IMPORTANCE: Patent foramen ovale (PFO)-associated strokes comprise approximately 10% of ischemic strokes in adults aged 18 to 60 years. While device closure decreases stroke recurrence risk overall, the best treatment for any individual is often unclear.
OBJECTIVE: To evaluate heterogeneity of treatment effect of PFO closure on stroke recurrence based on previously developed scoring systems.
DESIGN, SETTING, AND PARTICIPANTS: Investigators for the Systematic, Collaborative, PFO Closure Evaluation (SCOPE) Consortium pooled individual patient data from all 6 randomized clinical trials that com

This study aimed to investigate the heterogeneity of treatment effect for intra-aortic balloon pump (IABP) across various lactate trajectories in patients with acute myocardial infarction-related cardiogenic shock (AMICS) supported by veno-arterial extracorporeal membrane oxygenation (VA-ECMO). Retrospective data from the China Extracorporeal Life Support Registry included AMICS patients who received VA-ECMO. The latent class growth model was used to identify distinct lactate trajectories. The primary outcome was in-hospital mortality. Baseline characteristics and outcomes were compared across

BACKGROUND: Heterogenous outcome reporting in non-muscle-invasive bladder cancer (NMIBC) effectiveness trials of adjuvant treatment after transurethral resection (TURBT) has been noted in systematic reviews (SRs). This hinders comparing results across trials, combining them in meta-analyses, and evidence-based decision-making for patients and clinicians.
OBJECTIVE: We aimed to systematically review the extent of reporting and definition heterogeneity.
METHODS: We included randomized controlled trials (RCTs) identified from SRs comparing adjuvant treatments after TURBT or TURBT alone in patie

A novel autologous heterogeneous skin construct (AHSC) was previously shown to be effective versus standard of care (SOC) treatment in facilitating complete wound healing of Wagner 1 diabetic foot ulcers in an interim analysis of 50 patients previously published. We now report the final analysis of 100 patients (50 per group), which further supports the interim analysis findings. Forty-five subjects in the AHSC treatment group received only one application of the autologous heterogeneous skin construct, and five received two applications. For the primary endpoint at 12 weeks, there were signif

We desired to carefully evaluate a novel autologous heterogeneous skin construct in a prospective randomised clinical trial comparing this to a standard-of-care treatment in diabetic foot ulcers (DFUs). This study reports the interim analysis after the first half of the subjects have been analysed. Fifty patients (25 per group) with Wagner 1 ulcers were enrolled at 13 wound centres in the United States. Twenty-three subjects underwent the autologous heterogeneous skin construct harvest and application procedure once; two subjects required two applications due to loss of the first application.

INTRODUCTION: This study assessed the heterogeneous treatment effects (HTEs) of glucagon-like peptide-1 receptor agonists (GLP-1RAs) and sodium-glucose cotransporter-2 inhibitors (SGLT2is) on the risk of Alzheimer's disease and related dementias (ADRD).
METHODS: This target trial emulation study included adults (≥ 50 years) with type 2 diabetes (T2D) and newly prescribed a GLP-1RA, SGLT2i, or other second-line glucose-lowering drugs (GLDs). A doubly robust learning approach was deployed to estimate the risk difference (RD) of ADRD and identify key subgroups.
RESULTS: Both GLP-1RAs (RD, -1.5%

BACKGROUND: Overdose rates in the United States rose dramatically during the coronavirus disease 2019 (COVID-19) pandemic. Well-documented racial and sociodemographic inequities in the impact of the pandemic suggest the potential for similar inequities for overdose. Our objective was to identify subgroups of New York State Medicaid enrollees who experienced the greatest increases in nonfatal opioid overdose risk after the onset of the COVID-19 pandemic.
METHODS: Data are from a retrospective cohort of 1,021,889 people enrolled in New York State Medicaid from 2019 to 2020. To identify subgroup

OBJECTIVES: In patients with acute hypoxemic respiratory failure (AHRF), the use of lower tidal volume ventilation facilitated by veno-venous extracorporeal CO2 removal (vv-ECCO2R) does not improve clinical outcomes. The primary objective of this analysis was to evaluate for differences in indices of systemic inflammation and ventilator-induced lung injury between patients treated with lower tidal volume ventilation facilitated by vv-ECCO2R and standard care. Secondary objectives included an evaluation for heterogeneity of treatment effect.
DESIGN: Substudy of a randomized clinical trial.

