Isolated compression of the ulnar motor branch due to carpal joint ganglia: clinical series, surgical technique and postoperative outcomes.

The entrapment of the ulnar nerve in Guyon's canal (GC) is a well-known wrist canalicular syndrome which is usually followed by a gradual combination of both sensitive and motor symptomatology. However, GC nerve compression could also cause a pure hand motor dysfunction. This condition, less frequent than the classic Guyon's syndrome, can be difficult to diagnose. Authors report a case series of eight patients affected by isolated compression of the ulnar nerve motor branch, due to piso-triquetrum or triquetro-hamate joint ganglia. Surgical technique and postoperative outcomes are discussed in

[Entrapment neuropathies of the arm].

Compression-induced damage to the peripheral nerves can cause persistent, mostly exercise-related intensified pain syndromes. In the presence of brachialgia, compression neuropathies must always be considered when rendering the differential diagnosis, whether in the scapular region (e.g. suprascapular syndrome) in the elbow (e.g. supinator syndrome) or the hand (carpal tunnel syndrome, distal ulnar tunnel syndrome). Knowledge of neuroanatomy, detailed history of pain and provocative movements and a targeted examination technique including the findings of provocative testing will usually lead t

Cubital tunnel syndrome: anterior transposition as a logical approach to complete nerve decompression.

In it's native position, deep to Osborne's ligament, within the retrocondylar groove of the elbow, the ulnar nerve courses with a significant lever distance posterior to the elbow axis of rotation. In this position, flexion of the elbow places longitudinal traction and local compression forces on the nerve. This biomechanical consideration, as well as variations in anatomy, may potentially contribute to a decrease in the nerve's microcirculation and partial pressure of oxygen, leading to cubital tunnel syndrome. Anterior transposition of the ulnar nerve at the elbow for cubital tunnel syndrome

Ulnar Neuropathy in Cyclists.

The form and function of the cyclist exposes the ulnar nerve to both traction and compressive forces at both the elbow and wrist. Prevention of ulnar neuropathy and treatment of early symptoms include bike fitting, avoidance of excessive or prolonged weight-bearing through the hands, and the use of padded gloves. For persisting or progressive symptoms, a thorough history and physical examination is essential to confirm the diagnosis and to rule out other sites of nerve compression. The majority of compression neuropathies in cyclists resolve after appropriate rest and conservative treatment; h

Ultrasound-Assisted Precise In Situ Decompression for Cubital Tunnel Syndrome.

OBJECTIVE: To explore the effect of locating the ulnar nerve compression sites and guiding the small incision so as to decompress the ulnar nerve in situ on the elbow by high-frequency ultrasound before operation. METHODS: A retrospective analysis was conducted on 56 patients who underwent ultrasound-assisted in situ decompression for cubital tunnel syndrome from May 2018 to August 2019. The patients' average age was 51.13 ± 7.35 years, mean duration of symptoms was 6.51 ± 1.96 months, and mean postoperative follow-up was 6.07 ± 0.82 months. Nine patients had Dellon's stage mild, 39 had stage

Variation of the proximal insertion of the abductor digiti minimi muscle: correlation with Guyon's canal syndrome? Case report and literature review.

We describe the case of an original insertion and course of the abductor digiti minimi muscle on the medial part of the palmaris longus tendon. The anomalous muscle was only present on the left side. Various studies have reported the frequency of anomalous muscles in approximately 22 to 35% of hands and it was in majority an anomalous abductor digiti minimi muscle. The knowledge of this original insertion is important because it can sometimes be correlated with ulnar nerve compression at Guyon's canal. But Guyon's canal syndrome is less common than carpal tunnel syndrome, and the incidence of

Tardy ulnar tunnel syndrome caused by Galeazzi fracture-dislocation: a neuropathy with a new pathomechanism.

We present a case of late-onset ulnar tunnel syndrome following a Colles fracture. The nerve palsy was caused by a vascular branch that stretched over the ulnar head, compressing the nerve and generating friction against the ulnar head when the forearm was rotated. This is the first report of such a pathomechanism.

[Entrapment neuropathies involving the ulnar nerve at the wrist (and into the hand) and the peroneal nerve].

We report our experience with patients who underwent surgery for entrapment neuropathies involving the ulnar nerve at the wrist and into the hand and the peroneal nerve. For the ulnar nerve, the cause of the lesion was identified in all patients, generally a cyst which had developed in the Guyon canal. The patients usually recovered completely. For the peroneal nerve, there was a wide variety of causes, with mucoid cysts frequently involved. Recovery was often incomplete, because of the very marked initial axonal damage. We emphasized the need for rapid diagnosis and surgical treatment.

Neuropathic upper extremity pain: A double-crush scenario.

Neuropathic upper extremity pain has many causes. Cubital tunnel syndrome is the second most common compressive peripheral neuropathy after carpal tunnel syndrome. Entrapment, or compression, of the ulnar nerve at the elbow classically presents with elbow pain, numbness in the ulnar nerve distribution, and weakness in the hand. C8 radiculopathy and various brachial plexopathies can mimic cubital and carpal tunnel syndromes. Neoplastic brachial plexopathy typically is caused by local extension of a primary or metastatic tumor into the brachial plexus. Double-crush syndrome further complicates n

Ulnar Compression Neuropathy Secondary to Ganglion Cyst at the Elbow Joint: A Case Report.

Ulnar neuropathy is a common peripheral neuropathy, with cubital tunnel compression being the second most frequent upper extremity compression syndrome. Ganglion cysts, though rare, can contribute to ulnar nerve compression in the cubital tunnel. Here, we present a case of a 62-year-old farmer with longstanding diabetes and dyslipidemia who experienced progressive pain and numbness in his medial fingers, exacerbated by elbow flexion. Initially misdiagnosed and treated for Hansen's disease. Clinico-radiological evaluation revealed a multiloculated cystic lesion in the posteromedial aspect of hi

[Tunnel neuropathies].

