Translational evidence for the Inflammatory Response System (IRS)/Compensatory Immune Response System (CIRS) and neuroprogression theory of major depression.

Major depressive disorder (MDD) is a common, severe and disabling neuropsychiatric disorder with a heterogenous etiology. Among the most widely recognized etiological models, immunopathogenesis is a predominant one. Numerous studies have demonstrated aberrant levels of inflammatory markers in the peripheral blood, cerebrospinal fluid (CSF) and brain of patients with MDD. Multiple studies including meta-analyses have reported increased peripheral levels of acute phase proteins, and pro-inflammatory cytokines, particularly IL-1β, TNF-α, and IL-6 in MDD. Postmortem brain studies similarly demonst

Upadacitinib: Mechanism of action, clinical, and translational science.

Upadacitinib is a selective Janus kinase (JAK) inhibitor which is approved by the US Food and Drug Administration, the European Medicines Agency, as well as other agencies around the world for the treatment of several chronic inflammatory diseases, including rheumatic, dermatologic, and gastrointestinal diseases. Through inhibition of JAK, upadacitinib inhibits phosphorylation of downstream effector proteins, which consequently inhibits cytokine signaling for key pathways involved in inflammatory diseases. Upadacitinib more potently inhibits JAK1 than other JAK isoforms. The pharmacokinetics,

Towards prevention of ischemia-reperfusion kidney injury: Pre-clinical evaluation of 6-chromanol derivatives and the lead compound SUL-138✰.

Acute kidney injury (AKI) is a global healthcare burden attributable to high mortality and staggering costs of dialysis. The underlying causes of AKI include hypothermia and rewarming (H/R), ischemia/reperfusion (I/R), mitochondrial dysfunction and reactive oxygen species production. Inspired by the mechanisms conferring organ protection in hibernating hamster, 6-chromanol derived compounds were developed to address the need of effective prevention and treatment of AKI. Here we report on the pre-clinical screening of 6-chromanol leads that confer protection during I/R to select compounds with

Fanconi anemia-isogenic head and neck cancer cell line pairs: A basic and translational science resource.

Fanconi anemia (FA) is a heritable malformation, bone marrow failure and cancer predisposition syndrome that confers an exceptionally high risk of squamous carcinomas. These carcinomas originate in epithelia lining the mouth, proximal esophagus, vulva and anus: their origins are not understood, and no effective ways have been identified to prevent or delay their appearance. Many FA-associated carcinomas are also therapeutically challenging: they may be multi-focal and stage-advanced at diagnosis, and most individuals with FA cannot tolerate standard-of-care systemic therapies such as DNA cross

The National Paediatric Applied Research Translation Initiative (N-PARTI): using implementation science to improve primary care for Australian children with asthma, type 1 diabetes, and infections.

General practice-based care for Australian children is facing low levels of clinical guideline adherence particularly in three key areas: asthma, type 1 diabetes and antibiotic use. We offer an implementation science-informed position paper, providing a broad overview of how we aim to address this issue. This is the co-designed National Paediatric Applied Research Translation Initiative (N-PARTI), a bespoke, three-phased research solution by deploying mixed methods, simulation and scale-up of evidence into practice.

Reproducibility in biomedical research.

Reproducibility has long been considered a foundation of trustworthy science, yet practices that enable reproducible research remain inconsistently implemented, despite widespread awareness and establishment of policies and guidelines. Biomedical research that cannot be reproduced can lead to resource waste and adversely impact patient safety, public health outcomes, scientific progress and public trust. Rapid advances in technology, scientific dissemination and clinical translation have prompted renewed emphasis on validating research methods and findings to ensure transparency, rigor and cre

Clinical translation of carbon nanomaterials: the clinical translation paradox, regulatory barriers, and future pathways.

Carbon-based nanomaterials (CNMs) have generated intense research interest owing to their exceptional physicochemical versatility and broad biomedical potential. However, despite two decades of rapid innovation, the clinical translation of CNMs remains strikingly limited, owing to a persistent mismatch between performance-driven material innovation and predictability-driven clinical and regulatory demands-an enduring clinical translation paradox that urgently requires systematic resolution. This review examines four representative CNM classes-carbon dots, carbon nanotubes, carbon nanofibers, a

Mapping biomedical hydrogel microspheres through scientometric and topic modeling analysis of emerging nano-enabled design strategies.

