An upper cervical spine injury producing paraesthesia in the distribution of the mandibular division of the trigeminal nerve.

A case is reported of a 37-year-old man who developed paraesthesia in the distribution of the mandibular division of the trigeminal nerve following an injury to the upper cervical spine during a game of rugby. The central pathways of the trigeminal nerve are discussed and it is proposed that mandibular fibres descended beyond their expected level in the spinal trigeminal nucleus, and that it was these fibres which were compressed in the unstable dislocation at the atlanto-axial joint producing the paraesthesia described. Other remote anatomical explanations for the paraesthesia are considered.

CGRP receptor blockade by MK-8825 alleviates allodynia in infraorbital nerve-ligated rats.

BACKGROUND: Previous data showed that, in rats, anti-migraine drugs (triptans, olcegepant) significantly reduced mechanical allodynia induced by infraorbital nerve (ION) ligation but not that evoked by sciatic nerve (SN) ligation. Whether this also occurs with MK-8825, a novel anti-migraine drug also acting through CGRP receptor blockade (but chemically unrelated to olcegepant) was tested in the present study, which also investigated possible anti-neuroinflammatory effects of this drug. METHODS: Adult male Sprague-Dawley rats underwent unilateral chronic constriction injury (CCI) to either th

Cervicogenic headache: a review of diagnostic and treatment strategies.

Cervicogenic headache is a syndrome characterized by chronic hemicranial pain that is referred to the head from either bony structures or soft tissues of the neck. The trigeminocervical nucleus is a region of the upper cervical spinal cord where sensory nerve fibers in the descending tract of the trigeminal nerve (trigeminal nucleus caudalis) are believed to interact with sensory fibers from the upper cervical roots. This functional convergence of upper cervical and trigeminal sensory pathways allows the bidirectional referral of painful sensations between the neck and trigeminal sensory recep

Trigeminal autonomic pathways involved in nociception-induced reflex cardiovascular responses.

Reflex cardiovascular responses elicited by noxious oro-facial stimulation are well known but the neural pathways that underlie trigeminal cardiovascular reflex reactions remain to be elucidated. In previous studies, we have shown that noxious electrical stimulation of the mandibular incisor in the anesthetized rat elicits increases in mean arterial blood pressure and heart rate (Allen, G.V., Barbrick, B. and Esser, M.J., Trigeminal parabrachial connections: possible pathway for nociception-induced cardiovascular reflex responses, Brain Res., 715 (1996) 125-135). In this study, microinjections

Dietary grape seed polyphenols repress neuron and glia activation in trigeminal ganglion and trigeminal nucleus caudalis.

BACKGROUND: Inflammation and pain associated with temporomandibular joint disorder, a chronic disease that affects 15% of the adult population, involves activation of trigeminal ganglion nerves and development of peripheral and central sensitization. Natural products represent an underutilized resource in the pursuit of safe and effective ways to treat chronic inflammatory diseases. The goal of this study was to investigate effects of grape seed extract on neurons and glia in trigeminal ganglia and trigeminal nucleus caudalis in response to persistent temporomandibular joint inflammation. A Pre-Existing Myogenic Temporomandibular Disorder Increases Trigeminal Calcitonin Gene-Related Peptide and Enhances Nitroglycerin-Induced Hypersensitivity in Mice.

Migraine is commonly reported among patients with temporomandibular disorders (TMDs), especially myogenic TMD. The pathophysiologic mechanisms related to the comorbidity of the two conditions remain elusive. In the present study, we combined masseter muscle tendon ligation (MMTL)-produced myogenic TMD with systemic injection of nitroglycerin (NTG)-induced migraine-like hypersensitivity in mice. Facial mechanical allodynia, functional allodynia, and light-aversive behavior were evaluated. Sumatriptan, an FDA-approved medication for migraine, was used to validate migraine-like hypersensitivity.

Secondary traumatic stress increases expression of proteins implicated in peripheral and central sensitization of trigeminal neurons.

The pathology of migraine, a common neurological disease, involves sensitization and activation of trigeminal nociceptive neurons to promote hyperalgesia and allodynia during an attack. Migraineurs often exhibit characteristics of a hyperexcitable or hypervigilant nervous system. One of the primary reported risk factors for development of a hyperexcitable trigeminal system is chronic, unmanaged stress and anxiety. While primary traumatic stress is a commonly cited risk factor for many pain conditions, exposure to secondary traumatic stress early in life is also thought to be a contributing ris

MRI characteristics of trigeminal nerve involvement in patients with multiple sclerosis.

BACKGROUND: Trigeminal neuralgia and central sensory disturbances are common in patients with multiple sclerosis. The anatomic correlation to lesions in the trigeminal nuclei in the brainstem is not well studied. OBJECTIVE: We studied the anatomical characteristics of demyelinating lesions of the trigeminal complex in the brainstem on MRI in patients with MS and Clinically Isolated syndrome (CIS). MATERIALS AND METHODS: 43 Patients with MS or CIS and MRI lesions in the trigeminal complex in the brainstem were selected from a large database of patients referred for MRI because of trigeminal s

Thiazoline-related innate fear stimuli orchestrate hypothermia and anti-hypoxia via sensory TRPA1 activation.

