Scleral suture fixation of a foldable intraocular lens without a scleral flap or groove.

OBJECTIVE: This study aims to assess the surgical and visual outcomes of a sutured intraocular lens (IOL) fixation approach performed without creating a scleral flap or tunnel in aphakic eyes without posterior capsule support. METHODS: In this retrospective study, we evaluated aphakic patients who underwent surgery using the ab externo scleral fixation technique that involved tying IOL fixation sutures on the sclera and placing the suture ends under the Tenon's capsule without opening the scleral flap. Preoperative and postoperative visual acuity, refraction values, intraocular lens centraliz

Expression of bone morphogenetic proteins (BMPs), their receptors, and activins in normal and scarred conjunctiva: role of BMP-6 and activin-A in conjunctival scarring?

Bone morphogenetic proteins (BMPs) and activins are multifunctional growth factors, which also affect wound healing and tissue fibrosis. To determine their putative role in conjunctival wound healing and scarring, we investigated the expression of various BMPs, BMP receptors, and activins in normal and scarred human conjunctival tissue and in cultured human Tenon's capsule fibroblasts on the mRNA and protein level. Messenger RNA expression of BMP-2, BMP-3, BMP-4, BMP-5, BMP-6, BMP-7, the BMP receptors type I (BMPR-IA, BMPR-IB) and II (BMPR-II), and of activin A and B was investigated by semi-q

Herniated orbital fat.

Three patients with acquired orbital fat herniation are presented. Each was felt to have a dehiscence in Tenon's capsule, allowing fat to spontaneously herniate forward. Orbital fat herniation may be confused with other lesions, such as dermolipomas. The clinical features and differential diagnosis of orbital fat prolapse are discussed.

Diagnostic and therapeutic evaluation of multiple choroidal granulomas in a patient with confirmed sarcoidosis using enhanced depth imaging optical coherence tomography.

PURPOSE: Sarcoidosis is a major cause of granulomatous uveitis but rarely manifests as multiple choroidal granulomas. This report describes the use of enhanced depth imaging optical coherence tomography (EDI-OCT) to visualize changes occurring in multiple choroidal granulomas during treatment. METHODS: The patient was a 36-year-old Japanese man with histopathologically confirmed sarcoidosis, who was examined using EDI-OCT and showed multiple yellowish-white subretinal lesions in the peripapillary region and the arcade of the right eye. RESULTS: EDI-OCT revealed homogeneous hyporeflective cho

Increased intraocular pressure after extensive conjunctival removal: a case report.

A 50-year-old woman, who had undergone extensive removal of conjunctiva on the right eye for cosmetic purposes at a local clinic 8 months prior to presentation, was referred for uncontrolled intraocular pressure (IOP) elevation (up to 38 mmHg) despite maximal medical treatment. The superior and inferior conjunctival and episcleral vessels were severely engorged and the nasal and temporal bulbar conjunctival areas were covered with an avascular epithelium. Gonioscopic examination revealed an open angle with Schlemm's canal filled with blood to 360 degrees in the right eye. Brain and orbital mag

A triple-layered closing technique using the recti muscles to augment implant coverage in eye enucleation after maxillofacial radiotherapy: A case report.

Eye enucleation (EE) of a previously irradiated orbit can lead to difficult wound closure due to the soft tissues fibrosis such as Tenon's capsule and conjunctiva. We, here, report a technique modification for EE surgery after facial radiotherapy that may decrease implant exposure by using recti muscles native tissue to reinforce the anterior implant coverage. A 71-year-old African male presented in 2003 a right orbital extranodal marginal zone B-cell lymphoma of the mucosa-associated lymphoid tissue (MALT) type. He was treated with chemotherapy followed by whole orbital radiotherapy. His right

Decreased scleral Wnt5ahi fibroblasts exacerbate myopia progression by disrupting extracellular matrix homeostasis in mice.

Myopia is a common refractive error with high prevalence; its pathogenesis is poorly understood. Scleral single-cell RNA sequencing is used to determine whether there is an association between phenotypic heterogeneity of scleral fibroblasts and form-deprivation myopia in male mice. The number of unique Wnt5a-positive scleral fibroblasts is markedly lower in the form-deprived eyes, specifically in the temporal inner peripapillary sclera. Inhibition of Wnt5a expression by injection of shWnt5a-AAV within Tenon's capsule causes increased myopia progression, while decreasing COL1A1 protein content

Long-term effects of mitomycin on filtering blebs. Lack of fibrovascular proliferative response following severe inflammation.

BACKGROUND AND OBJECTIVE: The use of antimetabolites, such as fluorouracil and mitomycin, enhances the success rate of filtering surgery, especially in eyes at high risk for failure, and increases the likelihood of a thin, avascular filtering bleb. In addition, mitomycin may cause long-term inhibition of the fibroblast's ability to proliferate in the conjunctiva and Tenon's capsule. Preoperative and postoperative inflammation frequently contributes to scarring of filtering blebs. The purpose of this study is to evaluate the effect of intraoperative mitomycin use on the survival of filtering bl

A partial-thickness 'scleral belt loop' for MicroShunt implant.

The purpose of this article is to introduce a new surgical technique for the PreserFloTM MicroShunt implant. This technique involves creating a partial-thickness scleral belt loop before repositioning the Tenon's capsule and the conjunctiva toward the limbus. The distal end of the device is passed under the lamellar belt of the sclera, but the tip remains open and not covered by the loop. With this surgical technique, the MicroShunt adheres perfectly to the scleral profile of all eyes. As a result, the device's end tip does not stress the Tenon's/conjunctival flap during postoperative eye move

Thiel cadaver eye as a training model for sub-Tenon's blocks: a feasibility study.

BACKGROUND: Regional anaesthesia education, especially for ocular procedures, necessitates reliable surgical training models. While cadaveric models offer anatomical fidelity, conventional embalming methods may compromise tissue integrity. We aimed to assess the effectiveness of Thiel cadavers for training in sub-Tenon's blocks by evaluating ocular tissues and measuring insertion forces. METHODS: Experimental design, using twenty eyes from ten Thiel cadaver heads. These cadavers were specifically prepared to test the administration of sub-Tenon's blocks. The research was conducted in a contro

One-Year Outcome Predictors of Strabismus Surgery from Anterior Segment Optical Coherence Tomography with Multiple B-Scan Averaging.

Strabismologists are eager to identify preoperative or intraoperative strabismus surgery outcome predictors because of the variable effects in each patient. Conjunctival closure position recession after rectus muscle recession is effective for correcting large angle strabismus. The elasticity of the conjunctiva and Tenon's capsule is important for strabismus surgery management. In this longitudinal study, we evaluated the prognosis of conjunctiva and Tenon's capsule thickness (CTT) near the limbus 1 year after strabismus surgery with a limbal conjunctival incision using swept-source anterior s
