator interval are well defined, biceps function is not clearly understood. Biceps pathology is often associated with rotator cuff disease. Although careful clinical examinations along with diagnostic testing can accurately identify patients with biceps pathology, arthroscopy is extremely valuable in the diagnosis and treatment of biceps pathology. Surgical treatment options for biceps pathology include decompression, débridement, tenotomy, and tenodesis. Several factors must be considered in this decision. T

LEARNING OBJECTIVES: After reviewing the article, the participant should be able to: (1) Describe the anatomy of the extensor tendons at the level of the forearm, wrist, hand, and fingers. (2) Recognize variations in the anatomy. (3) Master the hand examination and define the relevant findings in acute injuries of the extensor tendon(s). (4) Delineate the techniques for extensor repair in both acute and secondary (delayed) management.
SUMMARY: Extension of the fingers is an intricate process that reflects the combined action of two independent systems. The interossei and lumbricals constitute

We retrospectively reviewed 445 patients with neglected tendon and nerve injuries treated between 1983 and 2003. Of these patients, 355 patients (447 tendons) had flexor tendon injuries, 62 patients (84 tendons) had extensor tendon injuries, and 28 patients had isolated nerve injuries. (In addition to these, 48 patients had nerve injuries associated with flexor tendon injuries; making a total of 76 patients with nerve injuries.) Most of the patients presented for treatment from 2 months to 2 years after the injury. The majority (73%) of patients with flexor tendon injuries were treated by free

BACKGROUND: Muscle and tendon injuries are among the commonest football injuries and can be very frustrating to treat because of the high risk of recurrence.
OBJECTIVE: This article aims to highlight the optimal management of, and risk factor for re-injury of, the common soft tissue injuries in football players.
DISCUSSION: The high risk of recurrence of soft tissue injuries is because most football players return to play before complete healing of the injury. For hamstring and quadriceps strains and thigh haematomas, early return is often a sensible strategy. Calf strains in older players a

The treatment options for the soft-tissue mallet finger, in both its acute and chronic forms, continue to generate some degree of controversy. Priority always should be given to nonoperative management of these injuries. This translates into a 6- to 8-week period of uninterrupted immobilization of the DIP joint with an external splint. Splinting has been shown to be highly effective, safe, and reproducible for both acute and chronic lesions. Even in the presence of an open injury, the value of splinting must be appreciated by the practitioner. The conversion of an acute closed soft-tissue inju

Diagnosis, aetiology and therapy of injuries to the extensor mechanism of the knee joint are presented. 22 cases of ruptures and discisions of the lig. patellae or the quadriceps tendon could be reexamined. All cases but one, with operation within the first week after injury, suturing with absorbable or non absorbable material, immobilisation for 6 weeks and following physical therapy, had a good functional result. No rerupture occurred. In one case a limitation of the range of movement had to be seen afer an open discision of the lig. patellae. After transposition of the tuberositas tibiae, d

From the prior discussion, it should be apparent that the multitude of operative procedures reflects the complexity inherent in the extensor mechanism and the often unpredictable outcome of operative intervention. Large studies with detailed analysis of data are few. Despite this, several themes are apparent and need to be respected. The chronic boutonniere deformity reflects long-term derangement with secondary compensation of the extensor mechanism. The surgeons rendering treatment must have a thorough appreciation of normal anatomy as well as an understanding of the evolving pathomechanics.

In essence there are three afflictions about the ankle that result from skiing: rupture of the Achilles tendon, Achilles tendinitis, and dislocation of the peroneal tendon. The cause of Achilles tendinitis and Achilles tendon rupture is, in fact, pressure within the fascial compartment of the Achilles tendon, which is caused by a swollen distal soleus muscle, occluding the circulation, and thereby producing avascular necrosis with subsequent tendinitis yielding to rupture. Four types of peroneal tendon dislocations and related problems are presented.

Due to their superficial course, the extensor tendons are frequently lacerated over the dorsum of the hand and fingers. Excellent functional results are obtained in repairs of simple tendon lacerations. 'Open' mallet lacerations over the distal IP joint or involving the central extensor slip over the proximal IP joint require more precise suturing methods. More proximal extensor tendon divisions near the wrist involve dissection of the retracted finger extensors or long thumb extensor in the distal forearm and more formal tendon repairs, including a possible tendon transfer to the thumb. 'Clos

Extensor mechanism injuries are frequently encountered in athletes and can lead to permanent disability or deformity if not promptly and properly treated. This article reviews basic anatomy, and then discusses mallet finger injuries, boutonniere deformity, and sagittal band rupture. Once treatment has begun, return to sport is highly variable because of the varied needs of each athlete and where they fall on the spectrum of disease. As such, each athlete must be carefully evaluated and closely followed to ensure a safe, prompt, and judicious return to athletic pursuits.

BACKGROUND: Flexor tendon injuries remain a challenge for hand surgeons. The constraints of the pulley system with the need for a strong repair require a delicate balance. Suture repair has been the gold standard. The 4-strand locked cruciate stitch is the technique of choice for many. This can be laborious and technically difficult. Tendon stapler devices (TSDs) aim to provide fast, strong, low-profile repairs. Industry data have shown stronger TSD repairs compared with suture repair techniques. We aim to further explore the relative strengths of TSD and 4-strand locked cruciate repairs in ca

BACKGROUND: While quadriceps tendon (QT) and patellar tendon (PT) tears are relatively rare in the National Football League (NFL), the impact they can have on a player's season and career is significant. Despite the importance of extensor tendon injuries within the NFL, there is a lack of recent literature describing their risk factors, incidence, and impact on performance.
PURPOSES: To determine the incidence and impact of QT and PT injuries in the NFL between the 2009-2010 and 2022-2023 seasons and to explore the impact that player and injury characteristics have on injury risk, return to p

Only 1% of all tendon injuries affect the triceps tendon, making triceps ruptures very rare. An acute rupture can therefore easily be missed due to a low degree of suspicion. A palpable gap, inability to flex the elbow against resistance and a positive modified Thompson test are indicative for a complete triceps tendon rupture. The mechanism of injury is most commonly a sudden eccentric muscle contraction. We present the case of a 48-year-old man with an acute complete triceps tendon rupture, after a fall from an unstable chair while performing a one arm push-up. The rupture was surgically tre

BACKGROUND: Patellar tendon injuries not amenable to primary repair present a challenging problem for surgeons and patients alike. No standard surgical technique exists for these injuries and few studies report outcomes after surgical treatment.
METHODS: A retrospective analysis was conducted for patients undergoing surgical treatment for irreparable patellar tendon tears. Patients were treated with an indirect tendon reconstruction technique using high-strength suture to set initial patellar height and hamstring autograft for biologic augmentation. Patients who underwent this procedure betwe
