Outcomes of Surgical Management of Intraosseous Ganglia of the Carpal Bones: A Case Series.

BACKGROUND: Intraosseous ganglia of the carpal bones are uncommon with sparse publications to guide treatment. The purpose of this study was to review a single-institution experience to determine the outcomes of patients with surgically treated intraosseous carpal ganglia. METHODS: Skeletally mature patients with intraosseous carpal ganglia between 1995 and 2016 treated operatively were identified. Demographic information, clinical data, and radiographic studies were evaluated. RESULTS: Thirty-three ganglia in 31 patients were identified. Intraosseous ganglia were located in the lunate (23),

The third metacarpal boss.

A swelling and ache on the dorsum of the wrist in relation to the base of the 3rd metacarpal bone may indicate a bony lesion or an enlarged styloid process. It may also be due to an old fracture of the process or an accessory ossicle (os styloideum). Approximately half the 14 patients (15 wrists) had a history of an injury, usually a hit with the fist. Recurrent occupational strain may also produce symptoms at this site. Excision of the metacarpal base with the adjacent capitate or trapezoid, and repair of the periosteum and extensor tendon, will remove the lump and usually relieve symptoms.

[Transtrapezoid carpal dislocation: a case report].

Fracture dislocations of the STT joint are extremely rare. We present the case of a 50-year-old worker, who sustained a crush injury of his left hand with a dorsal fracture dislocation of the trapezoid bone and the associated second metacarpal bone at the STT, and a concomitant compartment syndrome of the hand. The fracture was reduced immediately and compartment release was performed, followed by percutaneous stabilisation using three K-wires. Material removal after six weeks was followed by intensive physical therapy. At the final follow-up examination after three months, we observed a good

Various Diagnostic and Treatment Pitfalls of Combined Fracture Dislocations of Trapezoid and Multiple Carpometacarpal Joints.

We report a rare case of combined fracture dislocations of the trapezoid and multiple carpometacarpal joints that became chronic due to inappropriate treatments. Although an acceptable clinical result was obtained with limited intercarpal fusion, correct diagnosis and initial treatments including anatomical reduction and fixation for obtaining a good clinical result, are important for complex trapezoid injury.

A new surgical technique for the ligament reconstruction of the trapeziometacarpal joint.

Isolated traumatic dislocation of the trapeziometacarpal joint is rare compared with fracture-dislocation of the joint. The mechanism of injury is usually axial loading on a flexed thumb metacarpal, leading to dorsal dislocation of the joint. Closed reduction with immobilization is an acceptable method of treatment if the joint is stable after the reduction. Otherwise, early ligamentous reconstruction is recommended to reduce the likelihood of secondary arthritis. Various surgical techniques have been used to reestablish the ligamentous integrity of the joint; however, these techniques usually

Intraosseous Ganglion Spanning the Scaphoid and Lunate: A Case Report.

Intraosseous ganglions (IOGs) are actually quite common but one spanning two adjacent carpal bones is uncommon. We report a case with an IOG spanning the scaphoid and lunate, which was treated surgically. A 16-year-old right-handed female noticed left wrist pain that started spontaneously five years previously. Physical findings indicated carpal instability in the left wrist. Posteroanterior radiographs of the left wrist showed small cysts in the lunate and scaphoid, while the lateral radiograph revealed volar flexion of the lunate. Bone curettage was performed using sharp curettes, and due to

Chondrocalcinosis of the wrist.

Calcium Pyrophosphate Dihydrate Deposition (CPDD) disease has characteristic radiographic features including soft tissue calcification, joint space narrowing, bone sclerosis, subchondral cyst formation without osteophyte formation, and large intraosseous geodes. Triangular fibrocartilage calcification is frequently found and isolated scapho-trapezio-trapezoid (STT) arthritis is specific for CPDD. Distal radio-ulnar (DRUJ), isolated midcarpal joint and piso-triquetral joint involvement also occur. 127 patients were reviewed. Seventy-eight had symptomatic STT joint arthritis, for which 36 underw

The arterial anatomy of the human carpus. Part II: The intraosseous vascularity.

The intraosseous vascular anatomy of the carpal bones of 25 fresh cadaver limbs was studied by injection and Spalteholz clearing techniques. The carpal bones were classified into three general groups based on the size and location of nutrient vessels, the presence or absence of intraosseous anastomoses, and the dependence of large areas of bone on a single intraosseous vessel. Group I included the scaphoid, capitate, and 20% of the lunates in this series. Each had large areas of bone dependent on a single intraosseous vessel and was considered at greater risk to develop avascular necrosis foll

Simultaneous capitate and trapezoid fractures following a motor vehicle accident: A case report.

INTRODUCTION: Carpal fractures are relatively rare, and simultaneous fractures involving both the capitate and trapezoid bones are particularly uncommon, accounting for less than 2 % of all carpal injuries. These fractures usually result from high-energy trauma, such as motor vehicle accidents. Due to the deep anatomical position and strong ligamentous support of these bones, injuries may be subtle and easily missed on standard radiographs. Early diagnosis and appropriate management are essential to avoid long-term morbidity and ensure functional recovery. CASE PRESENTATION: We report the cas

[Rehabilitation of patients with complicated forms of urethral obliterations].

