[Unexpected tracheobronchomalacia during cardiac operation in a patient with Marfan's syndrome].

A 56-year-old man with Marfan's syndrome was scheduled for a valve-sparing aortic root replacement operation because of annuloaortic ectasia and aortic regurgitation. He had severe dyspnea. When the operation started, SpO2 decreased at the time of the median sternotomy, and increased by manual inflation. After sternal closure, PIP increased from 20 cmH2O to 28 cmH2O, SpO2 decreased from 98% to 66%, and the expiratory pattern indicated airway obstruction. Because hypoxemia persisted, we reopened the sternum, resulting in increased SpO2 and decreased PIP, but hypercapnea remained. Fiberoptic bro

Tracheobronchomalacia and Excessive Dynamic Airway Collapse: Current Concepts and Future Directions.

Tracheobronchomalacia (TBM) and excessive dynamic airway collapse (EDAC) are airway abnormalities that share a common feature of expiratory narrowing but are distinct pathophysiologic entities. Both entities are collectively referred to as expiratory central airway collapse (ECAC). The malacia or weakness of cartilage that supports the tracheobronchial tree may occur only in the trachea (ie, tracheomalacia), in both the trachea and bronchi (TBM), or only in the bronchi (bronchomalacia). On the other hand, EDAC refers to excessive anterior bowing of the posterior membrane into the airway lumen

A Case Report of Tracheobronchomalacia Requiring Stenting and Extracorporeal Membrane Oxygenation Treatment.

BACKGROUND: Tracheobronchomalacia (TBM) is characterized by the excessive expiratory collapse of the central airway due to structural weakness or membranous redundancy of the tracheal and bronchial cartilages. TBM may present with anatomical and physiological compromise of the airways. Aggressive treatments for severe respiratory failure include mechanical ventilation and extracorporeal membrane oxygenation (ECMO). Interventional approaches include short-term silicone stent trials to predict surgical benefits and definitive procedures, such as tracheobronchoplasty. CASE PRESENTATION: An Asian

Resorbable airway splint, stents, and 3D reconstruction and printing of the airway in tracheobronchomalacia.

Tracheobronchomalacia (TBM) is the most common tracheobronchial obstruction. Most cases are mild to moderate; therefore, they do not need surgical treatment. Severe tracheomalacia, however, represents a diagnostic and therapeutic challenge since they are very heterogeneous. In the armamentarium of resources for the treatment of dynamic airway collapse, splints and stents are two underused strategies and yet, they may represent the best alternative in selected cases. Lately, computed tomography 3D reconstruction of the airway has been used for the design of virtual models that can be 3D-printed

Multiple stents for tracheobronchomalacia caused by hydrochloric acid inhalation: a case report.

Tracheobronchomalacia (TBM) is a rare disease characterized by excessive collapsibility of the central airways during expiration. Potential consequences and treatment courses of the aspiration of erosive agents are seldom reported. To our knowledge, this is the first report of TBM caused by hydrochloric acid inhalation, which was successfully treated by multiple airway stents insertion. A 45-year-old female presented with dyspnea and pharynx discomfort after falling into a hydrochloric acid pool. The patient was successfully treated with multiple stents insertion. In 15 months after the stents

A case of tracheobronchomalacia due to relapsing polychondritis treated with Montgomery T-tube.

Relapsing polychondritis is a rare and multi-system autoimmune disease of unknown etiology characterized by inflammation and destruction of cartilaginous structures. Its clinical manifestations include recurrent chondritis of the ears, nose, pinna, peripheral joints, and laryngotracheobronchial tree and can be life-threatening in advanced cases of laryngotracheal stenosis. Because of the rarity of relapsing polychondritis and lack of understanding of its pathogenesis, there is no standard medical therapy, and treatment is tailored according to disease activity and site of organ involvement. A comparison of virtual bronchoscopy versus flexible bronchoscopy in the diagnosis of tracheobronchomalacia in children.

INTRODUCTION: Flexible bronchoscopy (FB) is the current gold standard for diagnosing tracheobronchomalacia. However, it is not always feasible and virtual bronchoscopy (VB), acquired from chest multi-detector CT (MDCT) scan is an alternative diagnostic tool. We determined the sensitivity, specificity, and positive and negative predictive values of VB compared to FB in diagnosing tracheobronchomalacia. METHODS: Children aged <18-years scheduled for FB and MDCT were recruited. FB and MDCT were undertaken within 30-min to 7-days of each other. Tracheobronchomalacia (mild, moderate, severe, very

Refractory asthma treatment is complicated by tracheobronchomalacia: case reports and review of the literature.

Tracheobronchomalacia (TBM) is defined as the condition where the airway lumen narrows more than 50 percent. The acquired TBM usually occurs in adults; however, the prevalence of TBM in asthma is unknown. We report two cases of severe asthma in elderly patients that could not be controlled with higher medication use. Case 1 was a 70-year-old woman with sever persistent asthma for 10 years, presented with uncontrolled symptoms for 4 months. A CT of the chest showed collapse of the trachea at the posterior wall. Case 2 involved a 72-year-old woman with partly controlled asthma presenting with un

Stridor and respiratory failure due to tracheobronchomalacia: case report and review of the literature.

CONTEXT: Tracheobronchomalacia (TBM) results from structural and functional abnormalities of the respiratory system. It is characterized by excessive collapse: at least 50% of the cross-sectional area of the trachea and main bronchi. In this paper, we present a rare case of a patient with TBM who first presented with stridor and respiratory failure due to exacerbation of chronic bronchitis. CASE REPORT: An 81-year-old Caucasian man was admitted presenting coughing, purulent sputum, stridor and respiratory failure. He had a medical history of chronic obstructive pulmonary disease (COPD) and si

Endoscopic successful management of tracheobronchomalacia with laser: apropos of a Mounier-Kuhn syndrome.

