Beyond cord clamping: Complexities of umbilical cord management after birth.

The benefits of delayed cord clamping have been investigated in multiple studies and supported by various professional associations. Other aspects of umbilical cord management strategies occurring after cord clamping have not been fully thoroughly analyzed. This article will explore and deliberate elements of umbilical cord nonseverance, vascular access management, and blood banking.

Placenta and Umbilical Cord Abnormalities in Stillbirth.

This is a review of the critical role of placental and umbilical cord abnormalities in stillbirth, emphasizing their strong association with adverse pregnancy outcomes. We discuss key lesions-maternal and fetal vascular malperfusion, infectious, inflammatory, genetic, and multiple gestation-related abnormalities-detailing their biology, clinical presentation, and pathologic characteristics. We further highlight the importance of thorough placental examination for determining the cause of stillbirth and guiding future pregnancy management. Standardized criteria, such as the Amsterdam guidelines

The Umbilical Cord and Complications of Twin Gestations.

The rate of twin pregnancies has increased over the last decades, largely because of the ongoing development of assisted reproductive technology and increased maternal age at childbearing. Twins have a higher risk of adverse outcomes during pregnancy and the perinatal period. The prevalence of umbilical cord abnormalities is higher for twin pregnancies compared with singleton pregnancies. Some of these abnormalities are nonspecific to twinning and can also be found in singleton gestations (such as velamentous cord insertion, vasa previa, and single umbilical artery). Other abnormalities are as

Umbilical Cord Prolapse: A Review of the Literature.

IMPORTANCE: Umbilical cord prolapse is a rare occurrence and is a life-threatening emergency for the fetus. These events are unpredictable and unpreventable. Umbilical cord prolapse requires swift diagnosis and management for optimal outcome. OBJECTIVE: The aim of this review is to describe the incidence, risk factor, pathophysiology, diagnosis, and management of this rare but potentially life-threatening event. EVIDENCE ACQUISITION: A PubMed, Web of Science, and CINAHL search was undertaken with no limitations on the number of years searched. RESULTS: There were 200 articles identified, wi

The umbilical cord, preeclampsia and the VEGF family.

INTRODUCTION: The VEGF family has been identified as abnormal in preeclampsia (PE). Hypertensive disorders of pregnancy (HDP) are major contributors to maternal and neonatal morbidity and mortality worldwide; likewise, umbilical cord anatomical abnormalities (UCAA) are linked to poor neonatal outcomes. Based on the relationship described between PE and UCAA and the role of the VEGF family in PE, this study explored VEGF expression in placental and UC tissued from patients with PE and with UCAA. METHODS: We performed an observational, analytical study on placentas, comparing protein and mRNA e

Distinct Presentations of Hernia of Umbilical Cord.

Hernia of umbilical cord is a well-known entity which presents with herniation of small bowel into the proximal part of umbilical cord. It has very good prognosis after surgical repair. Occasionally, it can have distinct presentations and varied malformations at the umbilicus which have bearing on the course of treatment and final outcome. Herein, we describe various presentations and malformations associated with hernia of umbilical cord. Embryological extrapolation is attempted for the malformations at umbilicus.

The crying sign: the winking umbilical cord.

A preterm baby girl, born at 34 weeks gestation, with features of Beckwith-Wiedemann syndrome was noted to have a relatively large umbilical stump. No fetal abnormalities had been detected on anatomy scan at 28 weeks and only mild polyhydramnios and macrosomia were noted on a 32-week ultrasound scan. Although there was no obvious omphalocoele, clinical assessment of the umbilical cord revealed an abdominal wall defect through which bowel would protrude into the umbilicus when the infant was crying. In keeping with an abdominal wall defect α-fetoprotein was found to be elevated. Surgical consul

Inflammatory responses in the placenta and umbilical cord.

Histopathological examination of the placenta is the gold standard for evaluating antenatal inflammatory processes that might influence fetal development. Histological chorioamnionitis develops through a well-characterised stereotyped progression of maternal and fetal cellular stages that vary from patient to patient and are amenable to quantification. Increases in the intensity of these responses and their gradual transformation into a chronic phase are important variables that can adversely affect fetal physiology. Under recognised placental inflammatory lesions affecting the decidua, placen

Hemangioma of the umbilical cord: report of a case.

A 31-year-old woman with a large placental tumor underwent a caesarean section. After delivery, the lesion, detected by ultrasound examination, was found to originate from the umbilical cord. A live female infant with cyanosis, hypotonia and diffuse edema was delivered. A review of the literature, which revealed 31 cases of umbilical cord hemangioma, showed that this tumor has a polymorphous presentation. Some fetuses and infants died from various causes, indicating that a close follow-up is necessary in these pregnancies

Obstetrically important lesions of the umbilical cord.

Many pathologic features of the umbilical cord affect fetal well-being adversely. Excessively long or short umbilical cords may be the cause of hematomas and thrombosis of cord vessels and the placental surface, thus causing fetal death and/or thrombocytopenia. In other cases, fetal hypoxia and central nervous system damage are possible outcomes. Thrombosis is also frequently induced by velamentous insertion of the cord, as are hemorrhages when the membranous vessels rupture during parturition. Entangling and knotting of the cord, especially of excessively long cords, may lead to similar lesio

Antepartum diagnosis of noncoiled umbilical cords.

OBJECTIVE: The null hypothesis is that fetuses with noncoiled umbilical cords diagnosed in the antepartum period will have outcomes no different from those with normally coiled cords. STUDY DESIGN: We prospectively gathered data from Jan. 1 through May 18, 1992, from all fetuses undergoing routine ultrasonographic evaluation. The outcomes of fetuses noted to have noncoiled umbilical cords were compared with those of a control group of fetuses with normally coiled cords. The control group consisted of those subjects undergoing ultrasonography during the study period who were ultimately transfe

[Angioma of the umbilical cord. Apropos of a case].

We report a case of umbilical cord angioma in a female newborn. Ultrasonographic antenatal diagnosis had suspected either an omphalocele or a tumor of the umbilical cord. A caesarean had been decided at term. Surgery was necessary to ligate the umbilical vessels. The cord was 37 cm long and 7 cm large in several places. It weighed 495 g. The interest of this case resides in its rarity. First description of this kind of tumor was made by Gerdes in 1864 and since, only 20 cases have been reported. Antenatal diagnosis difficulties in umbilical cord tumors are reviewed. Vitelin cyst, angioma, and

The umbilical cord: care, anomalies, and diseases.

The umbilical cord is the lifeline of the fetus and of the neonate in the first few minutes after birth. Care of the cord and stump in the immediate neonatal period varies according to social, cultural, economic, and geographic factors. Measures taken to insure sterility in cutting, tying, and painting the umbilical cord may prevent serious diseases such as tetanus neonatorum. Care of the umbilical cord may be less than optimal in babies born at home in unsupervised "home delivery." Minor congenital anomalies of the umbilical cord, such as umbilical hernia, and major anomalies, such as gastros

The water content of the human umbilical cord.

105 umbilical cords from 53 term and 52 preterm newborn infants were freeze-dried after removal of the blood vessels, to determine the water content. The mean umbilical cord water content (i.e. mean of water content of fetal and placental ends of the cord), was 88.9% (SD 2.73) for term cords and 91.9% (SD 1.99) for preterm cords. The mean water content fell with increasing gestation. The fetal end of the cord had a significantly higher water content than the placental end. Similarly, the volume of a 4-cm length segment of cord was significantly greater at the fetal than placental end. There wa
