Comparison of the mechanical properties and mechanical damages to tendon tissue in three suspensory fixation techniques.

BACKGROUND: Anterior cruciate ligament (ACL) injury is the most common traumatic injury to the knee joint. Suspensory fixation has become popular in ACL reconstruction because of its high primary stability, less invasiveness, and surgical convenience. There are two common types of suspensory fixation devices: those with fixed-length and those with adjustable-length loops. Owing to structural differences and differences in initial tensioning techniques, it is expected that mechanical property and damage to the tendons will vary from device to device; however, no literature has examined this so

Soft Tissue Structures at Risk With Percutaneous Posterior to Anterior Screw Fixation of the Talar Neck.

BACKGROUND: Fractures of the talar neck and body can be fixed with percutaneously placed screws directed from anterior to posterior or posterior to anterior. The latter has been found to be biomechanically and anatomically superior. Percutaneous guidewire and screw placement poses anatomic risks for posterolateral and posteromedial neurovascular and tendinous structures. The objective of this study was to determine the injury rate to local neurovascular and tendinous structures using this technique in a cadaveric model. In addition, we aimed to determine the number of attempts at passing the g

Conversion from Spanning External Fixation to Open Reduction Internal Fixation Contributes to Healing of Soft Tissues and Fractures in Open Distal Humeral Fractures.

BACKGROUND Open distal humeral fractures (DHFs) often lead to loss of elbow function, thereby seriously affecting patient quality of life. The aim of this study was to evaluate the treatment outcomes of 2 surgical techniques to determine the better method for repairing open DHFs. Both groups were treated with immediate debridement first, and then group I had only internal fixation (IF), while group II underwent initial external fixation (EF) followed by IF surgery. MATERIAL AND METHODS This retrospective study included 32 patients who had open DHFs between 2013 and 2018. Twelve patients underw

Conservative Treatment of Empyema Formation Following Intrathoracic Rib Fixation With Antibiotics and Tissue Plasminogen Activator/Dornase.

Rib plating is a recommended intervention for patients with multiple rib fractures or flail chest to improve shortness of breath, significantly reduce pain, and shorten the length of hospital stay. Here, we report a unique and extremely rare finding in a patient with empyema following intrathoracic rib fixation. A 32-year-old male with a history of alcohol use disorder presented to the emergency department trauma bay after a motor vehicle accident. Computed tomography (CT) showed right hemopneumothorax and fourth to ninth rib fractures with displacement. The right fifth and sixth ribs were the

Successful Bailout Retrieval of an Active-Fixation Atrial Leadless Pacemaker Following Suspected Tissue Entrapment During Repositioning.

An 81-year-old woman with a medical history of mechanical mitral valve replacement, Maze procedure, and multiple myeloma presented with symptomatic sinus arrest and underwent AVEIR DR implantation. Although the AVEIR VR was successfully implanted, optimal electrical parameters could not be achieved during AVEIR AR placement. During repositioning of the AVEIR AR, myocardial entrapment with helix stretching was encountered. Here, we report that advancing the protective sleeve to fully encapsulate the device potentially enabled safe and successful retrieval.

Case Report: Soft-tissue-preserving fixation for displaced segmental maxillary fractures involving partially detached bone segments.

BACKGROUND: Displaced segmental maxillary fractures involving partially detached bone segments are difficult to manage because treatment requires both stable fixation and preservation of residual vascularity. Excessive mucoperiosteal elevation may further compromise the blood supply of a displaced segment and increase the risk of bone resorption, sequestration, or necrosis. This report presents two cases of displaced segmental maxillary fractures treated with soft-tissue-preserving fixation strategies. CASE PRESENTATION: Two patients with displaced segmental maxillary fractures were managed u

Tissue fixation effects on human retinal lipid analysis by MALDI imaging and LC-MS/MS technologies.

Imaging mass spectrometry (IMS) allows the location and abundance of lipids to be mapped across tissue sections of human retina. For reproducible and accurate information, sample preparation methods need to be optimized. Paraformaldehyde fixation of a delicate multilayer structure like human retina facilitates the preservation of tissue morphology by forming methylene bridge crosslinks between formaldehyde and amine/thiols in biomolecules; however, retina sections analyzed by IMS are typically fresh-frozen. To determine if clinically significant inferences could be reliably based on fixed tiss

Behavior of a new long-chain cyanoacrylate tissue adhesive used for mesh fixation in hernia repair.