Mounier-Kuhn syndrome is a rare condition that combines tracheobronchomegaly (TBM) and severe tracheobronchomalacia. Symptoms can be severe with recurrent bronchopulmonary infections and cough-induced syncope. Therapeutic management is non-specific and limited to chest physiotherapy and antibiotics during infectious exacerbations. We report a case of Mounier-Kuhn syndrome that was successfully managed by treating the posterior collapse of the central airway with yttrium aluminum pevroskyte laser. Endoscopic aspects, respiratory symptoms, and lung function tests all improved and remained stable

Tracheobronchomalacia treated by inserting a long T-tube into the left main bronchus.

An 88-year-old woman with advanced Parkinson's disease (stage V on the Yahr scale) had difficulty in expectoration and underwent tracheostomy in 1999. In July 2004, granulation tissue was formed in the tracheal lumen at the tip of the tracheostomy tube, and a standard type silicone T-tube was inserted. Thereafter, she was cared for at home, where she was doing well until early March 2009, when a decrease in SaO(2) and difficulty in sputum aspiration were noted. Bronchoscopy showed crescent type tracheobronchomalacia involving the trachea down to the orifice of the left main bronchus. Consideri

Pectus excavatum repair using a costal cartilage graft for patients with tracheobronchomalacia.

BACKGROUND: Pectus excavatum is sometimes associated with tracheobronchomalacia, which usually manifests left mediastinal shift, atelectasis of the left lung, and recurrent pulmonary infection. Standard repair of pectus excavatum alone usually failed to improve symptoms. METHODS: Pexis of the great vessels and pericardium combined with the support of the lower sternum, using a contralateral costal cartilage graft following the standard Ravitch's repair of pectus excavatum, has been used in 6 children during the past 5 years. In addition to respiratory symptoms, diagnosis of tracheobronchomala

A shape memory airway stent for tracheobronchomalacia in children: an experimental and clinical study.

The authors have designed a coil airway stent using a thermal shape-memory titanium-nickel alloy (SMA) to relieve airway collapse in children. A characteristic of alloy allowed the stent to be enclosed in the thin introducer tube and to position it bronchoscopically in the collapsed airway. When the stent is warmed to 37 to 40 degrees C, it expands to the memorized diameter and stents the airway. In eight rabbits, an experimental model of potentially fatal tracheomalacia was created by fracturing the tracheal cartilages. The stents of 6 mm in diameter and 15 mm in length were placed, and then

Minimal Invasive Approach in Severe Tracheobronchomalacia: Türkiye's First Case of Vats Tracheobronchoplasty.

Tracheobronchomalacia is an expiratory collapse of the posterior wall of the trachea and at least one of the main bronchi. It may be idiopathic or may develop due to secondary causes. It results in a slowing of dynamic airflow and clinically presents with dyspnea, cough, and pneumonia. Although tracheal stents provide temporary relief in treatment, considering the problems encountered during the chronic phase, endobronchial therapy does not offer a long-term solution. Tracheobronchoplasty is a surgical treatment option. Examination of the few cases reported in the literature shows that, in cen

Airway Management for Ventilation Instability After Tracheostomy in Pediatric Patients With Tracheobronchomalacia.

INTRODUCTION: Pediatric patients with tracheobronchomalacia may require tracheostomy for long-term mechanical ventilation. Ventilator instability can persist despite a stable tracheostomy tube and is characterized by desaturations, rising pressure requirements, or the need for cardiopulmonary resuscitation. There is a paucity of data on the management of ventilatory instability in tracheostomy and ventilator-dependent pediatric patients. OBJECTIVES: To evaluate inpatient airway outcomes after tracheobronchoscopy in pediatric patients with tracheostomy and ventilatory dependence. METHODS: Ret

Prevalence of tracheobronchomalacia is higher than previously reported in children with cystic fibrosis.

BACKGROUND: Tracheobronchomalacia (TBM) is estimated to be present in 1 in 2100 children. Previous reports suggest the prevalence is higher in children with cystic fibrosis (CF). This has clinical implications with potential to influence airway clearance and lung health. AIM: To determine the prevalence and clinical associations of TBM in Western Australian children with CF. METHODS: Children with CF born between 2001 and 2016 were included. Operation reports from bronchoscopies performed until the age of 4 were retrospectively reviewed. Data were collected on the presence, persistence defin

Dynamic computed tomography for evaluation of tracheobronchomalacia in premature infants with bronchopulmonary dysplasia.

INTRODUCTION: Dynamic computed tomography (dCT) gives real-time physiological information and objective descriptions of airway narrowing in tracheobronchomalacia (TBM). There is a paucity of literature in the evaluation of TBM by dCT in premature infants with bronchopulmonary dysplasia (BPD). The aim of this study is to describe the findings of dCT and resultant changes in management in premature infants with TBM. METHODS: A retrospective study of 70 infants was performed. Infants included were <32 weeks gestation without major anomalies. TBM was defined as ≥50% expiratory reduction in cross-

Non-invasive diagnosis of tracheobronchomalacia using a modified ventilation radioisotope lung scan.

The use of radionuclide ventilation lung scan to characterise the physiological effects of tracheobronchomalacia is a novel application of this non-invasive technique. In the reported case the right upper lobe was found to be not ventilated below a pressure of 20 cm H2O despite evidence from a dynamic tracheobronchogram of the right upper lobe bronchus opening at the lower pressure of 15 cm H2O.