This structured narrative review evaluates endovascular thrombectomy (EVT) through an author-developed, population-level treatment-response framework. In this framework, treatment response is the causal contrast between EVT plus best medical care and best medical care alone on a specified patient-centered outcome; it is not the outcome observed after EVT, and it cannot be observed for an individual patient because the untreated counterfactual is unknown. The framework is an evidence-interpretation tool, not a statistical estimator, a validated decision aid, or an individual prediction model. I

Psychedelic-assisted therapies (PATs) can produce rapid and sustained antidepressant effects, yet variability in response remains substantial. Identifying predictors and moderators is essential for optimising patient selection, preparation, and delivery. To map and synthesise the evidence on the predictors of antidepressant response to classic/serotonergic psychedelics administered with psychotherapeutic support in adults with depressive disorders, including treatment-resistant depression. Following PRISMA-ScR principles, we conducted a scoping review of major biomedical and psychology databas

RATIONALE: Evidence-based medicine (EBM) replaced authority with appraisal and has reduced harm. Its own methodologists have documented a persistent gap between what its methods conclude and what its practice does: average effects that are marginal for most treated patients, guidelines that misfit the individual, widespread difficulty converting relative into absolute effects, and no built-in trigger for stopping a treatment that is not helping.
AIMS AND OBJECTIVES: To identify a common source of these problems in the application of population-level estimates to individuals, and to propose a

Psoriasis affecting special anatomical sites (scalp, face, intertriginous areas, and genitals) poses unique treatment challenges and often shows inadequate responses. This study evaluates the real-world efficacy and safety of IL-17 inhibitors in treating psoriasis affecting special sites, and identifies clinical determinants for achieving PASI75 and PASI90 responses. A retrospective cohort of 1,469 patients receiving IL-17 therapy was analyzed.Generalized linear models, univariate and multivariate logistic regression analyses were used to identify clinical factors influencing the PASI75 and PA

Heterogeneity in treatment responses to standard treatment regimens poses a critical challenge in clinical practice, compounded by the unobservability of counterfactual outcomes. To address this, we present temporal causal precision medication (TCPM), a generalist framework translating routine physiological indicators into personalized treatment strategies. Central to TCPM is the treatment-free physiological profile (TFPP), a representation of heterogeneous physiological states trained using counterfactual prediction and adversarial learning to model patient's intrinsic, unbiased physiological

Evidence-informed infectious disease policy requires estimates of the health effects of infections. However, pathogenic variation, whereby the risk of adverse outcomes depends on the strain-specific characteristics of the pathogen, might complicate the study of many infectious agents. Here, we consider the interpretation of epidemiologic studies of infectious diseases when there is such heterogeneity in strain-specific effects and when information on strain composition is unavailable. We use potential outcomes and causal inference theory for analyses in the presence of multiple versions of tre

Population heterogeneity in disease biology, drug exposure and treatment response is often addressed only after development has progressed, limiting generalizability and increasing late-stage risk. We propose an anticipate-verify-influence framework to integrate clinically relevant variability from research through development. The approach combines diverse human-derived models, real-world data, model-informed drug development and AI/ML to identify drivers of heterogeneity, test their clinical relevance early and translate validated findings into trial design, dosing and patient selection. SYNOPSIS: Decades of research have not identified a consistently superior treatment for musculoskeletal (MSK) pain conditions. Across trials, interventions such as exercise therapy and manual techniques demonstrate modest, broadly equivalent average treatment effects, likely mediated by overlapping, nonspecific, and contextual mechanisms. Treatment pluralism provides a practical way to interpret this evidence. It acknowledges that while treatments may appear comparable at the group level, substantial heterogeneity in treatment effects means that individual patients can respond quite differentl

We present a novel framework (TS+TT) to nest a target study (TS) within a target trial (TT) for evaluating the effects of interventions on disparities. The TS component grounds the measurement of disparity in ethical assumptions, based on the concept of allowability, and anchors it to an explicit population within calendar time. It specifies an enrollment plan of stratified sampling of eligible persons to yield a sample where social groups are distributionally similar on covariates deemed allowable for measuring disparity. Within this enrolled sample, the TT component specifies randomization o

Biologic therapies have revolutionized psoriasis management, yet treatment responses remain highly heterogeneous. A distinct subgroup of patients, termed super responders (SRs), achieves exceptionally rapid, complete, and sustained responses to biologic therapies. Understanding this phenotype is critical for advancing precision medicine in psoriasis. This review summarizes the latest advances in SRs, focusing on definitions, predictors, and therapeutic implications. Definitions of SRs vary widely across studies, differing in both temporal criteria and efficacy endpoints. Meanwhile, emerging ev

BACKGROUND: In the Handling Oxygenation Targets in the Intensive Care Unit (HOT-ICU) trial, a lower (8 kPa) vs a higher (12 kPa) PaO2 target did not affect mortality amongst critically ill adult patients. We used Bayesian statistics to evaluate any heterogeneity in the effect of oxygenation targets on mortality between different patient groups within the HOT-ICU trial.
METHODS: We analysed 90-day all-cause mortality using adjusted Bayesian logistic regression models, and assessed heterogeneous treatment effects according to four selected baseline variables using both hierarchical models of su
