FIA Score: a simple risk index for predicting fatality in aviation crashes.

BACKGROUND: Previous studies have identified a variety of risk factors for occupant fatality in aviation crashes. A simple composite index measuring the risk of fatality in a given crash, however, is lacking. METHODS: The FIA Score is a four-point (0-3) index based on the number of three risk factors for occupant fatality present in a given aviation crash: fire, instrument meteorological condition, and being away from airport. We assessed the validity of this risk index using aviation crash investigation data from the National Transportation Safety Board for the years 1983 to 2005. Sensitivit

Studies on the mechanisms of stress wave propagation in the chest subjected to impact and lung injuries.

To gain some quantitative understanding of the nature and properties of stress wave propagation and the pathological characteristics of lung injuries, pressure values at eight sites in the lungs during an impact period were recorded. The lung injury severity scores in the corresponding regions were determined in 11 dogs, which were subjected to blunt, left lateral impact at a velocity of 16.33 m/sec with a 24.5% compressive response. The results showed that the pressure value at the contact point was the greatest and that pressure decreased significantly from left side to right side. However,

The effect of the 1992 California motorcycle helmet use law on motorcycle crash fatalities and injuries.

OBJECTIVE: To determine the effects of the California motorcycle helmet use law on statewide fatalities and a large sample of nonfatal injuries before and after law implementation. DESIGN: Police reports and death certificates were collected for motorcycle crash fatalities in California for 1991 (prelaw) and 1992 (postlaw). Official counts of registered motorcycles provided a statewide basis for exposure to a motorcycle crash. Autopsy results were collected for fatalities in 11 counties. Hospital records were reviewed for nonfatal injuries in 28 hospitals in 10 of the 11 counties. Police repo

Predicting the Trauma and Injury Severity Score (TRISS) in patients admitted to trauma centers after trauma: A modeling study based on data from the French National Programme for Medicalization of Information.

BACKGROUND: Reliable mortality prediction is central to trauma research, quality assessment, and resource allocation. While the French national hospital discharge database (PMSI) provides comprehensive nationwide data, it does not capture clinical parameters required for established severity scores such as TRISS, which is used for predicting in-hospital survival after trauma and necessary for implementing national-level evaluations of health system performance. This study tested whether TRISS could nonetheless be accurately derived from PMSI data. METHODS: We conducted a retrospective study o

Swallowing disorders in trauma patients: impact of tracheostomy.

Tracheostomy is associated with increased aspiration rates, and swallowing disorders have not been well-studied in trauma patients with tracheostomy. Swallowing evaluations were conducted in 224 patients (102 trauma and 122 nontrauma patients). Half of the patients in each group had tracheostomies. Bedside swallow studies were conducted in 40 patients, videofluoroscopy swallow studies in 100 patients, and both studies in 84 patients. chi2, Fisher's exact test, Cramer's V, and descriptive statistics were used for data analysis. Aspiration occurred in 35 per cent (36 of 102) of trauma patients w

Systematic reviews of scores and predictors to trigger activation of massive transfusion protocols.

BACKGROUND: The use of massive transfusion protocols (MTPs) in the resuscitation of hemorrhaging trauma patients ensures rapid delivery of blood products to improve outcomes, where the decision to trigger MTPs early is important. Scores and tools to predict the need for MTP activation have been developed for use to aid with clinical judgment. We performed a systematic review to assess (1) the scores and tools available to predict MTP in trauma patients, (2) their clinical value and diagnostic accuracies, and (3) additional predictors of MTP. METHODS: MEDLINE, EMBASE, and CENTRAL were searched

A Lateral Fluid Percussion Injury Model for Studying Traumatic Brain Injury in Rats.

Traumatic brain injury (TBI) diagnoses have increased in frequency during the past decade, becoming a silent epidemic. The pathophysiology of TBI involves pathophysiological processes affecting the brain, induced by traumatic biomechanical forces resulting in temporary impairment of neurological function. Preclinical models have been generated to recapitulate the mechanical, neuroinflammatory, and behavioral outcomes observed in the clinical setting. The lateral fluid percussion (LFP) model is the most extensively used and well-characterized model of nonpenetrating and nonischemic TBI. The mod

Late versus early reduction in traumatic hip dislocations: a meta-analysis.

