Nebulised adrenaline to manage a life-threatening complication in a pug with trismus.

A 13-month-old pug with severe trismus because of suspected masticatory muscle myositis underwent anaesthesia for magnetic resonance imaging. When regurgitation occurred, the tongue was pulled from the mouth to enable suctioning but could not be repositioned into the oral cavity as it was not possible to open the mouth. Swelling due to venous congestion and a bite wound were treated using nebulised adrenaline and resolved within 2 hours allowing retraction of the tongue. The use of nebulised adrenaline offers a non-invasive method of managing this potentially life-threatening complication.

Coronoidotomy as treatment for trismus due to jaw-closing oromandibular dystonia.

Oromandibular dystonia is a focal dystonia involving the masticatory and/or tongue muscles. This report describes 2 female patients with jaw-closing dystonia treated by surgical resection of the coronoid process. The patients could not open their mouths due to involuntary jaw-closing muscle contraction. We first treated them by injecting lidocaine and alcohol (muscle afferent block) into the masseter and temporal muscles and then botulinum toxin. However, the trismus improved mildly and transitorily. Therefore, coronoidotomy was done under general anesthesia. The jaw opening increased to 50 mm

Post-traumatic severe trismus caused by impairment of the masticatory muscle.

Restriction of the mouth opening from a pathologic condition outside the temporomandibular joint is called a pseudo- or extra-articular ankylosis. The authors report two cases of severe post-traumatic pseudoankylosis. One case showed fibrous degeneration of the bilateral masseter muscles without a facial bone fracture, which caused severe trismus, a mouth opening of less than 2 mm, and gradually appeared after blunt injuries to the face. The other was a rare case accompanied with the bone formation in the masseter muscle and was diagnosed as myositis ossificans traumatica, which also presented

An association between imaging and acute posttraumatic ear bleeding with trismus.

OBJECTIVES: Computed tomography findings for each of 94 patients with unilateral ear bleeding and trismus correlated with either comminuted temporal bone fracture (26 cases) or bilateral temporomandibular joint fracture (68 cases). STUDY DESIGN: Ninety-four patients with post-traumatic unilateral ear bleeding and 10 asymptomatic adults underwent coronal computed tomography examinations of their temporomandibular joints. Of these, 26 patients with intact temporomandibular joints underwent axial computed tomography of the temporal bones. For 23 of the 94 symptomatic patients, computed tomograph

The lingual sulcus release technique for excision of tongue cancer with trismus.

Operable oral tongue cancers are managed best with surgery followed by adjuvant therapy as and when indicated. The only factor that affects the prognosis, and is under the control of a surgeon, is the tumour margin. Often in cases with trismus, which is prevalent in tobacco and areca nut users, obtaining a clear margin intraorally can be challenging. Approaches described until now to obtain clear margins in these cases have a great impact on the postoperative recovery and morbidity. In our experience, lingual sulcus release is a safe and reproducible technique for these cases. The transcervica

Criterion for trismus in head and neck cancer patients: a verification study.

PURPOSE: Several cut-off points for trismus in head and neck cancer patients have been used. A mouth opening of 35 mm or less is most frequently used as cut-off point. Due to the variation in cut-off points, prevalence, risk factors and treatment outcomes of trismus cannot be studied in a uniform manner. To provide uniformity, we aimed to verify the cut-off point of 35 mm or less. Additionally, we aimed to determine associated covariates with reported difficulties when opening the mouth. METHODS: In a cross-sectional design, we measured the mouth opening in 671 head and neck cancer patients a

Coronoidectomy in maxillary swing for reducing the incidence and severity of trismus--a reminder.

PURPOSE: The maxillary swing approach is a proven method for access to the nasopharynx. However, often, trismus is incurred postoperatively, hampering adequate oral care and follow-up and affecting patients' quality of life. CASE REPORTS: Coronoidectomy was performed in four patients undergoing maxillary swing. Minimal trismus was seen in one patient undergoing repeat irradiation and chemotherapy. After a 1-month period no trismus was observed in the other three patients. CONCLUSION: Coronoidectomy, usually performed in maxillectomy for reducing trismus is a useful adjunct in the maxillary s

[Trismus-pseudocamptodactyly syndrome: presentation and genealogy of a new European case].

