Is percutaneous proximal gracilis tenotomy as effective and safe as the open procedure?

PURPOSE: There is currently an increasing trend for percutaneous surgical interventions mainly in children with cerebral palsy (CP). The purpose of this study was to evaluate the effectiveness and safety of percutaneous proximal gracilis tenotomy (PPGT) in children with CP scheduled for hip adductor tenotomy. METHODS: This is a prospective study of 59 hips in 31 consecutive patients with CP scheduled for hip adductor tenotomy in the setting of multilevel tenotomies or hip osteotomy (femoral or Dega). A pediatric orthopedic surgeon conducted a percutaneous adductor longus and gracilis tenotomy

Chevron osteotomy with lateral release and adductor tenotomy for hallux valgus.

BACKGROUND: Distal chevron osteotomy is a procedure widely performed for the surgical treatment of painful hallux valgus. The risks and benefits of a lateral capsular release and adductor tenotomy combined with chevron osteotomy are still debated. The aim of our study was to report the clinical and radiographic outcomes of this combined procedure in mild and moderate incongruent bunion deformities, with a hallux valgus angle (HVA) up to 40 degrees and an intermetatarsal angle (IMA) up to 20 degrees. MATERIALS AND METHODS: Forty-two patients (52 feet) who consecutively underwent chevron osteot

Open adductor tenotomy in the prevention of hip subluxation in cerebral palsy.

We reviewed radiographs of 76 hips in 41 patients with cerebral palsy treated with open adductor tenotomy because of hip subluxation. The majority of patients suffered from spastic diplegia. The aim of our study was to evaluate the effect of adductor release on hip subluxation. We measured the centre-edge angle and migration percentage pre-operatively and 1 and 3 years post-operatively. We considered the results according to patients' age at time of surgery (younger or older than 4 years of age) and ability to walk. Children younger than 4 years of age had better results than children older th

Ultrasound-guided percutaneous longitudinal tenotomy for the management of patellar tendinopathy.

Thirty-eight athletes with unilateral patellar tendinopathy (17 with a tendinopathy of the main body of the tendon, and 21 with an insertional tendinopathy) underwent ultrasound-guided multiple percutaneous longitudinal tenotomy under local anaesthetic infiltration after failure of conservative management. Thirty-four patients were reviewed at least 24 months after the operation. Sixteen patients were rated excellent, nine good, eight fair, and five poor. Nine of the 13 patients with a fair or poor result had an insertional tendinopathy, and eight of them underwent a formal exploration of the

[Tenotomy of the psoas muscle in slow reduction of congenital hip dislocations].

PURPOSE OF THE STUDY: The aim of this study was to relate the efficacy of cutting the psoas tendon in case of high congenital hip dislocation treated by slow reduction according to Somerville-Petit. MATERIAL: Twenty psoas tenotomies were realised between 1982 and 1992. The average age was 11 months 15 days old, they were 15 girls and 5 boys. Among them, only 9 patients had never been treated. METHOD: By a short surgical approach close to the lateral lip of Scarpa's triangle, we cut the psoas tendon at the musculotendinous junction doing this suppresses the obstacle that interferes with the f

Posterior tenotomy of the superior oblique at the scleral insertion for A-pattern deviations.

A posterior (two-thirds) tenotomy of the superior oblique at the scleral insertion spares the anterior fibers whose action is primarily torsional and can avoid some complications such as cyclovertical diplopia or torticollis, which may be induced by a superior oblique tenotomy or a recession for A-pattern deviations associated with superior oblique overaction (SOOA). A retrospective review was performed of 11 cases with A-pattern, aged 5 to 51 years, who underwent the posterior tenotomy. The average preoperative A-pattern was 18.3 prism diopters and the posterior tenotomy resulted in 16.3 pris

Effects of tenotomy of the tensor tympani muscle on the acoustic reflex in dogs.

The acoustic reflex (AR) was recorded from 12 healthy mixed-breed dogs. Latency and amplitude were measured from ipsilateral and contralateral AR at stimulus frequencies of 1 and 2 kHz and intensities of 70 to 110 dB sound pressure level for ipsilateral AR and 70 to 120 dB hearing level for contralateral AR. Mean latencies for ipsilateral and contralateral AR were between 33.46 and 206.10 ms and between 45.26 and 180.89 ms, respectively, and amplitudes were between 0.14 and 1.79 cm3 and between 0.31 and 1.86 cm3 of air, respectively. Stimulus frequencies and intensities had significant effects

Open flexor tenotomy for hammer toes and curly toes in childhood.

