Repair of complete syndactyly by tissue expansion and composite grafts.

Repair of complete syndactyly by a combination of tissue expansion and composite grafts from the glabrous non-weight bearing areas of the foot has been performed on three syndactylies in two patients. The commissure and the lateral areas of the proximal and middle phalanges were covered with expanded skin and the separated fingertips were covered with composite grafts. Without using an ordinary skin graft, this method can provide aesthetically excellent results with good skin colour and texture.

Clinical applications of tissue expansion in head and neck surgery.

Tissue expansion offers a new frontier in head and neck reconstruction in that it provides the capability of creating additional new skin for restoration of defects with skin of identical color, thickness, and appendages. The applicability of tissue expansion is only beginning to be appreciated as it is used in various aspects of head and neck surgery. Currently, the technique is most frequently used in scalp surgery for correction of male pattern baldness. Other uses of tissue expansion are in cranial facial surgery and expansion of distant, regional, or microsurgical flaps. Perhaps its great

Tissue expansion in the treatment of giant congenital melanocytic nevi.

Treatment of giant facial congenital melanocytic nevi (CMN) is recommended due to the potential for malignant degeneration, although therapy may be complicated because of cosmetic and functional considerations and the results problematic. We present the case of a 2-year-old child with a giant CMN of the scalp, excision of which was facilitated by the use of a tissue expander. This technique allows the creation of skin similar in appearance and type to that immediately adjacent to the defect. The giant CMN, which covered approximately one third of the scalp, was excised and simultaneously resur

Mucosa-associated lymphoid tissue lymphoma with monoclonal T-cell expansion.

Prior studies of marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma) have mainly emphasized B-cell-intrinsic oncogenic mechanisms and the supportive role of T-cells. However, whether neoplastic B-cells' microenvironment can reciprocally drive T-cell clonality or even promote T-cell neoplastic evolution remains poorly understood. Prompted by an index case where MALT lymphoma evolved into composite lymphoma with peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS), we systematically investigated T-cell receptor (TCR) gene rearrangements in 147 consecutive MALT l

iPEX enables micrometre-resolution deep spatial proteomics via tissue expansion.

The number of spatial omics technologies being developed is increasing1. However, a missing tool is one that can locate proteins in tissues in an untargeted manner at high spatial resolution and coverage. Here we present in situ imaging proteomics via expansion (iPEX), which integrates isotropic tissue magnification2 with matrix-assisted laser desorption/ionization (MALDI) mass spectrometry imaging. iPEX provides scalable spatial resolution down to the micrometre scale and substantially increases the sensitivity of protein identification by 10-100-fold. Using the retina as a model, iPEX enable

Healthy versus Unhealthy Adipose Tissue Expansion: the Role of Exercise.

Although the hallmark of obesity is the expansion of adipose tissue, not all adipose tissue expansion is the same. Expansion of healthy adipose tissue is accompanied by adequate capillary angiogenesis and mitochondria-centered metabolic integrity, whereas expansion of unhealthy adipose tissue is associated with capillary and mitochondrial derangement, resulting in deposition of immune cells (M1-stage macrophages) and excess production of pro-inflammatory cytokines. Accumulation of these dysfunctional adipose tissues has been linked to the development of obesity comorbidities, such as type 2 di

B cell repertoire expansion occurs in meningeal ectopic lymphoid tissue.

Ectopic lymphoid tissues (ELT) can be found in multiple sclerosis (MS) and other organ-specific inflammatory conditions. Whether ELT in the meninges of central nervous system (CNS) autoimmune disease exhibit local germinal center (GC) activity remains unknown. In an experimental autoimmune encephalomyelitis model of CNS autoimmunity, we found activation-induced cytidine deaminase, a GC-defining enzyme, in meningeal ELT (mELT) densely populated by B and T cells. To determine GC activity in mELT, we excised meningeal lymphoid aggregates using laser capture microscopy and evaluated B cell reperto

Effects of rapid maxillary expansion on Holdaway soft tissue measurements.

The objective of the present study was to evaluate short-term soft tissue changes caused by rapid maxillary expansion (RME) in 18 subjects (15 females and 3 males) (mean age 13 years 6 months) with a bilateral posterior crossbite. Lateral cephalograms of the patients were obtained at three different time points: before RME (T1), after maxillary expansion (mean = 0.82 month) (T2), and after retention (mean = 5.95 months) (T3). Holdaway soft tissue measurements were used for the evaluation of soft tissue changes. Data were analysed statistically by means of paired t-tests. The facial soft tissue

Scrotal reconstruction using rapid intraoperative tissue expansion: a preliminary report.

PURPOSE: Extensive scrotal skin loss can pose a difficult management problem. In most cases delayed scrotal approximation is feasible due to the redundancy of scrotal skin. However, large defects usually require reconstruction using flaps or grafts. We present a novel technique of scrotal closure using rapid intraoperative tissue expansion to manage select extensive partial scrotal defects. MATERIALS AND METHODS: Between January 1996 and April 2006, 3 patients underwent delayed closure using rapid intraoperative tissue expansion for approximately 75% scrotal skin loss. Intraoperatively the re

[Local application of bFGF and sucralfate during continuous tissue expansion].

