Imaging of Wrist Injuries: A Standardized US Examination in Daily Practice.

The keys to successful ultrasonography (US) of the wrist include knowledge of the relevant anatomy and understanding the biomechanical aspects. A wide spectrum of pathological findings including bone fractures (scaphoid, triquetrum) and ligament lesions (dorsal intercarpal and radiocarpal ligaments, scapholunate ligament) can be caused by an identical traumatic mechanism determined on the basis of the position of wrist at the time of injury. In the setting of wrist trauma, an early diagnosis can minimize the potential for inappropriate or delayed treatment. We describe a practical radiological

Carpal fractures in children.

Carpal fractures in children are uncommon injuries, with the scaphoid being fractured most frequently. Thirty-three children with scaphoid fractures and one child each with fractures of triquetrum, trapezoid, hamate, and capitate were retrospectively reviewed. The mean age was 13 years, 4 months. The mechanism of injury was of low energy in most cases, with > 80% of injuries involving a fall. Localizing tenderness was present in 100% of cases. Of fractures, 97.3% were evident on the initial radiographs. One patient required a bone scan to confirm the diagnosis. All but one scaphoid fracture we

First Reported Case of Unilateral Lunotriquetral Coalition in a Saudi Patient: An Incidental Finding.

Congenital carpal anomalies, ranging from extra bone formation to fusion or absence of existing ones, pose diagnostic challenges. Lunotriquetral coalition (LTC), an abnormal connection between the lunate and triquetrum bones, is a common anomaly. LTC can manifest as incomplete or complete fusion, often incidentally detected on radiographic imaging. Although predominantly congenital, LTC can also be acquired. We present the first reported case of unilateral LTC in a Saudi patient, discovered incidentally during evaluation of a radial collateral ligament injury of the thumb. LTC, the most common

Diagnosis of scaphoid fracture and dedicated extremity MRI.

We evaluated the value of dedicated extremity magnetic resonance imaging (E-MRI) in patients with clinical suspicion of a scaphoid fracture and normal initial radiographs. 52 patients underwent E-MRI within a mean of 4 (2-10) days after trauma. Follow-up radiographs were performed at average 11 (8-14) weeks after trauma, and these images were used as the "gold standard". A T1-weighted turbo gradient echo 3D and a tau short inversion recovery STIR were performed, both in coronal planes. The imaging time was less than 10 min. The images were evaluated independently by two radiologists. E-MRI det

Abnormal type III fusion between lunate and triquetrum: A case report.

INTRODUCTION: The lunotriquetral coalition is the fusion of the lunate and triquetral bones of the wrist and is the most frequent carpal coalition type. It is frequently asymptomatic and discovered as an incidental due to chronic wrist pain, trauma, or fracture. This case aims to present an unusual unilateral lunotriquetral coalition and clinical significance for diagnosis and treatment. PRESENTATION OF CASE: In this case, we presented a 37-year-old male who exhibited with right-sided trauma and wrist pain. He was diagnosed to had a type III complete osseous lunotriquetral coalition on the ri

Effects of dividing the transverse carpal ligament on the mechanical behavior of the carpal bones under axial compressive load: a finite element study.

Transecting the transverse carpal ligament (TCL) is a routine procedure to surgically treat carpal tunnel syndrome; yet, its mechanical consequences on carpal bones are unclear. In this study, our intent was to perform a computational analysis of carpal biomechanics resulting from TCL release. A three-dimensional finite element model of the wrist was constructed, which included all the carpal bones, the distal ulna and radius, the proximal metacarpals and the interosseous ligaments. Cartilage layers of each bone were modeled manually according to anatomic visualization software. The TCL was al

Challenges in Postoperative Compliance and Follow-Up Among Trauma Patients: A Case Report of a Trans-scaphoid Perilunate Dislocation.

The wrist joint is a complex articulation that involves the distal radius, ulna, eight carpal bones (scaphoid, lunate, triquetrum, pisiform, trapezium, trapezoid, capitate, and hamate), and the bases of the metacarpals. Trans-scaphoid perilunate fracture-dislocations are complex wrist injuries, most commonly resulting from high-energy mechanisms involving hyperextension and ulnar deviation. Prompt diagnosis and surgical management are essential in restoring wrist function and stability. This report focuses on the case of a 28-year-old male who sustained a trans-scaphoid perilunate dislocation

Triquetral impingement ligament tear (tilt).