Sixty-two children were reviewed between 3 and 14 years (average 9.8 years) after flexor tenotomy for curly toes or hammer toes. No patients were aware of loss of flexor power in the toes. In only 5% of 188 toes was the operation unsuccessful. When the cause of failure was identifiable it proved to be that the scar crossed one or more flexor creases. None of the operated toes had an abnormally extended posture; only one toe was stiff and this resulted from tethering by a scar. It is concluded that open flexor tenotomy is an effective method for correcting curly toes and hammer toes in childhoo

Ruptured pseudoanuerysm of the posterial tibial artery after percutaneous Achilles tenotomy.

Congenital talipes equinovarus (CTEV), or clubfoot, is the the most common encountered musculoskeletal defect encountered at birth. Most cases present as an isolated deformity, with up to half of them presenting with bilateral deformity. CTEV have also been reported to present as part of syndromic phenomena. Dr Igancio Ponseti proposed a serial casting programme to correct the foot's cavus, forefoot adduction, varus and equinus. Up to 90% of infants will require a tendo-achilles (TA) tenotomy for the persisting equinus deformity. TA tenotomy is deemed a relatively safe procedure, with the most

Propofol sedation for infants with idiopathic clubfoot undergoing percutaneous tendoachilles tenotomy.

OBJECTIVES: To determine whether percutaneous tenotomy of the Achilles tendon in infants with idiopathic clubfeet can be safely performed under propofol sedation. BACKGROUND: Many orthopaedic surgeons prefer to do a tenotomy under general anesthesia. At our institution, we have been using 2 different methods of induction and maintenance of anesthesia: one method using combined propofol and sevoflurane and various types of airway management, and the other using only propofol with facemask. METHODS: We reviewed the medical records of all patients less than 1 year of age with idiopathic clubfoo

Fractional Fowler tenotomy for chronic mallet finger: a cadaveric biomechanical study.

PURPOSE: The Fowler tenotomy, adjusting the balance of the extensor mechanism by central slip and lateral band detachment, is a common surgical technique for chronic mallet finger. The purpose of this study was to determine how much tendon to detach from the middle phalanx by measuring the extensor lag of the distal interphalangeal (DIP) joint following the procedure and to quantify how often a boutonniere deformity occurred as a consequence of the procedure. METHODS: Sixteen fingers were obtained from 8 fresh-frozen cadaver hands. We made mallet finger deformity models by terminal tendon elo

Mini-tenotomy procedure to correct diplopia associated with small-angle strabismus.

PURPOSE: The mini-tenotomy is a novel minimally invasive surgical technique that weakens rectus muscles to treat small-angle strabismus. The mini-tenotomy is an alternative to the standard rectus muscle recession that requires hooking the muscle, suturing the muscle, removing the muscle from sclera, and reattaching the muscle to sclera. METHODS: This is a retrospective chart review of outcomes of the mini-tenotomy procedure on 15 consecutive adult patients. A central tenotomy of 3 mm was performed cutting through intact conjunctiva using a blunt Westcott scissors. RESULTS: Preoperatively 6 p

[An experimental pathological study of anterior segment ischemia following recti tenotomy].

The authors observed the anterior segment changes following recti tenotomy in 34 adult dog eyes. 16 eyes with tenotomy of any 2 recti showed no discernible signs of anterior segment ischemia (ASI). 18 eyes had tenotomy of 3 recti in various combinations, and 5 of 9 eyes (55.6%) with tenotomy of superior, inferior and lateral recti manifested serious ASI. The pathological characteristics of ASI were degeneration, necrosis and exfoliation of epithelial cells, inflammation of anterior segment tissues and cell membrane structural damages. The authors pointed out that simultaneous tenotomy of super

A biomechanical comparison of subscapularis tenotomy repair techniques for stemless shoulder arthroplasty.

BACKGROUND: One of the leading challenges for surgeons shifting to stemless anatomic total shoulder arthroplasty (TSA) is subscapularis repair. In the available literature reporting outcomes after stemless TSA, subscapularis tenotomy with side-to-side repair is the most common technique despite some concerns regarding this technique in the biomechanical and clinical literature. Accordingly, this study investigated subscapularis tenotomy repair with stemless TSA with 2 primary objectives: (1) to evaluate the subscapularis tendon dimensions with reference to subscapularis tenotomy to determine t

Extended Ponseti method for failed tenotomy in idiopathic clubfeet: a pilot study.

We evaluated the outcome of a new protocol of an extended Ponseti method in the management of idiopathic club foot with residual equinus following failed Achilles tenotomy. We also compared the failed with a successful tenotomy group to analyze the parameters for failure. The Ponseti technique-treated idiopathic club foot patients with failed percutaneous Achilles tenotomy (failure to achieve <15° dorsiflexion) were treated by continued stretching casts, with a weekly change for a further 3 weeks. Final dorsiflexion more than 15° if achieved with the above protocol was recorded as a success. T