OBJECTIVE: To investigate the effect of local application of bFGF and sucralfate during continuous tissue expansion (CTE). METHODS: CTE combined with local administration of bFGF and sucralfate was used in twelve patients with scar and nasal tip defects. Twenty three expanders were placed in the subcutaneous pockets through intralesion short incisions. Continuous expansion began at 1-3 days after expander implantation. The histomorphological changes and epidermal cell proliferation were observed. The clinical results were investigated. RESULTS: The average inflation time was 8.9 days. The av

Chest wall deformity following maximal tissue expansion for breast reconstruction.

Forty-three women, in a series of 150, participated in a prospective study that examined their chest walls for deformities 3 months after maximal tissue expansion for single-breast reconstruction. Computed tomography imaging was used for this purpose. Twenty-one patients underwent immediate breast reconstruction and the other 22 patients underwent delayed reconstruction. Fifty-three percent had some chest wall abnormality. In the delayed group, chest wall deformities were more statistically significant (p < 0.001). Our findings suggest that chest wall deformity is a common occurrence after max

Chest-wall deformity following soft-tissue expansion for breast reconstruction.

We have presented a case in which the presumed pressure effects of tissue expansion caused multiple nondisplaced rib fractures of the anterior thorax in a patient undergoing breast reconstruction. Although the deformity was severe, a satisfactory cosmetic result was obtained and there have been no clinically significant sequelae during a 1-year follow-up period. The degree of bony deformation was most likely enhanced by the combination of this patient's severe osteoporosis, chronic steroid use, and peripheral vascular disease. The fragility and ease of fracture in the bones of osteoporotic pos

Multivariate Analysis of Risk Factors for Complications in Pediatric Tissue Expansion.

BACKGROUND: Tissue expanders represent one of the main surgical options for skin reconstruction in cases of tumors, traumalike burn injury, scar contracture, and alopecia. However, the tissue expander device is also associated with complications such as infection and extrusion. The aim of this study was to analyze risk factors for major complications of use of tissue expanders in pediatric patients using multivariate analysis. METHODS: A retrospective, single-center observational study was performed over 10 years in pediatric patients who were treated with tissue expanders for tumors, nevus,

Prophylactic Local Antibiotics for Tissue Expansion (PLATE) Improve Breast Reconstruction Outcomes.

BACKGROUND: Infection following tissue expander (TE) breast reconstruction is frequent and impactful. Preliminary reports demonstrate value of local antibiotic delivery for implant salvage and prophylactic potential. This article is a multiinstitutional retrospective study using surgeon-crafted tobramycin-vancomycin polymethyl methacrylate plates (prophylactic local antibiotics for tissue expansion [PLATE]) during TE implantation for infection prophylaxis. The authors hypothesized that the intervention would be associated with fewer infections compared with historical practice. METHODS: In 20

Reconstruction of Scalp Anterior Hairline with Tissue Expansion and Skin Flap.

INTRODUCTION: Cutaneous defects involving the frontal region and anterior hairline of the scalp can result from congenital or acquired conditions. The negative esthetic impact can cause disturbances in the psychic and social sphere of the patient, causing problems in interpersonal relationships and in the body image itself. The use of skin expanders is usually effective in this region due to the bone base providing support and stability for its use. CASE REPORT: We describe the case of a 64-year-old woman submitted to reconstruction of the anterior hairline of the scalp due to scar sequelae a

Tenomodulin promotes human adipocyte differentiation and beneficial visceral adipose tissue expansion.

Proper regulation of energy storage in adipose tissue is crucial for maintaining insulin sensitivity and molecules contributing to this process have not been fully revealed. Here we show that type II transmembrane protein tenomodulin (TNMD) is upregulated in adipose tissue of insulin-resistant versus insulin-sensitive individuals, who were matched for body mass index (BMI). TNMD expression increases in human preadipocytes during differentiation, whereas silencing TNMD blocks adipogenesis. Upon high-fat diet feeding, transgenic mice overexpressing Tnmd develop increased epididymal white adipose

Contribution of Maladaptive Adipose Tissue Expansion to Development of Cardiovascular Disease.

The overweight and obesity epidemic has led to an increase in the metabolic syndrome and associated cardiovascular disease (CVD). These abnormalities include insulin resistance, type 2 diabetes mellitus, vascular stiffness, hypertension, stroke, and coronary heart disease. Visceral white adipocyte tissue (WAT) expansion and associated fibrosis/stiffness of WAT promote insulin resistance and CVD through increases in proinflammatory adipokines, oxidative stress, activation of renin-angiotensin-aldosterone system, dysregulation of adipocyte apoptosis and autophagy, dysfunctional immune modulation
