Residential rehabilitation for drug users: a review of 13 months' intake to a therapeutic community.

BACKGROUND: Residential rehabilitation based on 'therapeutic community' treatment for drug users is a treatment option which is attractive to GPs and others referring drug users for treatment. Whilst there is evidence that maintenance-based programmes for drug users are effective, there have been fewer attempts to evaluate the effectiveness of abstinence-based programmes which are relatively more intensive and expensive interventions. OBJECTIVE: This paper reports and evaluates the outcomes for 13 months' intake of 138 drug users to a residential community. METHODS: We carried out a retrospe

Optical Control of Therapeutic Synthetic Cells: Controlling Biomolecule Synthesis, Activation, and Communication.

Synthetic cells are emerging as promising platforms that combine cell-free biomolecule synthesis with the programmability of bottom-up engineered compartments. By decoupling biomolecule production from cellular growth and evolutionary constraints, synthetic cells enable the controlled generation of proteins, nucleic acids, peptides, and signaling molecules within defined artificial environments. However, many envisioned biomedical applications require precise regulation over biomolecule synthesis, activation, transport, release, and communication. In this Review, we examine how light can provi

Targeting the nervous system: Mechanistic insights into neuro-tumor communication and therapeutic opportunities.

The nervous system has emerged as an active regulator of the tumor ecosystem. Neural inputs from the central, peripheral, and enteric nervous systems influence tumor initiation, progression, metastasis, and therapeutic response through innervation and signal transmission. In addition to regulating tumor cell states, neural signaling reshapes stromal and immune compartments, promoting stemness, invasion, angiogenesis, metabolic adaptation, and immune evasion. Tumors, in turn, actively remodel their neural niche through neurogenesis, axonal remodeling, and tumor-nerve feedforward circuits. The b

Exosomes as Cellular Communicators and Therapeutic Agents in Orthopedic Diseases: From Mechanisms to Intervention.

Exosome-mediated intercellular communication has become a critical mechanism in the pathogenesis, progression, and regenerative repair of orthopedic diseases. By delivering bioactive molecules, exosomes dynamically regulate bone remodeling, cartilage homeostasis, and inflammatory responses-processes that are commonly disrupted in conditions such as osteoporosis, osteoarthritis, and bone non-union. Current therapeutic approaches often fail to achieve complete tissue repair or reverse disease progression, representing a major clinical challenge in orthopedics. This systematic review examines how

Vesicular Communication in the Bone-Muscle Unit: Physiological Functions, Aging, and Therapeutic Potential.

Extracellular vesicles (EVs) have emerged as fundamental pillars of intercellular communication, acting as primary mediators of the bidirectional biochemical crosstalk within the integrated bone-muscle unit. This review provides a comprehensive synthesis of EV-mediated signaling across the bone-muscle axis, offering a side-by-side mapping of vesicular biogenesis, cargo composition, and functional roles in both tissues. Under physiological conditions, skeletal muscle- and bone-derived EVs orchestrate tissue homeostasis, adaptations to physical exercise, myogenesis, and bone remodeling by transf

Exosomal Communication Between the Tumor Microenvironment and Innate Immunity and Its Therapeutic Application.

Exosomes, which are well-known nanoscale extracellular vesicles, are multifunctional biomaterials derived from endosomes and perform various functions. The exosome is a critical material in cell-cell communication. In addition, it regulates the pathophysiological conditions of the tumor microenvironment in particular. In the tumor microenvironment, exosomes play a controversial role in supporting or killing cancer by conveying biomaterials derived from parent cells. Innate immunity is a crucial component of the host defense mechanism, as it prevents foreign substances, such as viruses and othe

Quorum sensing in oral multispecies biofilms: from interkingdom communication to therapeutic targeting.

Oral biofilms are polymicrobial ecosystems where quorum sensing (QS) governs community assembly, virulence and homeostasis. This narrative review synthesizes evidence on QS pathways in key periodontal and cariogenic pathogens, emphasizing interspecies synergy, cross-kingdom interactions, and disease progression. We surveyed English-language publications up to 2026 across PubMed, Web of Science, and Scopus. Our analysis reveals three major QS circuits: the widely conserved LuxS/AI-2 system facilitating cooperative and competitive behaviors, such as Fusobacterium nucleatum-red complex coaggregat

Vagus nerve and brain-gut communication in neurodegenerative diseases: Mechanism and therapeutic perspectives.

Neurodegenerative diseases (NDs) are characterized by degenerative nerve damage, and available therapeutic drugs cannot prevent or slow down their progression. The pathogenesis of NDs is related to the brain-gut axis, but the regulatory mechanisms are not yet fully understood. The vagus nerve, the main neural pathway of the brain-gut axis, plays a crucial role in the occurrence and development of NDs. This study summarizes the molecular mechanisms by which the vagus nerve regulates intestinal immunity and neuroinflammatory homeostasis of NDs to improve movement disorders and mental disorders.
