National surgical, obstetric, anaesthesia and nursing plan, Nigeria.

Recent evidence suggests that strengthening surgical care within existing health systems will strengthen the overall health-care system. However, Nigeria's national strategic health development plan 2018-2022 placed little emphasis on surgical care. To address the gap, we worked with professional societies and other partners to develop the national surgical, obstetric, anaesthesia and nursing plan 2019-2023. The aim was to foster actions to prioritize surgical care for the achievement of universal health coverage. In addition to creating a costed strategy to strengthen surgical care, the plan

Nature Medicine Commission on dialysis policy in low- and middle-income countries.

The global demand for kidney replacement therapy (KRT) continues to increase, yet access remains limited in many low- and middle-income countries. Thailand has been recognized for integrating a sustainable KRT delivery model into its Universal Health Coverage scheme through a peritoneal dialysis-first ('PD-First') policy adopted in 2008. In 2022, the policy was revised to allow individuals to choose between hemodialysis or peritoneal dialysis as their first-line treatment. The intention was to improve patient choice and avoid high out-of-pocket costs, but the policy produced unintended consequ

Assessing the integration of social protection into infectious disease policies: a multi-country analysis of TB, HIV, and malaria strategies in Sub-Saharan Africa.

BACKGROUND: Tuberculosis (TB), HIV, and malaria continue to impose major economic hardship on households in Sub-Saharan Africa. While global frameworks such as the WHO End TB Strategy and Universal Health Coverage (UHC) emphasize social protection, evidence remains limited on how national policies translate these commitments into practice. OBJECTIVES: This study assessed how national TB, HIV, and malaria policies and strategic plans in Kenya, Malawi, Mozambique, Nigeria, and Zambia define and operationalize social-protection mechanisms to reduce household economic burden. It also examined ali

Procedural fairness in benefit package design: inclusion of pre-exposure prophylaxis of HIV in Universal Coverage Scheme in Thailand.

Since 2002, Thailand's Universal Coverage Scheme (UCS) has adopted a comprehensive benefits package with few exclusions. A positive-list approach has gradually been applied, with pre-exposure prophylaxis (PrEP) of HIV recently being included. Disagreements resulting from competing values and diverging interests necessitate an emphasis on procedural fairness when making any decisions. This qualitative study analyses agenda setting, policy formulation and early implementation of PrEP from a procedural fairness lens. Literature reviews and in-depth interviews with 13 key stakeholders involved in

Measuring the availability of human resources for health and its relationship to universal health coverage for 204 countries and territories from 1990 to 2019: a systematic analysis for the Global Burden of Disease Study 2019.

BACKGROUND: Human resources for health (HRH) include a range of occupations that aim to promote or improve human health. The UN Sustainable Development Goals (SDGs) and the WHO Health Workforce 2030 strategy have drawn attention to the importance of HRH for achieving policy priorities such as universal health coverage (UHC). Although previous research has found substantial global disparities in HRH, the absence of comparable cross-national estimates of existing workforces has hindered efforts to quantify workforce requirements to meet health system goals. We aimed to use comparable and standar

Voluntary health insurance in Nigeria: Effects on takers and non-takers.

Interventions aiming to simultaneously improve financial protection and quality of care may provide an important avenue towards universal health coverage. We estimate the effects of the introduction of the Kwara State Health Insurance program in Nigeria on not only the insured but also the uninsured. A subsidized voluntary low cost health insurance was offered by a private insurer as well as a quality upgrade in selected health care facilities. Using propensity score matching and panel data collected in 2009 and 2011 (n = 3509), we find that, for the insured, the program increased health care

The association of health insurance with use of prescription contraceptives.

CONTEXT: Given that substantial proportions of women of reproductive age lack health insurance coverage, it is important to assess whether lack of insurance is associated with the use of prescription contraceptives, which are the most expensive but also the most effective methods for preventing pregnancy. METHODS: Data from 26,674 females aged 18-44 who participated in the 2002 Behavioral Risk Factor Surveillance System survey (representing more than 25 million women in the U.S. population) were used to assess risk of unintended pregnancy, prescription contraceptive use and health insurance s

Measuring the composition and availability of human resources for health by sex in 204 countries and territories, 1990-2023, and workforce gaps for universal health coverage: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Understanding the size, gender composition, and cadre mix of the health workforce and how it has evolved across time and locations can inform policies for planning, recruitment, training, and retention of human resources for health (HRH), and improvement of access to the health-care workforce among populations. Using comparable and standardised data sources, we aim to describe the composition and density of health workers by sex among 20 cadres for 204 countries and territories over 1990-2023 and quantify workforce shortfalls relative to universal health coverage (UHC) attainment.

A scoping review of national policies for hierarchical medical system in China since the 2009 health reform.

