Atlas of Fshr expression from novel reporter mice.

The FSH-FSHR pathway has been considered an essential regulator in reproductive development and fertility. But there has been emerging evidence of FSHR expression in extragonadal organs. This poses new questions and long-term debates regarding the physiological role of the FSH-FSHR, and underscores the need for reliable, in vivo analysis of FSHR expression in animal models. However, conventional methods have proven insufficient for examining FSHR expression due to several limitations. To address this challenge, we developed Fshr-ZsGreen reporter mice under the control of Fshr endogenous promot

Split cranial bone grafting in children younger than 3 years old: debunking a surgical myth.

BACKGROUND: In cranioplasty patients split cranial bone provides excellent structural support and fundamentally "replaces like with like." However, traditional teaching in craniofacial surgery is that cranial bone cannot be split before the age of 3 years because of the lack of diploic space. The authors have found this not to be the case and describe their experience with splitting cranial bone in children with craniosynostosis younger than 3 years. METHODS: The authors completed a retrospective review of 418 cranioplasties performed between 1997 and 2013 by a single surgeon on patients youn

Hypnotherapy attitudes in Malaysian patients with major depressive disorder: a mixed-methods study.

Despite growing evidence for hypnotherapy as an adjunctive intervention for major depressive disorder (MDD), uptake remains limited, partly due to patient misconceptions and stigma. Attitudinal research in non-Western clinical populations remains scarce. This study examined attitudes toward hypnotherapy among Malaysian adults with MDD, the influence of demographic and clinical variables on those attitudes, and the psychosocial factors underlying them. A mixed-methods design was used. Sixty-two outpatients with a DSM-5-TR diagnosis of MDD completed the Malay-translated Attitudes Toward Hypnosis

Is there a continued need for behavioral pharmacology in the discovery and development of medicines?

Since their introduction in the 1950s by Peter Dews, behavioral pharmacology laboratories have collected experimental data for discovery, approval, and regulation of many new and improved therapeutic agents across a host of therapeutic domains. The ethical use of animals in research requires evaluation of the value of animal data to justify their use. The present commentary provides a discussion of the risk-benefit analysis of the use of behavioral methods in drug discovery and drug development using antidepressants as an example. It is concluded that there are some misconceptions both within

Investigational agents for the management of Huntington's disease.

An inherited, chronic progressive, neurodegenerative disorder is Huntington's disease, characterized by motor, cognitive, and psychiatric symptoms. Predictive genetic testing allows earlier diagnosis and identification of gene carriers for Huntington's disease. These individuals are ideal candidates for testing of therapeutic interventions for disease modification. Areas covered: According to queries in Pubmed, Embase and clinical register databases, research and clinical studies emerge on symptomatic and neuroprotective therapies in Huntington's disease. This review discusses novel agents for

Essential Truths for the Care and Management of Moderate-to-Severe Psoriasis.

Psoriasis is a systemic inflammatory disease. Effective management requires treatment with agents targeting inflammation in skin, joints, and other tissues. Biologics for psoriasis are directed at more specific targets, have a better safety profile, are better tolerated, and are more effective than conventional systemic agents. Despite these advances, many patients with psoriasis remain undertreated, and overall patient satisfaction remains low. The dichotomy between ideal therapeutic outcomes and suboptimal outcomes (which are currently commonplace) is likely largely due to misperceptions abo

Acute pain management for patients under opioid maintenance treatment: what physicians do in emergency departments?

The aim of this study was to analyze the current practices on acute pain management of patients under opioid maintenance treatment (OMT), that is, buprenorphine or methadone. A total of 706 physicians were solicited through a national network to answer a survey about pain perception and analgesic strategies. Among the prescribers, 323 (46%) answered the survey: 131 (40%) physicians estimated that patients under OMT when exposed to an acute painful event feel more pain than other patients and 170 (53%) estimated that the patients felt the same amount of pain. Use of WHO step 1 analgesics was re

miR-33 in cardiometabolic diseases: lessons learned from novel animal models and approaches.

miRNAs have emerged as critical regulators of nearly all biologic processes and important therapeutic targets for numerous diseases. However, despite the tremendous progress that has been made in this field, many misconceptions remain among much of the broader scientific community about the manner in which miRNAs function. In this review, we focus on miR-33, one of the most extensively studied miRNAs, as an example, to highlight many of the advances that have been made in the miRNA field and the hurdles that must be cleared to promote the development of miRNA-based therapies. We discuss how th

Diagnosis and management of rheumatic diseases in older patients.

