Thoracic outlet syndrome (TOS) is a condition arising from compression of the subclavian vessels and/or brachial plexus. Many factors or diseases may cause compression of the neurovascular bundle at the thoracic outlet. We describe the case of a 41-year-old woman with TOS who presented with vascular venous symptoms. Chest computed tomography (CT) scan showed a cystic mass at the level of cervico-thoracic junction, located between the left subclavian artery and vein, which appeared compressed. The cystic mass was removed through a cervical approach and it was found to be a cyst arising from the

Thoracic outlet syndrome (TOS) results from compression of the neurovascular structures that pass through the thoracic outlet. TOS may be classified as neurogenic TOS (NTOS), venous TOS (VTOS), or arterial TOS (ATOS). NTOS presents with upper-extremity symptoms attributable to compression of the brachial plexus. VTOS, also known as Paget-Schröetter Syndrome, involves subclavian-axillary venous occlusion, thrombus formation, and rare embolization. ATOS results from obstruction of the subclavian artery with claudication, thrombus formation, and possible embolization. Adjunct diagnostic studies f

UNLABELLED: In this paper the surgical treatment of thoracic outlet syndrome (TOS) is presented.
MATERIAL AND METHODS: The investigation of 74 patients treated for vascular complications of the TOS the Paget-Schroetter syndrome and postthrombotic syndrome (27 patients), as well as upper limb ischemia and/or the subclavian artery aneurysm (14 patients) and for neurological complications of the TOS (33 patients) was performed. In case of venous thrombosis the treatment consisted of thrombolytic therapy and decompressive procedures that included transaxillary first rib resection and other surgic

OBJECTIVES: Arterial complications due to compression of the thoracic outlet are uncommon. The objective of this study was to review our fairly extensive experience with this problem with particular reference to its management.
METHODS: Patients entered into the Vascular Clinic database were reviewed over an 11 year period. Twenty six records were found. In 24 patients the vasculopathy was caused by a cervical rib (complete in 15) and in two by an anomaly of the first rib. In all patients the basic arteriopathy was a fibrous structure with a post-stenotic aneurysm in 13. Seventeen presented w

PURPOSE: This article reviews experience with arterial injury caused by thoracic outlet syndrome. Special emphasis is placed on the influence of athletic or work activities on the axillary-subclavian artery system and the mechanism by which the humeral head compresses the axillary artery and the circumflex humeral arterial branches.
METHODS: Retrospective review identified 34 patients (age range 13 to 67 years) treated for upper extremity symptoms or ischemic complications of thoracic outlet syndrome from 1983 to 1993. Evaluation included assessment of occupational and recreational activities

This prospective study evaluated 50 patients with thoracic outlet syndrome. Detailed history and pain scale evaluation preceded physical examination, which included provocative tests (positional and compressive) and sensory evaluation (baseline and postprovocative vibration thresholds and two-point discrimination). Only one patient had a positive nerve conduction study/electromyograph at the brachial plexus level. Thirty-two percent of the patients had a compressive anatomic abnormality as seen on a computed tomography scan. Ninety-four percent had positive provocative position and compression

Fifteen adult patients with clavicular nonunions were evaluated over a 10-year period. Fourteen of these patients were treated initially with a "figure-of-eight" clavicle strap. Seven presented with narrowing of the costoclavicular space, and symptoms of intermittent brachial plexus impingement or thoracic outlet syndrome. Two of these resulted from resection of the mid-portion of the clavicle with subsequent impingement from hypertrophy of the lateral stump. Most commonly, hypertrophic nonunion of the clavicle caused the thoracic outlet syndrome, frequently many months after the initial injur

Thoracic outlet syndrome (TOS) is a rare condition in children. Compression of the neurovascular bundle as it passes from the neck through the thorax into the arm may be produced by numerous anatomic and structural abnormalities. This case represents compression produced by spasm of the pectoralis minor muscle originating from costochondritis.

Chest outlet syndromes are characterized by pain symptomatology caused by (congenital or acquired reasons) compression of the vasculo-nervous bundle. After a brief mention of the anatomy of the question, the indications and angiographic technique are reported, special reference being made to certain positions and dynamic tests which increase symptomatology and highlight arteriographic findings. These are clearly identifiable in the pictures and offer documentary evidence of the cervical rib syndrome, scalenus syndrome, small pectoral syndrome, the hyperabduction syndrome and the costoclavicula

A patient presenting with tetany was found to have thoracic outlet obstruction. This was treated by transaxillary first rib resection, and good relief of the carpal spasm was obtained. The mechanism whereby neurovascular compression may produce tetany is discussed.

Arterial thoracic outlet syndrome (TOS) is a condition in which anatomic abnormalities in the thoracic outlet cause compression of the subclavian or, less commonly, axillary artery. Patients are usually younger and typically have an anatomic abnormality causing the compression. The condition usually goes undiagnosed until patients present with signs of acute or chronic hand or arm ischemia. Workup of this condition includes a thorough history and physical examination; chest x-ray to identify potential anatomic abnormalities; and arterial imaging, such as computed tomographic angiography or dup

Thoracic outlet syndrome (TOS) most commonly manifests in overhead athletes (e.g., baseball pitchers, swimmers, weight lifters) due to nerve compression caused by skeletal abnormalities. We present the case of a 43-year-old recreational runner with unilateral upper extremity pain while running. Vascular imaging identified an aberrant subclavian artery origin with positional compression in the absence of cervical bone anomalies confirming arterial TOS. A first rib resection and anterior scalenectomy led to symptom resolution. This case emphasizes the importance of a broad differential and compl

This retrospective study aims to clarify the mechanisms, frequency, symptoms, treatment, and outcome of post-traumatic thoracic outlet syndromes. A total of 13 patients (7 men and 6 women) with a mean age of 41 +/- 16 years were studied. Underlying injuries resulted from sports, road, and household accidents. Bone lesions were pseudarthrosis, hypertrophic callus, and luxations. Congenital anomalies were observed in five patients (38%). Vascular lesions included aneurysm of the subclavian artery or vein, acute ischemia, and subclavian vein thrombosis. Operative treatment involving orthopedic an

Having read through the previous litany of potential disasters and complications, one could ask the obvious question, "Why would anyone want to do this kind of surgery?" The answer is that most people elect not to! Nevertheless, for those who decide to venture into this field, there is the tremendous reward of being able to help patients who would otherwise continue to live with extremely disabling and disheartening symptoms. A sound knowledge of the regional anatomy and tutelage by those knowledgeable in particular areas of surgery that may not have been a part of the individual surgeon's pri

Dental hygienists perform tasks that are repetitive, strenuous, and can contribute to cumulative trauma disorders (CTDs) of the upper body. Besides prevalent disorders of the hand and wrist, such as carpal tunnel syndrome (CTS), there are many other upper extremity disorders, including thoracic outlet syndrome (TOS). If a dental hygienist can identify symptoms and seek treatment early, prognosis for recovery is greatly improved.

INTRODUCTION: The title "Thoracic Outlet Syndrome" (TOS) was introduced by Peet in 1956 [1]. In 1958 Charles Rob defined TOS as a "set of symptoms that may exist due to compression on the brachial plexus and on subclavian vessels in the region of the thoracic outlet" [2]. Compression due to cervical rib was first described by Galenus and Veaslius in the 2nd century A.D. The first unsuccessful resection of the cervical rib in patients with TOS was performed by Coote in 1861 [4]. In 1905 Murphy first made a successful resection of the cervical rib in patients with TOS and subclavian artery aneur
