Microvascular decompression versus percutaneous procedures for typical trigeminal neuralgia: personal experience.

This study includes 127 patients treated for typical trigeminal neuralgia, 74 of whom underwent a percutaneous microcompression (PMC), 33 a radiofrequency thermocoagulation (RFT) and 20 were explored for microvascular decompression (MVD). A mean follow-up of 24 months was reached by all the groups. Pain relief was evaluated as an 'all or none' effect. At hospital discharge, pain had disappeared in 93.2% of the patients treated with PMC, in 81.8% of those treated with RFT, and in 85% of the MVD group. At 24 months the effect was maintained by 57.2% of the PMC group, 57.6% of the RFT, and by 75%

Distribution of pituitary adenylate cyclase activating polypeptide (PACAP) immunoreactive elements in the brain stem of rats studied by immunohistochemistry.

In the present work the distribution of pituitary adenylate cyclase activating polypeptide (PACAP) immunoreactive elements in rat brain stem were described using immunohistochemistry. The following structures were PACAP immunoreactive: 1. The dorsomedial and ventrolateral cell columns of the motor nuclei of cranial nerves. 2. Primary somatosensory cells in the mesencephalic nucleus of the trigeminal nerve and central axons of the branchial cranial nerves in the spinal trigeminal tract. 3. Visceral afferent fibers in the solitary tract and cell bodies in the dorsal motor nucleus of the vagus. 4

Common input of the cranial motor nuclei involved in phonation in squirrel monkey.

Our aim was to locate brain regions projecting to all cranial motor nuclei involved in phonation simultaneously, that is, the ambiguus, trigeminal motor, facial, and hypoglossal nuclei. For this purpose, four squirrel monkeys (Saimiri sciureus) were injected with horseradish peroxidase, each of the four nuclei in a different animal. Those regions retrogradely labeled in all four cases then were injected in another 29 animals with [3H]leucine for anterograde tracing. We found that the only region connected directly with all phonatory motor nuclei is a restricted portion of the pontine and medul

Restoration of the Topological Organization of the Trigeminal System Following Trigeminal Nerve Root Injury in the Lamprey.

Two issues were examined regarding the trigeminal system in larval lampreys: (1) for normal animals, double labeling was used to confirm that the trigeminal system has a topological organization; (2) following trigeminal nerve root transections, double labeling was used to test whether the topological organization of the trigeminal system is restored. First, for normal animals, Alexa 488 dextran amine applied to the medial oral hood (anterior head) labeled trigeminal motoneurons (MNs) in the ventromedial part of the trigeminal motor nuclei (nVm) and axons of trigeminal sensory neurons (SNs) in

GABAAergic inhibition or dopamine denervation of the A11 hypothalamic nucleus induces trigeminal analgesia.

Descending pain-modulatory systems, either inhibitory or facilitatory, play a critical role in both acute and chronic pain. Compared with serotonin and norepinephrine, little is known about the function of dopamine (DA). We characterized the anatomical organization of descending DA pathways from hypothalamic A11 nuclei to the medullary dorsal horn (MDH) and investigated their role in trigeminal pain. Immunochemistry analysis reveals that A11 is a heterogeneous nucleus that contains at least 3 neuronal phenotypes, DA, GABA, and alpha-calcitonin gene-related peptide (α-CGRP) neurons, exhibiting

Numbness matters: a clinical review of trigeminal neuropathy.

AIM: Trigeminal neuropathies are a group of clinical disorders that involve injury to primary first-order neurons within the trigeminal nerve. We review the spectrum of etiologies underlying both painful and non-painful trigeminal neuropathies, with attention to particularly dangerous processes that may elude the clinician in the absence of a meticulous evaluation. Complications and management issues specific to patients with trigeminal neuropathy are discussed. METHODS: Retrospective literature review. RESULTS: Facial or intraoral numbness, the hallmark of trigeminal neuropathy, may represe

The role of the trigeminal sensory nuclear complex in the pathophysiology of craniocervical dystonia.

Isolated focal dystonia is a neurological disorder that manifests as repetitive involuntary spasms and/or aberrant postures of the affected body part. Craniocervical dystonia involves muscles of the eye, jaw, larynx, or neck. The pathophysiology is unclear, and effective therapies are limited. One mechanism for increased muscle activity in craniocervical dystonia is loss of inhibition involving the trigeminal sensory nuclear complex (TSNC). The TSNC is tightly integrated into functionally connected regions subserving sensorimotor control of the neck and face. It mediates both excitatory and in

Uncommon syndromes of cerebellar vermis aplasia. I: Joubert syndrome.

Necropsy findings are reported for a case of Joubert syndrome (familiar aplasia of cerebellar vermis with episodic hyperpnea, abnormal eye-movements, ataxia and retardation). The findings consisted of an almost total aplasia of the cerebellar vermis; dysplasias and numerous heterotopias of cerebellar nuclei; an almost total absence of pyramidal decussation; and anomalies in the structure of the inferior olivary nuclei, the descending trigeminal tract, solitary fascicle and of the dorsal column nuclei. The lesion resembled the Dandy-Walker malformation or simple aplasia of the cerebellar vermis

Association of trigeminal neuralgia with multiple sclerosis: clinical and pathological features.

The clinical characteristics of facial pain in patients with trigeminal neuralgia (TN) and associated multiple sclerosis (MS) were studied. Among 900 patients with TN 22 (2.4%) had associated MS. Sixteen patients had typical TN, while six had atypical TN. Facial pain commenced at a younger age (P less than 0.005) and a significantly higher proportion had bilateral facial pain (P less than 0.01) in the group with MS compared to TN patients without MS. In three patients TN was the first manifestation of MS, in the remainder facial pain occurred on an average of 12 years after onset of MS. Sevent

Afferent and efferent connections of cerebellar lobe C1 of the mormyrid fish Gnathonemus petersi: an HRP study.