Analysis is made of 261 patients operated on for tunnel neuropathies. Of these, there were 152 men and 109 women aged 15 to 82 years, the mean age being 46 years. Among 22 patterns of neuropathy, there dominated compression of the ulnar nerve in the cubital canal (104 patients) and compression of the median nerve in the carpal canal (76 patients) accounting for 69% of all the cases. 76 patients had two and more tunnel syndromes; double operative interventions were made in 23 patients. 58 patients (22.2%) recovered, 163 (62.75%) improved, no changes were recorded in 40 (15.3%) patients. [Revision surgery for carpal and cubital tunnel syndrome].

BACKGROUND: Carpal tunnel syndrome, a compressive neuropathy of the median nerve at the wrist and cubital tunnel syndrome, a compressive neuropathy of the ulnar nerve at the elbow, are the two most common peripheral nerve compression syndromes. Chronic compressive neuropathy of peripheral nerves causes pain, paraesthesia and paresis. Treatment strategies include conservative options, but only surgical decompression can resolve the mechanical entrapment of the nerve with proven good clinical results. However, revision rates for persistent or recurrent carpal tunnel syndrome is estimated at arou

Cubital tunnel syndrome due to old lateral humeral condyle fractures with gouty stone: A case report.

INTRODUCTION: Cubital tunnel syndrome is a peripheral nerve compression disease, which is caused by the reduction of cubital tunnel space and the compression of ulnar nerve in the cubital tunnel. The main clinical symptoms were numbness and hypoesthesia in the distribution area of ulnar nerve. CASE PRESENTATION: We report a case of complex cubital tunnel syndrome caused by an old lateral humeral condyle fracture combined with gouty stone compression. Before surgery, the patient's left hypothenar, ring finger and little finger were numb, decreased grip strength, positive paper clamp test. And

[Double tunnel syndrome of the upper limb in tophaceous gout. Apropos of a case].

Association of two tunnel syndromes secondary to tophaceous gout is uncommon. This article presents a case of ulnar and carpal tunnel compression. It concerns a 71 year old man with gout and treated for that condition. He presented with paraesthesiae in the fingers and loss of muscular strength in right hand. Physical examination discovered two masses, one in the epitrochlear groove, the other in the olecranon bursa; a severe ulnar palsy and a carpal tunnel syndrome. Neurolysis of both ulnar and median nerves was performed. After 2 years follow-up, paraesthesiae disappeared but atrophy of ulna

Bony Cubital Tunnel Syndrome: A Case Report of Heterotopic Ossification Causing Circumferential Ulnar Nerve Encasement at the Elbow and Review of Current Management.

BACKGROUND: Heterotopic ossification (HO) is a rare but known complication of brain and nerve trauma, orthopedic trauma, and burns. Nerve compression due to HO is extremely rare; "bony cubital tunnel syndrome," or compression of the ulnar nerve at the elbow due to HO, is an unusual presentation that requires special considerations for treatment. CASE PRESENTATION: We present a 50-year-old man who presented to our hospital after vehicular polytrauma with associated car fire and prolonged extrication. He experienced extensive trauma, with all classically described risks for HO. He developed bon

Double entrapment neuropathy of the ulnar nerve at the elbow and the wrist : double crush syndrome?

BACKGROUND: Double crush syndrome refers to a nerve in the proximal region being compressed, affecting its proximal segment. Instances of this syndrome involving ulnar and cubital canals during ulnar neuropathy are rare. Diagnosis solely through clinical examination is challenging. Although electromyography (EMG) and nerve conduction studies (NCS) can confirm neuropathy, they do not incorporate inching tests at the wrist, hindering diagnosis confirmation. We recently encountered eight cases of suspected double compression of ulnar nerve, reporting these cases along with a literature review. M

Guyon's canal syndrome caused by mass effect after chronic dog bite hand injury.

Epidermal inclusion cysts are defined as dermal or subcutaneous epithelial cysts which are filled with keratin and lined with true epidermis. Although exceedingly rare, they can lead to pathological conditions, such as nerve compression syndrome. To the best of our knowledge, this is the first documented case of a 59-year-old woman with ulnar nerve compression at the wrist due to dog bite hand injury. An epidermal inclusion cyst was located adjacent to the ulnar nerve, resulting in significant compression. Notably, no direct connection was observed between the cyst and ulnar nerve. Surgical re

Evaluation of the function status of the ulnar nerve in carpal tunnel syndrome.

Many carpal tunnel syndrome (CTS) patients have symptoms in both the median and ulnar digits more frequently than in the median digits alone. This is possibly because of close anatomical contiguity of the carpal tunnel and Guyon's canal, and the high pressure may also affect the latter, causing indirect compression of ulnar nerve fibers. Thus, we evaluated the functional status of the ulnar nerve in patients with CTS in order to investigate the relationship between ulnar nerve impairment and sensory symptoms of the ulnar territory. Electrophysiological studies were conducted in CTS patients an

Ulnar nerve double crush by entrapment of a peri-cubital tunnel ganglion cyst and cubital tunnel: a case report.

BACKGROUND: Double crush syndrome (DCS) is a relatively rare nerve compression syndrome among peripheral nerve compression diseases. However, ulnar nerve double entrapment caused by peri-cubital tunnel ganglion cysts has been rarely reported. CASE PRESENTATION: Here, we present a case of a 54-year-old woman who experienced occasional pain, numbness and paralysis in her right half hand for 1 year. A B-ultrasound of the right elbow initially revealed cubital tunnel syndrome only. Further Magnetic Resonance Imaging (MRI) showed a ganglion cyst near the cubital tunnel.
