INTRODUCTION: Open reduction and internal fixation of crista supinatoris fractures is required when the elbow is unstable despite appropriate nonoperative management and when a patient is undergoing surgical treatment of a periarticular elbow fracture-dislocation.
STEP 1 SKIN INCISION AND SURGICAL APPROACH: Use a posterior or lateral skin incision according to your preference and then utilize the Kocher interval to access the joint, lateral collateral ligament, and crista supinatoris or, in the setting of a proximal ulnar fracture, use the Boyd interval.

OBJECTIVE: To determine the biomechanical properties of plating options for distal ulna fractures.
METHODS: Fourth-generation ulna artificial bones were osteotomized and fixed with 4 different constructs: 2 locking compression plates (a straight 2.7-mm plate and a 2.4-mm T-plate) with both nonlocking and locking screws. The artificial bones underwent nondestructive tests to determine construct stiffness in flexion/extension and lateral bending. The final testing consisted of cyclical loading in axial torsion until implant failure.

Isolated distal ulna epiphyseal plate injuries are very rare and are often associated with early epiphyseal plate arrest. A 13-year-old boy sustained an isolated minimally displaced Salter-Harris type II fracture of the left distal ulna following a fall from a bicycle. The fracture was reduced, and a long arm plaster cast was applied for four weeks. A 10-year-old girl presented to the emergency department having sustained a fall onto an outstretched left hand while playing soccer. We report an unusual clinical presentation and surgical treatment of a Galeazzi-equivalent fracture in which initial closed treatment failed. A series of 103 acute fractures of the coronoid process of the ulna in 101 patients was reviewed to determine their frequency. The Regan-Morrey classification, treatment, associated injuries, course and outcomes were evaluated. Bilateral stress fractures of the ulna are extremely rare and have only been described in non-adults. In this case report we present a young adult weightlifter who reported bilateral stress fractures of both ulnae after increasing his training frequency and load. Stress fractures of the ulnae are examples of bone stress injuries due to repetitive muscle pull. The diagnosis can be made by radiography, isotope bone scan, CT or MRI. Thorough training analysis and adaptation is essential to treat this rare injury. Return to full activity can take up to three months.

Almost one third of fractures of the distal radius are associated with some complication, either bony or soft tissue. Proper reduction and appropriate stabilization of the fracture are necessary to prevent many of these complications, particularly those involving the articular end of the radius. Although closed treatment is usually satisfactory, an open reduction and internal fixation of the fracture often is indicated in younger patients.
