Twenty cases of carpal bone dislocation were encountered during a 7-year period, with an average of 27 months of follow-up. There were ten types of dislocation in this series; the most common type was transscaphoid perilunate dislocation which was seen in nine cases. In addition, there were two scaphoid subluxations; one volar lunate dislocation; one dorsal perilunate dislocation; one scaphoid perilunate dislocation; one hamate and pisiform dislocation; one transhamate pisiform dislocation; one trapezoid dislocation with dislocation of carpometacarpal joints two to five; one dislocation of the

Metastasis to the bones of the hand and wrist is not common, and its discovery may reveal an advanced primary tumor located centrally. Clinically, hand metastasis is hard to differentiate from other more common hand pathologies. Its rarity, coupled with a lack of unique clinical manifestations, makes hand and wrist metastasis difficult to diagnose. However, its diagnosis is critical to initiate an appropriate course of treatment. We present a patient in whom lung carcinoma metastasis to the trapezium was definitively diagnosed upon surgical management of symptoms that were consistent with thum

BACKGROUND: Thumb carpometacarpal (CMC) joint arthroplasty is a common procedure in the surgical management of symptomatic thumb basal joint arthritis. Following trapeziectomy, a number of suspensionplasty techniques are often used, but limited comparative evidence exists between these techniques. The central aim of this study was to prospectively compare the outcomes of 2 suspensionplasty techniques following trapeziectomy: suture button (TightRope) versus ligament reconstruction and tendon interposition (LRTI).
METHODS: Prospective data were collected on 112 consecutive patients with Eaton

PURPOSE: We investigated the complication profile and effectiveness of arthrodesis, trapeziectomy without suspensionplasty or ligament reconstruction (S/LR), trapeziectomy with S/LR, and joint replacement (JR) for the management of base-of-thumb arthritis. We included cases of interposition material placement within the trapeziectomy without the S/LR group.
METHODS: A systematic search of PubMed, Embase, and Web of Science was performed from their date of inception to November 2024. The primary complication outcome of interest was the reoperation risk, whereas the primary efficacy outcomes of

An accessory dorsal-distal portal (D-2) was developed to improve the visualization of the medial aspect of the trapezium during an arthroscopic trapeziectomy. This portal is ulnar to the extensor pollicis longus tendon and 1 cm distal to the V-shaped cleft at the juncture of the index and thumb metacarpal bases. The entry site is marked before the tourniquet is elevated to allow palpation or Doppler ultrasound of the radial artery in the anatomic snuffbox. A 22-gauge needle is inserted with proximal, radial, and palmar angulation to penetrate the joint space, which is viewed from either the 1-

The most important problem of trapezial dysplasia with thumb metacarpal instability is of bony origin. Together with the progressive capsuloligamentous decompensation it evolves in a progressive adduction deformity of the thumb metacarpal secondary to the dysplasia of the trapezium with its increased articular slope. The addition-subtraction osteotomy restores the anatomy combining two techniques: an abduction-extension osteotomy of the first metacarpal to correct the axis of the first metacarpal and an opening wedge osteotomy of the trapezium to reorientate the trapezial saddle. We present a

Simultaneous multiple injuries of the thumb ray are rare. An unusual case of a simultaneous fracture of proximal and distal phalanges, trapezium and scaphoid is reported. The patient had an acceptable result six months after injury. This type of injury of the thumb ray may be a specific form of injury pattern caused by the mobile phone.

INTRODUCTION: Fibroblastic rheumatism (FR) is a rare disease characterized by polyarthralgia associated with painless subcutaneous nodules, sclerodactyly and finger contractures.
CASE REPORT: A 45-year-old man presented with peripheral vascular disease, polyarthralgia and subcutaneous nodules. The diagnosis of FR was confirmed by histological examination of a biopsy sample of a nodule, which showed myofibroblast proliferation. Radiographs of both hands showed bilateral erosion of the distal and proximal interphalangeal joints, of the right trapezium bone and acro-osteolysis of the third phala

This study describes the age and sex distribution, trauma mechanism, treatment and influence of patient-reported outcomes of 6542 carpal fractures from the Swedish Fracture Registry (SFR). The most commonly fractured carpal bone was the scaphoid (60%), followed by the triquetrum (25%), hamate (5%) and trapezium (4%). The mean age at injury was 41 years, and 69% of patients were male. The age and sex distribution of carpal fractures differed substantially between the different carpal bones. Men were more likely to sustain a carpal fracture after high-energy trauma and were more likely to be tre