The corpus of the highly developed cerebellum of the weakly electric fish Gnathonemus petersi is differentiated into four lobes, numbered C1 to C4. The present paper deals with the extrinsic connections of the rostralmost lobe C1. Relevant nuclei were studied in normal histological material and HRP injections were placed in lobe C1, the neighbouring pedunculus valvulae, and the brainstem. The largest number of afferents to lobe C1 originates from the nucleus lateralis valvulae, a large nucleus of tightly packed small cells in the dorsal midbrain tegmentum. In Gnathonemus this nucleus encompass

[Origin of olfactory and rhinosensory evoked cortical potentials in diseases of the central nervous system].

This is the first report to be published on olfactory evoked potentials in patients with well-defined lesions of the central nervous system and the trigeminal nerve. Absence of olfactory evoked potentials is seen in post-central and parietotemporal lesions. The first peak of the so-called olfactory evoked twin potential is absent in lesions of the basal nuclei and sectioning of the trigeminal or ophthalmic nerve, whereas there is no second peak in subcortico-frontal and cortico-temporal lesions. Tumours of the corpus callosum and sectioning of the maxillary and mandibular nerves do not disturb

Quantitative light microscopic autoradiographic localization of cholinergic muscarinic receptors in the human brain: brainstem.

We have investigated the localization of muscarinic cholinergic receptors in the brainstem of eight patients free of neurological disease following quantitative autoradiography of microtome sections of postmortem tissue labeled in vitro with N-[3H]methyl scopolamine as a ligand. Receptor densities were quantified by microdensitometry with the aid of a computer assisted image analysis system. Our results reveal a heterogeneous distribution of receptor sites. High concentrations of muscarinic cholinergic receptor sites were associated with many nuclei and areas of the brainstem including the nuc

Tic douloureux, Parkinson's disease and the herpes connection.

Several cases encountered in psychiatric practice are described in which herpetic involvement of the trigeminal nerve appeared to be crucial to the pathophysiology of the patient's illness. A brief history of herpes is discussed in terms of the anatomy and physiology of the trigeminal nerve. Particular attention is given to the effects of intraneuronal herpes on gasserian ganglion and spinal nuclei discharge thresholds, as well as viral pathways directly impacting the substantia nigra. Evidence characterizing Parkinson's disease as one of several important manifestations of CNS herpes is cover

Latent herpes simplex virus trigeminal ganglionic infection in mice and demyelination in the central nervous system.

Mice were inoculated with herpes simplex virus (HSV) type 1 by gently scraping the skin of the nose with a fine needle. About 80% of the animals developed latent inapparent HSV infections in trigeminal ganglia. Virus was demonstrable for at least 6 months post inoculation (p.i.) by cocultivation of ganglionic tissue with GMK cells. Histologically, trigeminal ganglia revealed infiltrations of inflammatory cells even 6 months p.i. In addition, lesions occurred in the brainstem corresponding to the entry of trigeminal roots, trigeminal tracts and nuclei. Inflammatory cell infiltration, disruption

[Partial vertical nucleotomy. A neurosurgical intervention in some forms of therapy-resistant facial neuralgia].

The partial vertical nucleotomy is presented as a new neurosurgical method for the treatment of therapy-resistant pain in the spreading area of the N. trigeminus. Strict indications are laid down which are mainly restricted to therapy-resistant pain in the above mentioned spreading area in the presence of diffusely growing malignant tumours, after traumas and infections and predominantly in forms of idiopathic trigeminal neuralgia that has been treated without any results over a period of many years. In the experimental part of the article; material and method as well as results of anterograde

Effects of substance P on nociceptive and non-nociceptive trigeminal brain stem neurons.

As little information is available on the chemistry of synaptic transmission in trigeminal brain stem nuclei, an iontophoretic study was done on the effects of glutamate and substance P on single neurons in trigeminal nuclei oralis and caudalis in cats anesthetized with chloralose and paralyzed. The neurons were additionally studied for their responses to natural noxious and innocuous cutaneous and intraoral stimuli as well as to bipolar stimulation of the ipsilateral and contralateral canine tooth pulps, the exposed infraorbital and superior laryngeal nerves and forepaw. Differential activation of proapoptotic molecules between mouse and rat models of distal motor trigeminal denervation.

BACKGROUND: We previously developed a rat trigeminal motor neuron axotomy model involving masseter and temporal muscle resection to study pathological changes of the central nucleus after peripheral nerve injury caused by oral surgery. Because motor neurons are reported to be more vulnerable to axotomy in mice than rats, we compared the degeneration process of the trigeminal motor nucleus in the rat model with a similar mouse model. METHODS: We removed masseter and temporal muscles of adult mice or rats. Animals were sacrificed at 3, 7, 14, 28, 42, and 56 days post-operation, and the trigemin

Neural mechanisms underlying somatic tinnitus.

Somatic tinnitus is clinically observed modulation of the pitch and loudness of tinnitus by somatic stimulation. This phenomenon and the association of tinnitus with somatic neural disorders indicate that neural connections between the somatosensory and auditory systems may play a role in tinnitus. Anatomical and physiological evidence supports these observations. The trigeminal and dorsal root ganglia relay afferent somatosensory information from the periphery to secondary sensory neurons in the brainstem, specifically, the spinal trigeminal nucleus and dorsal column nuclei, respectively.
