Tracheostomy in Infants in the Neonatal Intensive Care Unit.

Approximately half of all pediatric tracheostomies are performed in infants younger than 1 year. Most tracheostomies in patients in the NICU are performed in cases of chronic respiratory failure requiring prolonged mechanical ventilation or upper airway obstruction. With improvements in ventilation and management of long-term intubation, indications for tracheostomy and perioperative management in this population continue to evolve. Evidence-based protocols to guide routine postoperative care, prevent and manage tracheostomy emergencies including accidental decannulation and tube obstruction,

Tracheostomy in neurologically compromised paediatric patients: role of starplasty.

OBJECTIVES: Starplasty tracheostomy is an alternative to traditional tracheostomy. This paper reviews neurologically compromised paediatric patients with tracheostomies and discusses the role of starplasty tracheostomy. METHOD: A retrospective review was conducted of paediatric patients with a neurological disorder who underwent tracheostomy between 1997 and 2011. RESULTS: Forty-eight patients, with an average age of 7.3 years, were identified. The most common indications for tracheostomy were: ventilator dependence (39.6 per cent), an inability to tolerate secretions or recurrent aspiration

Incidence and Long-Term Outcomes of Pediatric Tracheostomy in Japan: A Cohort Study Using a Japanese Claims Database.

BACKGROUND: Epidemiological data on pediatric tracheostomy are limited, particularly in Asia. We estimated the incidence and evaluated long-term post-tracheostomy outcomes and their predictors using a large Japanese claims database. METHODS: We used the Japanese administrative claims database (JMDC) from 2005 to 2025. All individuals aged < 20 years observed during continuous enrollment contributed person-time to the incidence analysis. Individuals who underwent incident tracheostomy constituted the post-tracheostomy cohort. Cumulative incidences of decannulation and death were estimated usin

Tracheocutaneous Fistula After Pediatric Open Airway Reconstruction.

OBJECTIVES: Tracheocutaneous fistula (TCF) is a common occurrence after pediatric tracheostomy decannulation. However, the persistence of TCF after staged reconstruction of the pediatric airway is not well-described. The primary objective was to determine the rate of persistent TCF after successful decannulation in children with staged open airway reconstruction. METHODS: A case series with chart review of children who underwent decannulation after double-stage laryngotracheal reconstruction between 2017 and 2019. RESULTS: A total of 26 children were included. The most common open airway pro

Glutathione peroxidase 4 (GPX4) as an oncogenic factor in human tumors: a pan-cancer analysis.

BACKGROUND: Tracheostomy is one of the most common procedures done in intensive care unit (ICU) patients. Decannulation is the weaning off from tracheostomy to maintain spontaneous respiration and/or airway protection. However, this step needs a near perfect coordination of brain, swallowing, coughing, phonation, and respiratory muscles. However, despite its perceived importance, there is no universally accepted protocol for this vital transition. In this systematic review of decannulation we focus attention to this important aspect of tracheostomy care. AIM: To compare the two methods of dec

Factors that influence successful decannulation after surgery for laryngo-tracheal stenosis in children.

The process to decannulation requires attention to details from the time of initial tracheostomy, through the pre- and peri-operative period up until the decannulation event. Important points to consider during this process are: a formal tracheostomy rather than a tracheotomy; other potential sites of obstruction than the laryngeal stenosis; gastroesophageal reflux; prevention and control of infection and the use of prophylactic antibiotics; method and type of suture material; stenting; movement and method of feeding. Decannulation itself requires attention to removal of granulation tissue and

Pediatric Tracheostomy Year in Review.

Tracheostomized children have higher mortality compared to adults due to smaller airway anatomy and greater medical complexity and are at high risk for life-threatening complications. Following new tracheostomy placement, caregivers are required to successfully complete extensive training before discharge home. Training for tracheostomy emergencies such as tube obstruction and accidental decannulation is challenging without real-life, hands-on experience, but simulation training has shown promising effects on improving caregiver knowledge and comfort in preparing for emergency situations. The severe burns patient with tracheostomy: implications for management of dysphagia, dysphonia and laryngotracheal pathology.

INTRODUCTION: Insertion of a tracheostomy for a severe burn patient is not uncommon. The method of decannulation, effects of the tracheostomy on voice and swallowing and subsequent complications have not been described in the literature specifically for this population. The aim of this study was to investigate the risk of dysphagia, dysphonia and laryngotracheal pathology in severe burn patients with tracheostomy and following decannulation. METHOD: A retrospective chart review was conducted for severe burn patients admitted from January 2000 to December 2007 that received tracheostomy as par

Early Rehabilitation Reduces Time to Decannulation in Patients With Severe Acquired Brain Injury: A Retrospective Study.

Purpose: Early decannulation is considered a main rehabilitative goal in tracheostomized patients. Our aim is to evaluate whether a very early rehabilitation protocol helps to reduce the tracheostomy duration in patients affected by an Acquired Brain Injury (ABI). Methods: Data about consecutive tracheostomized patients admitted in our Neuro-Rehabilitation Unit (NRU) were retrospectively collected. We defined two groups: Early Rehabilitation Group patients came from our ICU, where they started the rehabilitative treatment; Delayed Rehabilitation Group patients arrived from external ICUs and st

Unmasking of tracheomalacia following short-term mechanical ventilation in a patient of adult respiratory distress syndrome.

Patients with chronic obstructive pulmonary disease (COPD) are susceptible to airway malacia, which may be unmasked following mechanical ventilation or tracheostomy decannulation. Dynamic imaging of central airways, a non-invasive test as effective as bronchoscopy to diagnose airway malacia, has increased the recognition of this disorder. We describe a 70-year-old woman admitted with adult respiratory distress syndrome. She had cardiorespiratory arrest on admission, from which she was successfully resuscitated. She had obesity, hypertension, diabetes mellitus, recurrent ventricular tachycardia

Laser arytenoidectomy.

An endoscopic laser arytenoidectomy was performed on 20 patients for bilateral abductor vocal cord paralysis due to various causes. Ten patients had a previous tracheostomy prior to the laser arytenoidectomy and all were subsequently decannulated. One patient without a previous tracheostomy required an immediate tracheostomy after the completion of the laser arytenoidectomy due to a severe laryngeal edema and was also subsequently decannulated. All of these patients had a satisfactory voice and an adequate laryngeal airway. The advantages of the laser for endoscopic arytenoidectomy are facilit