The trismus pseudocamptodactyly syndrome also called Hecht syndrome or Dutch-Kentucky syndrome is characterized by loss of the ability to fully open the mouth (trismus) and a finger contracture with progressive flexion of the fingers upon extension of the wrist (pseudocamptodactyly). Deformities of the foot may be associated e.g. hammer and claw toes, tightening of the muscles of the posterior part of the leg producing an equinovarus foot. The authors presents two affected individuals from one family from which nine members had some involvement. The expression of the syndrome may vary. The pat

[Restricted mouth opening as a leading symptom of trismus-pseudocamptodactyly syndrome].

In the absence of pain or dental findings, incapacity to open the mouth fully, concomitant with a typical restriction of movement of the fingers is defined as the trismus-pseudocamptodactyly syndrome. This autosomal dominantly inherited feature is quite rare. So far only affections of families in the USA, the Netherlands, Canada and, recently, Japan, have been reported in the literature. Now, a German case affecting six members of the same family has been found for the first time. An important factor to be considered in the differential diagnosis of painless chronic trismus, this syndrome is d

Mannitol Reduces Pain and Trismus After Impacted Mandibular Third Molar Surgery.

BACKGROUND: The removal of the lower third molar is a routine procedure in oral surgery, yet it often leads to postoperative side effects, particularly inflammation. Despite various interventions explored in prior studies, there is still a need for effective strategies, such as anti-inflammatory substances, to address postoperative side effects. PURPOSE: The purpose of this study is to answer the following clinical question: Does the local injection of 0.9 M mannitol reduce postoperative pain, trismus, and swelling in patients undergoing bilateral symmetrically impacted mandibular third molar

Patterns of Postoperative Trismus Following Mandibulectomy and Fibula Free Flap Reconstruction.

The factors that contribute to postoperative trismus after mandibulectomy and fibula free flap reconstruction (FFFR) are undefined. We retrospectively assessed postoperative trismus (defined as a maximum interincisal opening ≤35 mm) in 106 patients undergoing mandibulectomy with FFFR, employing logistic regression to identify risk factors associated with this sequela. The surgical indication was primary ablation in 64%, salvage for recurrence in 24%, and osteonecrosis in 12%. Forty-five percent of patients had existing preoperative trismus, and 58% of patients received adjuvant radiation/chemo

Severe Spastic Trismus without Generalized Spasticity after Unilateral Brain Stem Stroke.

A 62-year-old female patient diagnosed with left brain stem stroke 2 months ago was admitted to our clinic for rehabilitation. She had no generalized spasticity on both extremities, but could open her mouth only approximately 2 mm between her upper and lower teeth due to severe trismus. On needle electromyography, the left masseter muscle showed paradoxically increased muscle activity during mouth opening. We injected 50 units of type A botulinum toxin (Botox®) into the left masseter muscle, and 20 units into the left temporalis muscle with guidance of ultrasonography. The interincisal distanc

Limited mouth opening and shortened flexor muscle-tendon units: 'trismus-pseudocamptodactyly'.

The association of hand and oro-facial deformities is not uncommon. A case is reported, the literature reviewed and a plea made to discard the term Trismus-Pseudocamptodactyly in favour of the original or a more descriptive title.

Congenital Trismus From Brainstem Dysgenesis: Case Report and Review of Literature.

Trismus refers to any condition inducing limited mouth opening and may present as a result of acquired or congenital pathology. We present the case of a newborn who presented with severe, congenital trismus due to brainstem dysgenesis. We describe the course of his investigations, and a multidisciplinary approach to the management of his care and follow-up. To our knowledge, this is one of the earliest reported cases of congenital trismus attributable to brainstem dysgenesis. A literature review was conducted to provide an overview of the differential pathogenesis as it presents in congenital