BACKGROUND: Universal health coverage (UHC) is driving the global health agenda. In 2009, the Chinese government launched a new round of healthcare reform toward UHC, and made remarkable progress in UHC through the establishment of a hierarchical medical system (HMS). We aim to summarize and review the evolution of China's hierarchical medical system policy. METHODS: Eligible policies were identified between March 7, 2009 and December 31, 2022, by searching the official websites of the PKULAW Database, the Chinese State Council, and related ministries. Policy screening and data extraction wer

Disaggregating catastrophic health expenditure by disease area: cross-country estimates based on the World Health Surveys.

BACKGROUND: Financial risk protection (FRP) is a key objective of national health systems and a core pillar of universal health coverage (UHC). Yet, little is known about the disease-specific distribution of catastrophic health expenditure (CHE) at the national level. METHODS: Using the World Health Surveys (WHS) from 39 countries, we quantified CHE, or household health spending that surpasses 40% of capacity-to-pay by key disease areas. We restricted our analysis to households in which the respondent used health care in the last 30 days and categorized CHE into disease areas included as WHS

Swiss-CHAT: Citizens Discuss Priorities for Swiss Health Insurance Coverage.

BACKGROUND: As universal health coverage becomes the norm in many countries, it is important to determine public priorities regarding benefits to include in health insurance coverage. We report results of participation in a decision exercise among residents of Switzerland, a high-income country with a long history of universal health insurance and deliberative democracy. METHODS: We adapted the Choosing Healthplans All Together (CHAT) tool, an exercise developed to transform complex healthcare allocation decisions into easily understandable choices, for use in Switzerland. The 'Health Transformation Plan' in Iran: A policy to achieve universal health coverage in slums and informal settlement areas.

Universal health coverage (UHC) is one of the strategies that health decision- and policy-makers worldwide are implementing to guarantee a good health status to everyone. Living in slums is characterized by several issues, including homelessness and malnutrition, environmental challenges, lack of sanitation and access to safe, healthy drinking water, waste disposal problems, widespread social disruptions, job insecurity, feelings of dissatisfaction and inadequacy. In Iran, the 'Health Transformation Plan' (HTP), despite its weaknesses, has had good effects on the health level of people living

Insurance for autosomal dominant polycystic kidney disease patients prior to end-stage renal disease.

There recently has been substantial dialogue about access to health insurance for Americans. This discussion has highlighted the issues of pre-existing diseases and portability as barriers to adequate health insurance coverage. For these reasons we decided to investigate the issues relating to health insurance and life insurance coverage experienced by patients with autosomal dominant polycystic kidney disease (ADPKD). A questionnaire-based study was conducted. Two hundred thirty-eight of 354 subjects responded. There was no significant difference in gender, age, number of children, or level o

Financial protection failure among urban poor populations in Karachi, Pakistan: a cross-sectional household survey of catastrophic health expenditure prevalence, determinants and insurance effectiveness.

INTRODUCTION: Pakistan has one of South Asia's highest burdens of out-of-pocket health expenditure. The disaggregated evidence for urban poor populations in major cities remains limited and hidden under the national data. Among the big cities in Pakistan, Karachi's informal settlements concentrate the country's most financially vulnerable households where formal health insurance coverage is negligible. This study measured catastrophic health expenditure prevalence among urban poor households in Karachi, determined its socioeconomic distribution and evaluated the financial protective effect of

Universal Health Coverage in Pakistan: Challenges, Reforms, and the Road to 2030.

Background: Universal Health Coverage (UHC) aims to ensure that all individuals have access to essential health services without financial hardship. Pakistan, despite constitutional commitments and policy reforms, faces significant hurdles in progressing toward this global goal due to systemic, structural, and socioeconomic challenges. Methods: This narrative review synthesizes available literature, policy documents, and programmatic evidence to examine Pakistan's preparedness for UHC. The analysis is structured around four thematic levels: policy, institutional, population, and service delive

Impact of health intervention coverage on reducing maternal mortality in 126 low- and middle-income countries: a Lives Saved Tool modelling study.

BACKGROUND: There is a continued and urgent need to address the stagnation of the global maternal mortality ratio (MMR), especially for low- and middle-income countries (LMICs). We aimed to assess the impact of scaling up health intervention coverage on reducing MMR under four scenarios for 126 LMICs. METHODS: We conducted the modelling study to estimate MMR and additional maternal lives saved by intervention by 2030 for 126 LMICs using the Lives Saved Tool (LiST). We applied four scenarios to assess the impact of scaling up health intervention coverage with no scale-up (no change), a modest

Assessing the equity and coverage policy sensitivity of financial protection indicators in Europe.

Progress towards universal health coverage is monitored by the incidence of catastrophic spending. Two catastrophic spending indicators are commonly used in Europe: Sustainable Development Goal (SDG) indicator 3.8.2 and the WHO Regional Office for Europe (WHO/Europe) indicator. The use of different indicators can cause confusion, especially if they produce contradictory results and policy implications. We use harmonised household budget survey data from 27 European Union countries covering 505,217 households and estimate the risk of catastrophic spending, conditional on household characteristi