The carpal arch of the wrist is formed by irregularly shaped carpal bones interconnected by numerous ligaments, resulting in complex structural mechanics. The purpose of this study was to determine the three-dimensional stiffness characteristics of the carpal arch using displacement perturbations. It was hypothesized that the carpal arch would exhibit an anisotropic stiffness behavior with principal directions that are oblique to the conventional anatomical axes. Eight (n=8) cadavers were used in this study. For each specimen, the hamate was fixed to a custom stationary apparatus. An instrumen

OBJECTIVE: To determine whether trapezioscaphoid (TS) joint alterations are associated with calcium pyrophosphate dihydrate (CPPD) crystal deposition disease and, if so, to determine the nature of these alterations.
METHODS: Radiographs of 160 wrists with evidence of chondrocalcinosis were evaluated with regard to TS joint abnormalities, and findings were compared with a similar number of radiographs in an age and sex matched control population in whom no evidence of chondrocalcinosis or other calcification in the wrist was seen. Two radiologists in consensus recorded radiographic findings in

Scaphoid fractures are the most prevalent type of carpal bone fractures. High-spatial-resolution sonography detects direct signs of scaphoid fractures such as scaphoid cortical disruption; nevertheless, indirect signs such as radiocarpal effusion and scapho-trapezium-trapezoid effusion can also be visible. The diagnosis is performed when both direct and indirect signs of scaphoid fracture are presented. The presence of indirect signs alone is not enough to complete the diagnosis, for which more advanced imaging modalities are usually required. Here, we review the anatomy of the scaphoid, the c

Basal osteoarthritis of the thumb is extremely common and causes pain and difficulty with essential 'pinching' tasks such as writing and dressing. It has been shown, in high-quality studies, that physiotherapy can result in clinically important improvements in pain and function, but the delivery of nonoperative treatment currently varies considerably throughout the NHS in the UK. Trapeziectomy is an effective, simple, and low-cost procedure, and the most common of surgical treatment for basal osteoarthritis of the thumb in the UK. However, recovery can be lengthy and complications include subs

BACKGROUND AND AIM: Rhizarthrosis represents 10% of all arthritic manifestations and its prevalence increases with age and in women. The hyperextension of the metacarpophalangeal joint (MCPj) is consequent to a progressive dorsoradial subluxation of the trapeziometacarpal joint (TMj) in advanced osteoarthritis. The aim of this retrospective study is to evaluate the clinical and functional results of 32 patients affected by advanced rhizarthrosis who underwent to modified Burton-Pellegrini's trapeziectomy in absence of surgical correction of MCPj hyperextension in order to understand when this

Trapeziometacarpal arthrodesis is a beneficial surgery for young and higher-demand patients who have trapeziometacarpal arthritis. Surgical techniques can vary, but certain aspects of the surgery are critical, including bony preparation and positioning of the thumb in space. Reported outcomes are generally favorable, although the quality of evidence is poor. There are no comparison studies assessing different arthrodesis techniques, and only a few comparing arthrodesis to the trapeziometacarpal arthroplasty procedures. Most published results are case series with retrospectively collected data.

Osteoarthritis of the scaphotrapeziotrapezoidal joint is frequent but often pain-free or well tolerated with non-surgical treatment. Surgical options are numerous and none seems to have clear evidence of superiority. In addition to well-established procedures, such as scaphotrapeziotrapezoidal joint arthrodesis, distal scaphoid resection and trapeziectomy, more recent surgical techniques have been reported. These include scaphotrapeziotrapezoidal arthroplasty and arthroscopic treatment. This article provides an overview of the procedures and their outcomes that can be proposed for isolated sca

The intraosseous vascular anatomy of the carpal bones of 25 fresh cadaver limbs was studied by injection and Spalteholz clearing techniques. The carpal bones were classified into three general groups based on the size and location of nutrient vessels, the presence or absence of intraosseous anastomoses, and the dependence of large areas of bone on a single intraosseous vessel. Group I included the scaphoid, capitate, and 20% of the lunates in this series. Each had large areas of bone dependent on a single intraosseous vessel and was considered at greater risk to develop avascular necrosis foll
