OBJECTIVE: To propose a practical and accessible model that enables the training of this technique.
METHODS: The model comprises a structure composed of a wood board resting on a PVC tube. A foam block on which the pig trachea is supported is placed on this structure. A foam sheet covered with fabric is placed over the trachea to simulate the soft parts of the neck and the skin. This is a practical training model for practice of surgeons in percutaneous tracheostomy. A questionnaire on the assessment of the proposed model to each participating surgeon was applied.
RESULTS: Ten surgeons under

The number of tracheotomized patients with dysphagia and their need for treatment are continuously increasing in clinical and community settings. The revised version of the directive on home care and community-based intensive care of the Federal Joint Committee (G-BA) requires that tracheotomized patients are regularly evaluated with the aim of identifying and promoting the therapeutic potential after hospital discharge. Dysphagia treatment plays a crucial role as without improvement of severe dysphagia there is practically no possibility for decannulation. Tracheotomized patients with dysphag

Children with tracheostomies have multiple challenges with respect to achieving normal deglutition. These children may have underlying neurologic or genetic conditions that can predispose to dysphagia, but even in children without underlying comorbidities, the presence of a tracheostomy tube impacts the mechanics of swallowing, leading to difficulty with different consistencies as well as management of normal oral secretions. Intubation prior to tracheostomy also impacts sensation in the upper aerodigestive tract increasing the risk of aspiration. Occlusion of the tracheostomy with a speaking

PURPOSE OF REVIEW: Pediatric decannulation failure can be associated with large morbidity and mortality, yet there are no published evidence-based guidelines for pediatric tracheostomy decannulation. Tracheostomy is frequently performed in medically complex children in whom it can be difficult to predict when and how to safely decannulate.
RECENT FINDINGS: Published studies regarding pediatric decannulation are limited to reviews and case series from single institutions, with varying populations, indications for tracheostomy, and institutional resources. This article will provide a review of

Patients with acute respiratory failure who are anticipated to have a significant recovery and require prolonged mechanical ventilation, defined as ventilation lasting 7 days or longer, should be evaluated for the potential need for a tracheostomy. A tracheostomy reduces the necessity for sedation and aids in the process of weaning patients. The popularity of percutaneous tracheostomy techniques, which can be carried out at the patient's bedside, has increased due to their ability to save costs associated with time in the operating room. This video tutorial provides a comprehensive guide compr

Percutaneous dilatational tracheostomy (PDT) and percutaneous endoscopic gastrostomy (PEG) tube placements are routine procedures performed in the intensive care units (ICUs). They are performed to facilitate care and promote healing. They also help prevent complications from prolonged endotracheal intubation and malnutrition. In most cases, both are performed simultaneously. Physicians performing them require knowledge of local anatomy, tissue and vascular relationships, along with advance bronchoscopy and endoscopy skills. Although PDTs and PEGs are considered relatively low-risk procedures,

Objective Tracheostomy is one of the oldest operations for the management of airway obstruction. With time, indications expanded to prolonged mechanical ventilation, and currently, the majority of tracheostomies are done for this reason. There are several techniques used in a tracheostomy procedure, depending on surgeon preference. Immediate complications such as bleeding, pneumothorax, pneumomediastinum, airway fire, and posterior tracheal wall perforation with esophageal injury are rare, although they do occur, and must be managed accordingly. This study aimed to assess differences in types

We describe a modified technique for percutaneous dilatational tracheostomy using a 15F tube exchanger or Eschmann catheter. A retrospective review of 1180 procedures using this modified technique demonstrated it to be effective with a failure rate of only 0.25% (3 patients). Moreover, it provides an additional safeguard with the ability to rapidly reintroduce the endotracheal tube into the trachea guided by the exchange catheter in the event of accidental extubation during the procedure. This technique needs no additional special devices or equipment (eg, a bronchoscope). However, a prospecti

The incidence of chronically ill subjects with prolonged mechanical ventilation has significantly increased over the last decade. Many patients get discharge to Skilled Nursing Facilities with an artificial airway, which do not have the means to properly progress on weaning. In Portugal this prevalence is unknown. Our aim was to establish the prevalence of tracheostomized patients at SNF in the North of Portugal, characterizing these units and its population, in a cross-sectional study, through an online questionnaire answered on the same day. Of the 75 SNF, 30 answered: 13 long-term, 2 medium