Thiazoline-related innate fear-eliciting compounds (tFOs) orchestrate hypothermia, hypometabolism, and anti-hypoxia, which enable survival in lethal hypoxic conditions. Here, we show that most of these effects are severely attenuated in transient receptor potential ankyrin 1 (Trpa1) knockout mice. TFO-induced hypothermia involves the Trpa1-mediated trigeminal/vagal pathways and non-Trpa1 olfactory pathway. TFOs activate Trpa1-positive sensory pathways projecting from trigeminal and vagal ganglia to the spinal trigeminal nucleus (Sp5) and nucleus of the solitary tract (NTS), and their artificia

Current Applications of Ablative Therapies for Trigeminal Neuralgia.

Trigeminal neuralgia (TN) is a syndrome consisting of episodic neuropathic facial pain. Although the precise symptoms vary across individuals, TN is typically described as lancinating electrical shocks triggered by sensory stimuli (light touch, talking, eating, and brushing teeth) that improve with antiepileptic medication (especially carbamazepine), remit spontaneously for weeks to months (pain-free intervals), and do not involve any changes in baseline sensation. The etiology of TN has not been definitively established, but many cases are associated with compression of the trigeminal nerve b

Central projections of the ethmoidal nerve of the cat as determined by the horseradish peroxidase tracer technique.

The ethmoidal nerve innervates the nasal mucosa and constitutes the afferent limb of several upper airway protective reflexes. Protective reflexes, such as sneezing, coughing, and apnea, are those reflexes that either expel foreign substances from the respiratory tract or stop them from gaining access to the lungs. The afferents for nasal receptors are thought to be a part of the trigeminal system rather than olfactory in nature. The objective of this study was to localize the cell bodies of these ethmoidal afferents and to trace the central projections of these neurons. Horseradish peroxidase

Differential organization of thalamic projection cells in the brain stem trigeminal complex of the rat.

Following large injections of horseradish peroxidase into the thalamus of the adult rat, labeled neurons can be detected in the contralateral principal sensory nucleus and the subnucleus interpolaris of the spinal trigeminal nucleus. Thalamic projection neurons in the principal sensory nucleus are arranged in discrete aggregates which replicate the organization of the vibrissae and other parts of the face. This cellular organization corresponds very well with the pattern of vibrissae related afferent termination in this region of the nucleus as seen in SDH preparations. There is no such cellul

Effect of trigeminal tractotomy on dental sensation in humans.

Two patients with intractable pain of the head and neck due to cancer underwent trigeminal tractotomy and rhizotomy of the ninth and 10th cranial nerves, as well as an upper cervical dorsal rhizotomy. Postoperatively, cutaneous analgesia ws present ipsilaterally in peripheral portions of the face and the neck, but paramedian facial regions and the oral mucosa were hypalgesic. The procedures had no significant effect on dental pain perception in these patients, as tested electrically and thermally. These results are similar to those in recent reports of dental sensation after trigeminal tractot

Identification of brain areas in mice with peak neural activity across the acute and persistent phases of post-traumatic headache.

BACKGROUND: Post-traumatic headache is very common after a mild traumatic brain injury. Post-traumatic headache may persist for months to years after an injury in a substantial proportion of people. The pathophysiology underlying post-traumatic headache remains unknown but is likely distinct from other headache disorders. Identification of brain areas activated in acute and persistent phases of post-traumatic headache can provide insights into the underlying circuits mediating headache pain. We used an animal model of mild traumatic brain injury-induced post-traumatic headache and c-fos immuno

Central nervous system distribution of the transcription factor Phox2b in the adult rat.

Phox2b is required for development of the peripheral autonomic nervous system and a subset of cranial nerves and lower brainstem nuclei. Phox2b mutations in man cause diffuse autonomic dysfunction and deficits in the automatic control of breathing. Here we study the distribution of Phox2b in the adult rat hindbrain to determine whether this protein is selectively expressed by neurons involved in respiratory and autonomic control. In the medulla oblongata, Phox2b-immunoreactive nuclei were present in the dorsal vagal complex, intermediate reticular nucleus, dorsomedial spinal trigeminal nucleus

Topographic organization of descending projection neurons to the spinal cord of the goldfish, Carassius auratus.

Several neurons from different nuclei give rise to descending spinal tracts and project to various levels in the spinal cord of goldfish, Carassius auratus. These were visualized by retrograde transport of horseradish peroxidase (HRP) administered to the bilaterally transected spinal cord at 6 levels, corresponding to 1st, 5th, 10th, 15th, 20th and 25th segments of the vertebral column. As many as 16 brain nuclei or neuronal aggregations and the Mauthner cells project posteriorly up to the 20th spinal segment. [Progressive spinal muscular atrophy of proximal location in adults].

The authors report two cases of familial progressive muscular atrophy of proximal localization, with late onset of signs which was associated with involvement of the motor nucleus of trigeminal nerve and hypoglossus nerve nucleus as well as with presence of gynaecomastia. Cases of coexistence of gynaecomastia with spinal muscular atrophy are rare in the literature and the present report may serve for explanation whether the association of these signs is accidental or whether it is a separate nosological entity.
