Determination of the Optimal Landmark for Tube Thoracostomy in Trauma Patients: A Retrospective Study.

Background/Objectives: Accurate and prompt tube thoracostomy (TT) placement within the safety zone while avoiding diaphragmatic injury remains challenging, particularly in trauma patients with distorted thoracic anatomy. This study evaluated the accuracy and safety of landmark-based TT techniques, including a novel mid-sternum method. Methods: In this retrospective study, chest computed tomography scans of 245 adult trauma patients who presented to a Level I trauma center in Korea between February and June 2022 were analyzed. TT insertion routes using the mid-sternum, nipple, and mid-arm point

A Retrospective Analysis of Needle Thoracostomies at a Tertiary Level 2 Trauma Center.

BACKGROUND: A tension pneumothorax is a condition that results in elevated pressure within the pleural space. The effective management of tension pneumothorax relies on needle decompression, commonly performed at the second intercostal space (ICS) midclavicular line (MCL). However, some literature suggests that catheters placed in the second intercostal space midclavicular line are prone to higher failure rates compared to the fifth intercostal space midaxillary line (MAL) (42.5% versus 16.7%, respectively). In this study, we aim to identify and scrutinize the prevalence of prehospital needle

An Analysis of Tube Thoracostomy in Combat Implications for Improved Prehospital Recognition and Treatment.

BACKGROUND: Thoracic trauma occurs frequently in combat and is associated with high mortality. Tube thoracostomy (chest tube) is the treatment for pneumothorax resulting from thoracic trauma, but little data exist to characterize combat casualties undergoing this intervention. We sought to describe the incidence of these injuries and procedures to inform training and materiel development priorities. METHODS: This is a secondary analysis of a Department of Defense Trauma Registry (DoDTR) data set from 2007 to 2020 describing prehospital care within all theaters in the registry. We described al

A Novel Tube Thoracostomy Simulation Model for the Deployed or Resource-Limited Environment.

Tube thoracostomy placement is a critical, life-saving procedure often performed in the setting of trauma for the treatment of intrathoracic injury, such as hemothorax or pneumothorax. It also represents a high-acuity low-occurrence (HALO) event for many medical providers who may have limited opportunities for practice and mastery, particularly those in austere or resource-limited environments, such as in the deployed setting. Simulation offers opportunities for practicing the techniques required to properly perform and master the procedure. Finding affordable and accessible models for trainin

Chest tube thoracostomy: A simple life-saving procedure with potential hazardous risks.

INTRODUCTION AND IMPORTANCE: Chest tube thoracostomy is a simple life-saving procedure with many benefits but comes with significant potential morbidity. Potentially all intra-thoracic organs are at risk of possible injury as well as peritoneal. CASE PRESENTATION: We present four patients who had chest tube thoracostomy with potential complications fortunately were managed promptly and recovered fully. CLINICAL DISCUSSION: Complications related to tube thoracostomy is reported up to 25 % especially when done under emergency conditions. While the procedure is reported safe, it's associated mo

Managing tube thoracostomy with thoracic ultrasound: results from a randomized pilot study.

PURPOSE: Tube thoracostomy (TT) is a simple and a life-saving procedure; nevertheless, it carries morbidity, even after its removal. Currently, TT is managed and removed by chest X-ray (CXR) evaluation. There are limitations and these are directly linked to complications. The use of thoracic ultrasound (US) has already been established in the diagnosis of pneumothorax (PTX) and hemothorax (HTX); its use, in substitution of CXR can lead to improvement in care. Our aim is to evaluate the efficiency and safety of US in the management of TT. METHODS: Prospective and randomized study with patients

Safety of patent ductus arteriosus closure in premature infants without tube thoracostomy.