The article describes open surgical modalities for managing patients with severe obliterations of the anterior urethra. The mean age of patients was 70 years. The causes of urethral strictures and obliteration included prostatic adenomectomy, scrotal abscess with pelvic bone osteomyelitis, accidental removal of the bulbar and penile urethra during herniotomy and a prolonged use of an indwelling urethral catheter. Two patients underwent perineostomy (median and trapezoidal). In the third patient, scrotal skin was used to form "scrotal hypospadias" (a modified Johansons operation) and the fourth

Evans Osteotomy Complications.

In 1975, Evans published an article describing the surgical management of the "calcaneo-valgus deformity," pointing out that the deformity was due to relative shortening of the lateral column of the foot. Correction involved "equalizing" both columns by performing an osteotomy in the neck of the calcaneus 1.5 cm from the calcaneocuboid joint, where a trapezoidal wedge of tricortical bone was placed. Although it was considered a success, there were complications, including sural nerve injury, surgical wound dehiscence, undercorrection, and graft subsidence. The osteotomy grew in popularity. Ind

Osteosynthesis Metal Plate System for Bone Fixation Using Bicortical Screws: Numerical-Experimental Characterization.

This study reports the numerical and experimental characterization of a standard immobilization system currently being used to treat simple oblique bone fractures of femoral diaphyses. The procedure focuses on the assessment of the mechanical behavior of a bone stabilized with a dynamic compression plate (DCP) in a neutralization function, associated to a lag screw, fastened with surgical screws. The non-linear behavior of cortical bone tissue was revealed through four-point bending tests, from which damage initiation and propagation occurred. Since screw loosening was visible during the loadi

The carpus in congenital anomalies of the hand.

Congenital anomalies of the carpus associated with congenital hand and upper limb anomalies were investigated from the point of view of the development of the hand. Defects of the carpus were categorized as follows: 1) Radial ray defect: loss of the scaphoid, trapezium and trapezoid. 2) Central ray defect: loss of the capitate and loss of a part of the trapezium and hamate. 3) Ulnar ray defect: loss of the triguetrum, pisiformis and hamate. Deficiencies of the carpus and more distal parts of the hand proved to be secondary to defects of the forearm bones in both the radial and ulnar ray. On th

Trapezoidal Osteotomy for Treatment of Long-Standing Nonunion of Lateral Humeral Condyle Fracture With Cubitus Valgus Deformity.

PURPOSE: Long-term nonunion of the lateral humerus condyle fracture may lead to progressive cubitus valgus, elbow pain and instability, and secondary ulnar neuritis. A number of techniques of osteotomy are available for correction, but each has its disadvantages. The aim of this study was to present a technique of medial trapezoidal osteotomy for correcting nonunion of the lateral humeral condyle with an elbow valgus deformity >20°. METHODS: Eight patients (mean age, 7.5 years) with cubitus valgus, after neglected nonunion of a lateral humeral condyle fracture of greater than 2 years duration

The connective tissue graft wall
technique to improve root coverage and clinical attachment levels
in lingual gingival defects.

The present case report describes the application of the connective tissue graft wall (CTGW) technique for the treatment of deep lingual gingival recessions associated with probing pockets and bone loss. Two deep lingual gingival recessions affecting the mandibular central incisors associated with severe lingual attachment and bone loss were treated. The surgical technique comprised a connective tissue graft (CTG) placed below a trapezoidal-type coronally advanced flap (CAF) acting as a lingual soft tissue wall of the infrabony defect. One year after the surgery, clinically significant root co

Trapezial Design Technique for Autogenous Bone Graft in Esthetic Zone Implant Treatment.

We report bone augmentation for alveolar bone loss at the bottom of the nasal cavity in conjunction with simultaneous implant placement in the same operative field in the esthetic zone. The patient was a 33-year-old man who was referred to us requesting implant treatment after undergoing tooth extraction (#12) due to root fracture. An examination was performed using cone beam computed tomography (CT) and simulation software. The results indicated insufficient volume of labial bone for the requested procedure, especially at the planned site of the implant neck. Therefore, bone augmentation was

Analysis of the capsule and ligament insertions about the acromioclavicular joint: a cadaveric study.

PURPOSE: The purpose of this study was to analyze the capsular and ligamentous insertions about the acromioclavicular (AC) joint to determine the amount of bone that can be removed without destabilizing the joint. METHODS: We dissected 28 cadaveric shoulders. The AC ligament insertions were measured under loupe magnification with a digital caliper on the acromial and clavicular sides on the anterior, posterior, superior, and inferior edges. We measured the distance to the coracoacromial (CA) and coracoclavicular ligaments. In addition, the axial and coronal angle of the AC joint was measured.

Biomechanical influence of plate configurations on mandible subcondylar fracture fixation: a finite element study.

Mandible subcondylar fractures have very high complication rate, yet there is no consensus on suitable plate design for optimal patient outcomes. Our study is aimed at comparing single mini, trapezoid, lambda, strut and double mini plates. A finite-element (FE) model of intact mandible was developed based on healthy CT-scan data, which was further virtually osteotomized and fixated with plates. The cortical and cancellous bones were assigned region-specific orthotropic and heterogenous isotropic material properties respectively. The models were subjected to six load cases representing the mast
