Clinical outcome of transthoracic esophagectomy with thoracic duct resection: Number of dissected lymph node and distribution of lymph node metastasis around the thoracic duct.

The number of dissected lymph nodes (LNs), surgical outcomes, and postoperative recurrence-free survival (RFS) were compared between thoracic duct (TD)-preserved and TD-resected groups. The distribution of metastasis in LNs around TD (TDLN) was reviewed. Transthoracic esophagectomy (TTE) with TD resection for esophageal cancer patients has been one of the standard procedures. Because the adipose tissue surrounding the TD contains LNs, TD resection might be necessary for radical LN dissection. However, few studies have investigated the oncological outcome of TTE with TD resection. Two hundred f

Nutritional and prognostic effects of thoracic duct obliteration during esophagectomy.

Thoracic duct obliteration (TDO), either ligation or resection, is performed during esophagectomy to enhance oncological radicality or manage chylothorax. However, its nutritional and oncological effects are unclear. We retrospectively analyzed patients undergoing esophagectomy for esophageal cancer at our institution between January 2016 and May 2024. We assessed laboratory parameters and computed tomography-derived body composition indices from diagnosis to 9-12 months postoperatively (15-25 months for laboratory data). We also analyzed nutritional indices, overall survival (OS), and recurre

Thoracic duct cyst of posterior mediastinum: a "challenging" differential diagnosis.

Thoracic duct cysts of the mediastinum are extremely rare entities and their pathogenesis still remains unknown. Imaging methods are not specific and show a cystic mass, however the real nature of the lesion is confirmed only with the help of histopathological examination after surgical excision. Here, we present a case of thoracic cyst in a 28-year-old female, lining in posterior lower mediastinum. The cyst was removed by video-assisted thoracic surgery (VATS) and the histopathological findings were that of thoracic duct cyst. Through this case, we propose an ideal surgical approach and diagn

Pancreaticopleural fistula: a rare thoracic complication of pancreatic duct disruption.

Pancreaticopleural fistula resulting in a chronic pleural effusion is a rare complication of pancreatic duct disruption. We describe the presentation and management of 2 children with pancreaticopleural fistulas. Pleural fluid amylase concentration and contrast computed tomography were sufficient to establish the diagnosis in both cases. The initial management of these fistulas should be conservative, by tube thoracostomy and suppression of pancreatic secretion. Operative treatment is necessary for those who fail to resolve. Complete diversion of the pancreatic juice into the gastrointestinal

Thoracoscopic pericardial window creation and thoracic duct ligation in neonates.

OBJECTIVE: We describe 2 newborn infants with persistent pericardial effusion treated with thoracoscopic pericardial window and thoracic duct ligation. METHODS: Patient 1 was a premature female newborn who presented with severe cardiac anomalies, including dextrocardia. She was treated with pulmonary artery handing and pacemaker placement for complete cardiac block. Postoperatively, she developed pericarditis with persistent symptomatic pericardial effusion. She did not improve despite pericardial drain placement. She was treated with a thoracoscopic pericardial window. Patient 2 was a newbor

Lymphocyte involvement in rheumatoid arthritis. Studies during thoracic duct drainage.

Grip strength, ring size, duration of morning stiffness, and the number of tender joints improved significantly in 9 patients with severe rheumatoid arthritis during prolonged continuous removal of thoracic duct lymphocytes through a surgical fistula. There was no improvement in 4 subjects in whom surgery failed to establish satisfactory lymph drainage. Reinfusion of unlabeled or 51Cr-labeled autologous lymphocytes resulted in transient exacerbation of disease activity in 3 subjects. Following reinfusion, some 51Cr-labeled lymphocytes could be found in the inflamed synovium and synovial fluid

[Mediastinal Thoracic Duct Cyst Under Thoracoscopic Surgery:Report of a Case].

A 58-year-old woman was pointed out an abnormal electrocardiogram during a physical examination. A chest computed tomography( CT) scan revealed a 50-mm-sized cystic mass in the posterior mediastinum near the tracheal bifurcation. The mass exhibited growth during follow-up imaging, leading to a surgical resection. The tumor appeared as a smooth mass lesion with milky white and turbid internal content, suggestive of a chyle. The presence of a contiguous lymphatic duct and positive CD31 staining on histopathological examination confirmed the diagnosis of a thoracic duct cyst. Thoracic duct cysts

Fluorescence-Guided Thoracic Duct Dissection in Robotic en Bloc Esophagectomy.

En bloc resection of the thoracic duct compartment enhances adequate lymph node removal and may improve oncologic outcomes in esophagectomy for malignant esophageal diseases. However, it also increases the risk of postoperative chylothorax, with a reported incidence of 5% to 20%. This report describes a technique that facilitates intraoperative identification of the thoracic duct, as well as proximal and distal ligation, during robot-assisted esophagectomy by lymphangiography-guided injection of indocyanine green in the right groin in a patient in the left lateral position. This approach can b

Lymphatic Interventional Treatment for Chyluria via Retrograde Thoracic Duct Access.

Chyluria is the leakage of intestinal lymph (chyle) into the urine. Novel lymphatic intervention techniques, such as interstitial lymphatic embolization, proved to be a useful treatment option for chyluria. However, one of the challenges of this approach is the difficulty in identifying connections between the lymphatic system and kidney collecting system. Here, embolization of the abnormal lymphatic connection through retrograde thoracic duct access in 3 chyluria patients is introduced.

Thoracic duct decompression: An idea whose time has come - again.

[Editorial] Thoracic duct decompression (TDD) is an idea first proposed and applied as a novel therapeutic strategy by lymphologists in the 1960's. TDD is recently being reexamined and, in selected patients with portal hypertension from hepatic cirrhosis or with central venous hypertension from isolated right-sided heart failure, undertaken using advanced surgical and image-guided interventional radiologic approaches.

Thoracic Duct Narrowing-Innovative Technique Restraining Weight Gain in Rats.

BACKGROUND: The lymphatic system is responsible for the absorption of fats from the digestive system, conveying 60-70 % of ingested fat to the blood stream. From the anatomical point of view, all the lymphatic drainage from the lower half of the body converges in the abdomen to enter the thoracic duct. This experimental study aim was to study the result of thoracic duct narrowing (TDN), an innovative surgical technique, on weight gain restrain in high-fat diet-fed rats. METHODS: Forty-seven rats were allocated into three groups: thoracic duct narrowing ("S"-surgery), sham operation ("CS"-cont

Evaluation of thoracic duct healing after experimental laceration and transection.

Healing of the thoracic duct (TD) was evaluated clinically and histologically in six healthy dogs. A 2.5 cm longitudinal laceration of the caudal TD was created in three dogs and the caudal TD was completely transected in three other dogs. The site of the defect was identified by placing one 4-0 stainless steel suture in the tissue adjacent to the TD defect. All dogs developed a chylous effusion confirmed by biochemical analysis. By five days after surgery in dogs with TD lacerations, and by 10 days after surgery in dogs with TD transections, thoracic effusion had ceased. Lymphangiography, per

Thoracic duct-subclavian vein anastomosis in management of cirrhotic ascites.

Reanastomosis of the thoracic duct and subclavian vein has been performed in three patients with intractable ascites. Following surgery, these patients were placed on a loop-blocking diuretic plus Aldactone, and prompt diuresis ensued with gradual improvement in the ascites. Improvement in the ascites was probably the result of several factors including removal of obstruction at the upper end of the thoracic duct, mobilization of fluid in the intravascular space, improved lymphatic drainage, and diuretic effect. Complications included necessity for re-exploration in one patient and development