Ulnar-wrist pain is a complex problem whose cause often remains elusive. A diagnostic triad of localized triquetral pain, history of a wrist hyperflexion injury, and normal radiographs, accompanied by ulnar wrist swelling and limited wrist motion, is often attributable to a syndrome we have termed triquetral impingement ligament tear (TILT). The mechanism of injury causes a cuff of fibrous tissue to be displaced distally from the ulnar sling mechanism, resulting in chronic impingement on the triquetrum that causes hyperaemia, loss of articular cartilage, and softening of the bone. Surgical rep

Proximal row carpectomy with capitate osteochondral autograft transplantation.

Proximal row carpectomy (PRC) is an effective treatment option for degenerative or posttraumatic osteoarthritis of the wrist in conditions such as scapholunate advanced collapse, scaphoid nonunion advanced collapse, Kienbock disease, and chronic fracture dislocations of the carpus. PRC involves excision of the scaphoid, lunate, and triquetrum, and relies on the articulation of the remaining capitate from the distal row to articulate with the lunate fossa. PRC offers the potential advantage of greater range of motion, technical ease, and decreased immobilization, and eliminates specific complic

Pisotriquetral arthrodesis for pisotriquetral instability: case report.

A 22-year-old professional downhill mountain bike rider developed increasing posttraumatic pisotriquetral instability. To preserve full function of the pisiform bone, we performed pisotriquetral arthrodesis using a Herbert screw. Ten months after the splint was removed, the patient was free of symptoms and returned to professional downhill mountain biking without limitations. This uncommon method seems to be a feasible treatment strategy and can be recommended in high-demand patients.

The dorsal arterial network of the wrist with reference to the blood supply of the carpal bones.

The gross examination of 50 minium injected specimens showed us the various patterns of the posterior arterial network of the wrist. This is chiefly provided by the radial artery which gives off a constant dorsal carpal branch over the posterior aspect of the trapezium. This vessel crosses the carpus transversely to the inner border of the hand and is joined in half of the cases by the interosseous artery; contribution of the posterior carpal branch of the ulnar artery was more rarely found than usually described (25%). From the arch thus formed are given off short branches which run upwards a

Scaphoid excision and midcarpal arthrodesis: the effect of triquetral excision--a biomechanical study.

PURPOSE: To evaluate the biomechanical alterations that occur after traditional scaphoid excision and midcarpal arthrodesis with and without excision of the triquetrum. The hypothesis of this study was that removal of the triquetrum increases the radiolunate contact pressure. METHODS: We cyclically moved 10 fresh cadaver wrists using a wrist joint motion simulator while measuring the contact pressures between the proximal carpal row and the distal radius and ulna using a dynamic pressure sensor. We acquired data in the intact wrist, after a midcarpal arthrodesis with the scaphoid excised, and

Distribution pattern of MRI abnormalities within the knee and wrist of juvenile idiopathic arthritis patients: signature of disease activity.

OBJECTIVE: The aim of this study in clinically active juvenile idiopathic arthritis (JIA) was to assess the frequency and distribution pattern of synovitis as hallmark of disease and additional soft-tissue and bony abnormalities on MRI in the knee and wrist as two target joints. MATERIALS AND METHODS: MRI datasets of 153 clinically active JIA patients (110 with knee and 43 with wrist involvement) were evaluated independently by two readers for the presence of literature-based imaging features: "synovial hypertrophy," "bone marrow changes," "bone erosions," "tenosynovitis" (only in the wrist),

Comparison of Biomechanical Results about the Effect of Three Surgery Methods in Decompression of Lunate Bone.

Objective  Kienbock's disease is an unusual disorder caused by osteonecrosis and the collapse of lunate bone which leads to pain and a chronic decrease in wrist function. The treatments in this disease aim to relieve pain and maintain wrist function and movement. Various surgical procedures have been recommended for the subjects with Kienbock's disease; however, the main question posed here is which of the selected procedures are more successful in relief of the pressure applied on lunate. Methods and Materials  Computed tomography (CT) scan images of a normal subject were used to create a thr

[Is there pisotriquetral instability after carpal tunnel release? Retrospective study of 55 cases].

The aim of this study was to determine if pisotriquetral instability is present after neurolysis of the median nerve in the wrist. Fifty-five patients who underwent carpal tunnel release between December 2005 and March 2009 were included in this retrospective study. The surgical procedure consisted of cutting the transverse carpal ligament under local anesthesia through an anterior approach. Instability was evaluated clinically and radiologically by measuring the pisometacarpal angle. The mean patient age was 61years and the mean follow-up 42months. Only 9% of patients complained of pain on th