During a 30-month period, 34 premature infants underwent surgical closure of a patent ductus arteriosus. The mean gestational age at birth was 25 +/- 0.3 weeks and the mean age at the time of operation was 3 +/- 0.3 weeks (mean weight, 829 +/- 54 g). Indomethacin therapy had failed in 32 patients, and 2 had contraindications to its use. The initial 8 patients had parascapular incision and ligation of the patent ductus arteriosus; the last 26 patients had a short transaxillary incision and clipping. The average duration of the operation from the time of incision to skin closure was 36 +/- 2 min

The Impact and Description of a Training Program With a Novel Landmark Device for Needle Thoracostomy.

BACKGROUND: Needle thoracostomy (NT) is a lifesaving procedure for patients with tension pneumothorax but requires correct anatomical placement to ensure effectiveness and minimize complications. We describe a novel emergency medical services (EMS) training using a lateral NT landmark device, the SAM ThoraSite, and report provider feedback on the training. METHODS: A 90-minute training was delivered to EMS providers. Providers completed pre- and post-training surveys regarding confidence in procedural performance, concerns about complications, and overall educational experience. RESULTS: The

Use of an Endotracheal Tube for a Thoracostomy in a Child in Cardiac Arrest: A Case Report.

BACKGROUND: Traditionally, when patients arrive at an emergency department in cardiac arrest with a concern for a pneumothorax, a thoracostomy tube is placed for decompression. A literature review found no case reports of an endotracheal tube being used in lieu of a thoracostomy tube for a pneumothorax in a child in cardiac arrest. CASE REPORT: A case of a 4-week-old female infant who presented to the emergency department in cardiac arrest with a pneumothorax is discussed. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: It is important for clinicians to be aware of alternative management

Comparison of outcomes between observation and tube thoracostomy for small traumatic pneumothoraces.

BACKGROUND: Traumatic pneumothorax management has evolved to include the use of smaller caliber tube thoracostomy and even observation alone. Data is limited comparing tube thoracostomy to observation for small traumatic pneumothoraces. We aimed to investigate whether observing patients with a small traumatic pneumothorax on initial chest radiograph (CXR) is associated with improved outcomes compared to tube thoracostomy. METHODS: We retrospectively reviewed trauma patients at our level 1 trauma center from January 1, 2016 through December 31, 2020. We included those with a pneumothorax size

Loculated empyema in a neonate successfully treated with chest tube thoracostomy and antibiotics.

Empyema thoracis, defined as the accumulation of pus in the pleural space, is a rare entity in the neonatal period. There are very few cases described in the medical literature and there are still no treatment protocols in the management of empyema in neonates. In older infants and children, intrapleural fibrinolytics and surgery are often utilized since treatment of complicated parapneumonic effusions with chest tube and antibiotics alone often fail due to the viscous fluid and presence of loculations. Presented here is a case of a term neonate who exhibited symptoms of respiratory distress o

Thoracostomy in children with severe trauma: An overview of the paediatric experience in Victoria, Australia.

OBJECTIVE: Thoracic trauma is a leading cause of paediatric trauma deaths. Traumatic cardiac arrest, tension pneumothorax and massive haemothorax are life-threatening conditions requiring emergency and definitive pleural decompression. In adults, thoracostomy is increasingly preferred over needle thoracocentesis for emergency pleural decompression. The present study reports on the early experience of thoracostomy in children, to inform debate regarding the best approach for emergency pleural compression in paediatric trauma. METHODS: Retrospective review of Ambulance Victoria and The Royal Ch

Needle Thoracostomy for Patients with Prolonged Transport Times: A Case-control Study.

UNLABELLED: Introduction The use of prehospital needle thoracostomy (NT) is controversial. Some studies support its use; however, concerns exist regarding misplacement, inappropriate patient selection, and iatrogenic injury. Even less is known about its efficacy in situations where there is a delay to definitive care. Hypothesis/Aim To determine any differences in survival of patients who underwent NT in the setting of prolonged versus short transport times, and to describe differences in mechanisms and complications between the two groups. METHODS: This was a retrospective, matched, case-con