The young and the old move differently, yet there are standards of normality for each. Chronic diseases of the joints, with their concomitant stiffness and pain, cause more fear of an impending crippling disability and loss of life space and content than actual pain and stiffness. Thus, a prime goal of physicians who treat patients who have chronic musculoskeletal diseases is to allay this fear by correcting misconceptions about arthritic diseases. Drug therapy can relieve the pain and stiffness and control the inflammation that causes these symptoms. Important aspects of rheumatoid arthritis,

Portrayals of Medical Research in Prime-Time Dramas: A Content Analysis.

BACKGROUND: Television (TV) dramas are a significant source of information about health and medical research for lay audiences, blending accurate depictions with dramatic distortions that can shape beliefs about how research is conducted and governed. This study systematically analyzed selected U.S. television dramas to characterize how medical research is portrayed, including the frequency, context, accuracy, and ethical framing of research-related storylines. METHODS: A systematic content analysis was conducted of episodes from prime-time legal and medical dramas aired over the past 20 year

Nerve Transfers in the Upper Extremity: A Practical User's Guide.

Nerve injuries above the elbow are associated with a poor prognosis, even with prompt repair and appropriate rehabilitation. The past 2 decades have seen the development of numerous nerve transfer techniques, by which a denervated peripheral target is reinnervated by a healthy donor nerve. Nerve transfers are indicated in proximal brachial plexus injuries where grafting is not possible or in proximal injuries of peripheral nerves with long reinnervation distances. Nerve transfers represent a revolution in peripheral nerve surgery and offer the potential for superior functional recovery in seve

Rapid Ethical Appraisal: A tool to design a contextualized consent process for a genetic study of podoconiosis in Ethiopia.

BACKGROUND: Obtaining valid informed consent from research participants in low-income settings can be difficult, partly due to participants' limited familiarity with scientific research concepts. The situation is complicated when conducting genomic research on a stigmatizing familial disease. METHODS: We used Rapid Ethical Appraisal as a tool to assess local factors that were barriers to obtaining valid informed consent prior to conducting a genetic study of podoconiosis (non-filarial elephantiasis) in two Zones of Ethiopia. The tool included in-depth interviews and focus group discussions wi

Varicosities of the lower extremity, new approaches: cosmetic or therapeutic needs?

Varicose veins of the lower extremity (VVLE) are a frequently encountered vascular disorder in the general population. The general view that VVLE are a non-serious disease with primarily aesthetic impact is a common misconception, as the disease can have a significant negative impact on generic and disease-specific quality of life. Further, VVLE may be associated with potentially threatening clinical conditions, such as chronic venous ulceration, venous thromboembolism and haemorrhage from ruptured varicose veins. In the case of symptomatic varicose veins, in the presence of advanced lower lim

"If People Know, They Will Think Something Is Wrong with Me": An Ethnography of Sickle Cell Hemoglobinopathy, Stigma, and Health Disparities in Tribal Odisha, India.

Sickle cell hemoglobinopathy is not merely a genetic disorder but a deeply embedded socio-cultural phenomenon in the remote regions of Odisha. Drawing on long-term ethnographic fieldwork in Koraput and surrounding districts, this study explores the lived experiences of individuals with sickle cell trait (SCT) and sickle cell disease (SCD), focusing on social discrimination, medical uncertainty, and healthcare accessibility. The findings reveal reluctance toward screening, particularly among unmarried women, due to fears of marital exclusion and family stigma. Misconceptions about SCT, includin

Mystery and misery of locally-delivered drug therapy in periodontics. Historical concepts and current state.

The primary objective is to critically analyze approaches to local therapy for periodontal disease, particularly in light of its well-established systemic implications. We specifically address common misconceptions and overestimations regarding the potential of local treatments in contrast to their actual clinical effectiveness. Historically, the therapeutic approach to periodontitis has concentrated on local intervention within periodontal pockets. Local drug delivery systems show potential for targeting periodontal infections with high drug concentrations while minimizing systemic side effec

Feedback of individual genetic and genomics research results: A qualitative study involving grassroots communities in Uganda.

BACKGROUND: Genetics and genomics research (GGR) is associated with several challenges including, but not limited to, methods and implications of sharing research findings with participants and their family members, issues of confidentiality, and ownership of data obtained from samples. Additionally, GGR holds significant potential risk for social and psychological harms. Considerable research has been conducted globally, and has advanced the debate on return of genetic and genomics testing results. However, such investigations are limited in the African setting, including Uganda where researc