AIMS: Traumatic hip dislocations are considered orthopaedic emergencies that are treated with urgent reduction to decrease the rate of osteonecrosis of the femoral head. The aim of our study was to systematically review the literature that compares late (>6 h from the time of injury) to early (<6 h from the time of injury) reduction in all traumatic hip dislocations. MATERIALS AND METHODS: We searched five databases from 1951 to 2016 for studies that evaluated timing of reduction and osteonecrosis of the femoral head in all traumatic hip dislocations. We performed a meta-analysis using a rand

The combined benefits of motorcycle antilock braking systems (ABS) in preventing crashes and reducing crash severity.

OBJECTIVE: Several studies have reported the benefits of motorcycle antilock braking systems (ABS) in reducing injury crashes, due to improved stability and braking performance. Both aspects may prevent crashes but may also reduce the crash severity when a collision occurs. However, it is still unknown to what extent the reductions in injury crashes with ABS may be due to a combination of these mechanisms. METHODS: Swedish hospital and police reports (2003-2012) were used. The risk for permanent medical impairment (RPMI) was calculated, showing the risk of at least 1 or 10% permanent medical

[Expert evaluation of certain immune system disturbances after mild traumatic brain injuries].

The authors present results of evaluation of immune disorders in 47 patients presenting with brain concussion (BC) and in 28 subjects with mild cerebral contusion (MCC). It was shown that in patients of the former group all immune characteristics were totally restored within 3 weeks after the injury. In the latter group, the clinical picture of secondary immunodeficiency with impaired parameters of the immune status persisted for at least 28 days after the injury. It is concluded that the dynamics of immune system disturbances after a traumatic brain injury may be used as an objective criterio

A 6-year analysis of German emergency medical services helicopter crashes.

BACKGROUND: Annually, there are about 80,000 helicopter emergency medical services (HEMS) missions for patients in Germany. In comparison to the commercial transport of passengers, the risk of aviation crashes increases during rescue operations. The aim of this study was to evaluate helicopter crashes related to HEMS in Germany within a 6-year period (1999-2004) and to analyze contributing factors. METHODS: Flight crashes were identified in the annually published flight crash reports of the Federal Agency for Flight Accident Investigation. Data were completed by telephone interview of the ope

Traumatic brain injury and hypopituitarism in children and adolescents: is the problem under-estimated?

Traumatic brain injury (TBI) is the leading cause of death in children. Only recently, the importance of hypopituitarism as a consequence of TBI has been highlighted in adult patients. Data from systematic clinical studies in adults and patients in transition from adolescence to adulthood point to the presence of hypopituitarism in nearly a third of patients hospitalized for TBI. But, no systematic studies on posttraumatic hypopituitarism exist in children and adolescents. Case reports and small series of patients, however, suggest that children are affected to a comparable extent. Since norma

[An anesthesiologist-resuscitator's differential tactics in acute traumatic disease].

The detection of four forms of the course of acute traumatic disease (traumatic shock, acute respiratory failure, acute heart failure, and traumatic coma) leads to the conclusion that the multi pathogenic mechanism responsible for progression of the disease generates a need for differentiating the tactics of an anesthesiologist-resuscitator, beginning from the first hours after injury. Therapeutic tactics may be standardized within each specific form of an acute period of traumatic disease. Implementation of specific tactic programs for anesthesiological and resuscitative care in acute traumat

Mangled extremity severity score in children.

Treatment of the severely traumatized or mangled lower extremity poses significant challenges. The Mangled Extremity Severity Score (MESS) is a scale that uses objective criteria to assist with acute management decisions. Most research on the MESS has been in adults or combined series with few children. The study was performed to investigate the MESS in children exclusively. The MESS was applied retrospectively to 36 patients with grades IIIB and IIIC open lower extremity fractures collected from two level 1 pediatric trauma centers. Patients were divided into limb salvage and primary amputati