Hydrogel microspheres (HMs) are injectable microscale hydrogel platforms formed from crosslinked polymeric networks, with tunable porosity, stiffness, degradation behavior, surface chemistry, and cargo-loading capacity. Recent advances have expanded HMs beyond conventional controlled-release carriers into multifunctional biomedical platforms incorporating micro/nano-architectural designs, nanocomposite components, and biofunctional interfaces. However, the field-level evolution of biomedical HMs, including emerging nano-enabled design strategies, application landscapes, and validation requirem

A future 'rodentopia'? Imagining a good life for rats and mice used in research (and the humans who work with them).

This invited perspective imagines 2066, this journal's centennial. Building on current understandings of rodent cognition, affect and welfare needs, we envision a world with dramatically altered societal attitudes toward rats and mice: widely kept as companion animals (pets), they are valued as intelligent, sociable beings. Scientific validity, animal welfare and human welfare are also considered inseparable priorities, further transforming biomedical animal-use. Within this context, animal-based research now involves two complementary models. Laboratory research persists, under conditions rad

Case report: Novel FHR2 variants in atypical Hemolytic Uremic Syndrome: A case study of a translational medicine approach in renal transplantation.

Atypical hemolytic-uremic syndrome (aHUS) is a severe thrombotic microangiopathy in which kidney involvement is common. aHUS can be due to either genetic or acquired abnormalities, with most abnormalities affecting the alternative complement pathway. Several genetic factors/alterations can drive the clinical presentation, therapeutic response, and risk of recurrence, especially recurrence following kidney transplantation. We report here the case of a 22-year-old man who developed a severe form of aHUS. Renal biopsy revealed thrombotic microangiopathy and features of chronic renal damage. Despi

Perspective: Understanding the Intersection of Climate/Environmental Change, Health, Agriculture, and Improved Nutrition - A Case Study: Type 2 Diabetes.

Efforts to promote health through improved diet and nutrition demand an appreciation of the nutritional ecology that accounts for the intersection of agriculture, food systems, health, disease and a changing environment. The complexity and implications of this ecology is exemplified by current trends and efforts to address nutrition-related non-communicable diseases (NCDs), most prominently type 2 diabetes. The global prevalence of type 2 diabetes continues to rise unabated. Of particular concern is how to address the unhealthy dietary patterns that are contributing to this pandemic in a chang

When the science alone is not enough: embracing our responsibility as science communicators.

The US biomedical research enterprise has led the world in driving medicine forward but now faces unprecedented threats. Systematic efforts to defund and dismantle the academic research infrastructure, compounded by misinformation and questionable public support for science, have already impeded innovation and discouraged students from pursuing careers in research and healthcare, which ultimately will harm patients. This moment calls on us, as scientists and physicians, to effectively communicate the relevance of our research to the public and elected officials and ensure that the value of bas

Single-pixel imaging flow cytometry for biomedical research.

High-throughput single-cell analysis and screening have become essential tools in life science research. Imaging flow cytometry, in particular, enables large-scale image-based profiling of heterogeneous cell populations, allowing statistical analysis of cellular morphology, subcellular features, and functional responses. However, its analytical capability is often limited by the use of conventional two-dimensional (2D) image sensors. In this review, we highlight recent advances in single-pixel imaging flow cytometry, which replaces 2D image sensors with single-pixel photodetectors. This approa

Little Patients Big Discoveries: Potential Pediatric to Adult Neurogastroenterology Translation.

Pediatric neurogastroenterology conditions, including disorders of gut-brain interaction and motility disorders, affect millions of children worldwide. Owing to limited pediatric data, reference ranges and management are often extrapolated from adult studies. We review 4 pediatric neurogastroenterology areas where clinical science may translate and inform adult gastroenterology. Sucrase-isomaltase deficiency can be diagnosed through disaccharidase enzyme testing from duodenal mucosal biopsies. Interleukin-1 blockade in cardiac sarcoidosis: study design of the multimodality assessment of granulomas in cardiac sarcoidosis: Anakinra Randomized Trial (MAGiC-ART).

BACKGROUND: Sarcoidosis is an inflammatory disease characterized by the formation of granulomas, which involve the heart in up to 25% of patients. Cardiac sarcoidosis can lead to life threatening arrhythmias and heart failure. While corticosteroids have been used as a treatment for over 50 years, they are associated with hypertension, diabetes, and weight gain, further increasing cardiovascular risk. Interleukin-1 (IL-1) is the prototypical proinflammatory cytokine that works to activate the nuclear transcription factor NF-kB, one of the targets of glucocorticoids. IL-1 also plays an important