Purpose The purpose of this clinical focus article is to describe the frequency, indications, and outcomes of fenestrated tracheostomy tube use in a large academic institution. Method A retrospective chart review was conducted to evaluate the use of fenestrated tracheostomy tubes between 2007 and 2017. Patients were included in the study if they were ≥ 18 years of age and received a fenestrated tracheostomy tube in the recent 10-year period. Results Of 2,000 patients who received a tracheostomy, 15 patients had a fenestrated tracheostomy tube; however, only 5 patients received a fenestrated tr

BACKGROUND: Tracheostomy is the most obliging and one of the oldest operations in the history of surgery. It is channel between the trachea and skin in the midline in the neck. Most common indications are upper airway obstruction, mechanical respiratory insufficiency and pulmonary toileting etc.
METHODS: It is a retrospective study done in a teaching hospital in eastern Nepal from 2005-2008. Medical records of 175 tracheostomized patients were reviewed. Demographic variations, indications and outcome were recorded and analyzed.
RESULTS: Age ranged from 1 to 88 years. Most common age group wa

BACKGROUND: This study aimed to assess early and long-term results after anterior mediastinal tracheostomy (AMT) as a salvage operation for recurrent neck malignancies.
METHODS: Between October 2006 and February 2013, 12 patients (mean age, 57 years) underwent AMT. All patients had experienced stomal recurrence, with or without esophageal involvement, after laryngectomy. All patients had undergone previous radiotherapy (50.3 ± 6.2 [43-60] Gy) and previous surgical treatment: total laryngectomy (n = 11) and thyroidectomy (n = 2).
RESULTS: The mean length of resected trachea was 3.7 ± 2.7 (2.5

A 63-year-old man underwent anterior mediastinal tracheostomy for postradiotherapy stoma ulceration exposing the left anterolateral tracheal side, cervical vessels, and pharynx. An anterior chest wall defect (hemiclaviculectomy, manubriectomy, and resection of anterior third of first and second ribs bilaterally) was covered by a myocutaneous pectoral flap, and a new tracheostoma was constructed in the middle of the skin island. At postoperative day 7, a protrusion of the right upper lobe outside the thoracic cavity through the anterior chest wall defect was detected. Surgical repair by a right

Tracheostomy is a common airway procedure for life support. This procedure is safe, although occasional early and late complications are known to occur. Fracture and hence aspiration of a tracheostomy tube in the tracheobronchial tree is a rare late complication, which can be potentially life threatening. Published reports of a fractured tracheostomy tube presenting as a foreign body in the tracheobronchial tree are few. The most common dislodged sites reported were the trachea and the right main bronchus, the inner flange in our patient was lodged in the trachea and the left main bronchus. Fo

OBJECTIVE: To identify factors contributing to unplanned tracheostomy following cardiac surgery in children under 12 months without prior airway support.
METHODS: Case series with chart review.
RESULTS: Eleven patients were identified (eight male, three female). Eight were term, three preterm. Four had syndromes associated with cardiac disease. Age at cardiac surgery was 2.2 (0.1-5.2) months. Time between surgery and tracheostomy was 1.2 (0-3) months. Two groups were identified. The first had tracheobronchomalacia as the primary diagnosis (n = 9). Time post-surgery for tracheostomy was 1.2 (

A rare case of nasal polyposis requiring urgent tracheostomy is reported. A 70-year-old male presented to the hospital with stridor. He had a 10-year history of nasal obstruction. A large proliferative mass was found to be occupying both the nasal cavities. An emergency tracheostomy and biopsy of the lesion was performed. A computerized axial scan (CT) demonstrated that the lesion occupied the pharynx reaching up to the laryngeal inlet. To our knowledge this is the first case reported in the literature. Upper airway obstruction from nasal polyps is uncommon but can cause significant morbidity

BACKGROUND: Patients with cervical spine injury may require both anterior cervical spine fusion and tracheostomy, particularly in the setting of associated cervical spinal cord injury (SCI). Despite the close proximity of the two surgical incisions, we postulated that tracheostomy could be safely performed after anterior spine fixation. In addition, we postulated that the severity of motor deficits in patients with cervical spine injury would correlate with the need for tracheostomy.
METHODS: A retrospective review was undertaken of all adult trauma patients diagnosed with cervical spine frac