BACKGROUND: Synthetic tissue adhesives (TA) are sometimes used in hernia repair surgery. This study compares the use of a new, noncommercial, long-chain cyanoacrylate (n-octyl) TA and Ifabond for mesh fixation. MATERIALS AND METHODS: In two implant models in the rabbit, expanded polytetrafluorethylene meshes were fixed to the parietal peritoneum using a TA or tacks (intraperitoneal model), or polypropylene meshes used to repair partial abdominal wall defects were fixed with a TA or sutures (extraperitoneal model). Animals were euthanized 14 or 90 d postsurgery and implant specimens were proce

Management of infected fractures of the tibia with associated soft tissue loss: experience with external fixation, bone grafting and soft tissue reconstruction using pedicle muscle flaps or microvascular composite tissue grafts.

The aim of this study was to assess the results of treatment given by a team of orthopaedic and plastic surgeons in a series of infected unhealed fractures of the tibia associated with loss of adjacent soft tissues. Twenty-five lower leg fractures, treated during a 10 year period, entered the study and were grouped according to the principles of treatment followed. In the earlier Group A (nine patients), the osteosynthesis implants were retained or changed to more stable internal fixation devices, the soft tissue defects were closed by conventional muscle or musculocutaneous flaps and bone gra

Intraoperative Application of Ultrasonic Energy to Bioabsorbable Craniosynostosis Fixation Devices and Adverse Tissue Reaction. Is There a Correlation?

Bioabsorbable internal fixation is a well-accepted modality that is especially suitable for application in craniosynostosis. When first introduced, high rates of adverse tissue reactions were observed that have since been ameliorated with more biocompatible polymer formulations. However, the phenomenon has not entirely disappeared, and such reactions remain vexing. It has been suggested that intraoperative use of ultrasonic activation to heat and remodel 50:50 poly(D-lactide-co-L-lactide) fixation devices could potentiate an adverse tissue response following craniosynostosis procedures. The ev

Making a science out of preanalytics: An analytical method to determine optimal tissue fixation in real-time.

Modern histopathology is built on the cornerstone principle of tissue fixation, however there are currently no analytical methods of detecting fixation and as a result, in clinical practice fixation is highly variable and a persistent source of error. We have previously shown that immersion in cold formalin followed by heated formalin is beneficial for preservation of histomorphology and have combined two-temperature fixation with ultra-sensitive acoustic monitoring technology that can actively detect formalin diffusing into a tissue. Here we expand on our previous work by developing a predict

Locking plate as a definitive external fixator for treating tibial fractures with compromised soft tissue envelop.

INTRODUCTION: Tibial fractures with compromised soft tissue envelop may lead to significant complications. The optimal management of these injuries remains controversial. Recently, locking plate used as a definitive external fixator is attractive because it not only minimizes trauma to the soft tissues, but also overcomes the shortcomings of standard external fixators. The objective of this study was to evaluate the outcome of using locking plate as a definitive external fixator for treating tibial fractures with compromised soft tissue envelop. PATIENTS AND METHODS: A prospective series of 1

External fixator kickstands for free soft tissue flap protection: case series and description of technique.

BACKGROUND: Protected elevation represents a critical component of postoperative care, particularly in posteriorly located flaps, to prevent pressure on the flap's vascular pedicle and ensure a successful skin graft. Although several short case series and technique papers have described kickstand placement to prevent heel ulcers as an adjuvant to fixator placement for fracture management, there remains a paucity of reports describing external fixator placement solely for extremity elevation and pressure alleviation in the postoperative care of flap coverage procedures. METHODS: Patients who u

Perineal body repair in patients with third degree rectocele: a critical analysis of the tissue fixation system.

AIM: We describe the technique of tissue fixation system (TFS) perineal body repair in patients presenting with symptomatic third degree rectocele. METHOD: The single sling TFS perineal body repair is performed in three surgical steps: (i) dissection of the rectum off the vagina and laterally displaced perineal body; (ii) identification of the deep transverse perineii muscles beyond their insertion point behind the descending pubic ramus; (iii) elevation and approximation of the separated and laterally displaced perineal bodies by insertion, without tension, of non-stretch 7 mm polypropylene

Transalveolar fixation of the peri-implant soft tissue in the mandible: surgical method and clinical follow-up.

PURPOSE: Firm, immobile mucosa around endosteal implants is one prerequisite for a reliable long-term result. In the atrophic mandible, this may be achieved by a vestibuloplasty on the facial side. However, on the lingual side, the floor of the mouth is often high and the mucosa mobile, especially after unintentional detachment of the lingual flap during implant surgery. To overcome this problem, a surgical technique applying a transalveolar suture circumscribing the inserted implants was developed. PATIENTS AND METHODS: The operation was performed in six patients, and they were evaluated ove
