[Current evidence on intraoperative ultrasound : A position paper of the German Society for Ultrasound in Medicine (DEGUM)].

Intraoperative ultrasound (IOUS), also potentially as contrast-enhanced (CE)-IOUS, is a versatile tool for surgical decision-making. It enhances intraoperative orientation, enables precise resection guidance and helps prevent complications and follow-up procedures; however, in Germany, IOUS is currently only selectively used, often in nonsurgical disciplines. While this is reasonable in certain scenarios, it reaches its limits when imaging directly influences surgical actions. The clinical benefits of IOUS are well-documented in several specialties, particularly liver surgery, neurosurgery and

The hallucal interphalangeal ossicle: anatomy and basis for ultrasound-guided surgical shaving.

Painful lesions on the plantar aspect of the first interphalangeal joint (IPJ) of the hallux can be attributed to structures called ossicles, nodules, or sesamoids. The aims of the present study were first to verify that ultrasonography (US) is a high-sensitivity tool for diagnosing an interphalangeal ossicle (IO), and second to prove that US-guided-shaving surgery ("milling") is a safe and feasible technique for remodeling the IO. The study is divided into three parts. In the first part, the prevalence of IOs was estimated in 12 cadaver feet using US, anatomical dissection, and fluoroscopy. I

Ultrasound-Guided Musculoskeletal Injections.

Musculoskeletal injections serve a variety of diagnostic and therapeutic purposes, with ultrasonography (US) guidance having many advantages: no ionizing radiation, real-time guidance, high spatial resolution, excellent soft tissue contrast, and the ability to identify and avoid critical structures. Sonography can be cost effective and afford flexibility in resource-constrained settings. This article describes US-guided musculoskeletal injections relevant to many radiology practices and provides experience-based suggestions. Structures covered include multiple joints (shoulder, hip), bursae (i

Ultrasound-guided interventions in primary carpal tunnel syndrome: perineural injection to thread carpal tunnel release.

Carpal tunnel syndrome (CTS), the most common entrapment neuropathy, is compression of the median nerve deep to transverse carpal ligament at wrist. Ultrasonography and electrophysiological study are complementary in the diagnosis and grading of CTS in appropriate clinical settings. The initial management of patients with CTS is conservative with medical therapy and splinting. However, surgical interventions are indicated in patients in whom medical management has failed. With evolution of the concept of safe zone on ultrasonography and identification of the sonoanatomical landmarks of carpal

Practical Imaging Strategies for Intussusception in Children.

OBJECTIVE. Intussusception is the most common cause of intestinal obstruction in young children. Radiology has a key role in its diagnosis and treatment. This systematic review summarizes the currently available evidence for best practices in radiologic management of pediatric intussusception. CONCLUSION. High diagnostic accuracy and lack of ionizing radiation make ultrasound (US) the preferred imaging modality for diagnosing intussusception. For intussusception reduction, fluoroscopy-guided pneumatic enema and US-guided hydrostatic enema are equally dependable and safe techniques. The areas t

Diagnosis of stenosis within the popliteal-femoral venous segment upon clinical presentation with a venous ulcer and subsequent successful treatment with venoplasty.

This case study reports the diagnosis and treatment of a lower limb venous ulcer with abnormal underlying venous pathology. One male patient presented with bilateral varicose veins and a right lower limb ulcer. Upon investigation, full-leg duplex ultrasonography revealed total incompetence of the great saphenous vein in the left leg. In the right leg, duplex ultrasonography showed proximal incompetence of the small saphenous vein, and dilation of the anterior accessory saphenous vein, which remained competent. Incidentally, two venous collaterals connected onto the distal region of both these

USG-Guided Percutaneous Thread Carpal Tunnel Release.

Ultrasonography (USG) is utilized for the diagnostic and interventional management of patients with carpal tunnel syndrome (CTS). The management of patients with severe and/or refractory CTS includes the surgical (open/endoscopic) release of transverse carpal ligament. With the advancements in the resolution of USG, the release of transverse carpal ligament can be done safely with the help of transecting/cutting devices. We discuss the technical report on the principle of percutaneous thread carpal tunnel release with the help of surgical grade thread.

Radiologic investigation and treatment of gastrointestinal fistulas.

Diagnostic radiology has assumed an increasingly prominent role in the diagnosis, investigation, and treatment of gastrointestinal fistulas during the past 15 years. This development largely has been the result of the application of computed tomography and ultrasonography to the diagnosis of intra-abdominal inflammatory processes and the use of these cross-sectional imaging modalities to guide percutaneous abscess drainage by the interventional radiologist. Effective percutaneous techniques have been developed that allow many gastrointestinal fistulas to be managed nonoperatively with less mor

Ultrasound-Guided Occipital Nerve Blocks to Reduce Tinnitus-Associated Otalgia: A Case Series.

Ear pain is mediated by cranial nerves V, IX, and X, as well as branches of C2 and C3, including the occipital nerve. Occipital neuralgia may play a role in the development or worsening of tinnitus and otalgia. The authors reviewed and report 33 cases of ultrasound-guided occipital nerve blocks in patients with tinnitus and otalgia, with postprocedure follow-up intervals of up to 2 years. We found that greater occipital nerve blocks may be a valuable treatment method for these patients.

Transthoracic ultrasonography for the respiratory physician.

Transthoracic ultrasonography is still not utilized to its full potential by respiratory physicians, despite being a well-established and validated imaging modality. It allows for an immediate and mobile assessment that can potentially augment the physical examination of the chest. Ultrasound (US)-assisted procedures can be performed by a single clinician with no sedation and with minimal monitoring, even outside of theatre. The main indications for the use of transthoracic US are: the qualitative and quantitative description of pleural effusions, pleural thickening, diaphragmatic dysfunction

Ultrasound-guided procedures for the management of chronic thoracic back pain: a technical review.

Pain arising from the thoracic region has been reported to be potentially as debilitating as cervical or lumbar back pain, and may stem from a vast number of spinal sources, including zygapophysial, costovertebral and costotransverse joints, intervertebral discs, ligaments, fascia, muscles, and nerve roots. Over the last two decades, the use of ultrasound in interventional spinal procedures has been rapidly evolving, due to the ultrasound capabilities of visualizing soft tissues, including muscle layers, pleura, nerves, and blood vessels, allowing for real-time needle tracking, while also redu

Artisanal model replaces animal use for ultrasound-guided fine needle puncture practice in medical sciences teaching.

Fine needle puncture (FNP) is a widespread technique used to collect cellular samples. Its efficiency can be enhanced by the use of ultrasonography to guide the procedure. Ultrasound-guided FNP is therefore an operator-dependent exam. For this reason, it demands the acquisition of psychomotor skill, ability to recognize structures, and dexterity during the needle puncture. This study describes the development of an artisanal simulator made with gelatin to replace the use of live animals during practical classes in veterinary or medical sciences education. The experimental set consisted of thre
