Creutzfeldt-Jakob disease (CJD) is a transmissible subacute spongiform encephalopathy that invariably leads to death. The presumed causative agent, the prion protein, is highly resistant to inactivation and has a long incubation period. Physical contact with CJD patients (as in clinical care) entails no risk of transmission. During procedures such as lumbar puncture where contact with infected material is possible, precautions are necessary. Precautions are: the use of gloves, maximal protection of people who come in contact with contaminated tissue (e.g. pathologist and histological laborator

The number of new cases--or incidence--of tuberculosis is increasing in nearly every region of the world. A number of forces have resulted in the increased incidence of TB in developed countries, including the HIV epidemic, homelessness, and emigration from highly endemic regions. Although the number of new cases in Canada is relatively constant, the TB experience in the United States serves as a reminder that this situation could change rapidly. The appearance of multidrug-resistant tuberculosis has added to the urgency of situation. The basic methods of preventing TB transmission include pre

Exposure to ionizing radiation combined with multiple trauma is a very rare but severe event. There are some important basic principles for the early inpatient management. An externally exposed patient poses no risk to the treatment team. Injuries require treatment in order of priority as known for example by ATLS(®). Against external contamination, the treatment team is adequately protected by wearing protective clothing and gloves in conformity with universal medical precautions. Treatment of life threatening injuries takes priority over decontamination. Specialized treatment centres should

This investigation was carried out in order to: identify the occurrence of professional accidents with perforate cutting potentially contaminated material among nurses and nursing auxiliaries that work in a general teaching hospital in the interior of the state of São Paulo. The other objectives were: to associate the occurrence of accident with the professional category and period of work; to identify the type of perforate cutting material most involved in accidents, to identify the attitudes taken by the person accidented. The data were collected on individual interview based on a structured

A cross sectional study was conducted to determine occupational factors associated with the human immunodeficiency virus (HIV)) infection. The survey covered 30 health institutions throughout Zambia. A total of 370 consenting midwives took part in the study. The main outcome measures were percentage of midwives reporting adverse occupational exposures and HIV status. Out of 370 midwives, 146 (39.5%) were positive for HIV infection. Of the 370 respondents, 48 (13.0%) thought that they were HIV infected. None of these 48 midwives thought she became infected because of occupational exposure to HI

BACKGROUND: Several studies have found that compliance with universal precautions (UP) reduces the risk of exposure to diseases transmitted through blood and body fluids. Several efforts were made during the 2014 Ebola virus disease (EVD) outbreak in Nigeria to ensure a better behavioral change toward the practice of UP.
OBJECTIVES: This study assessed knowledge, attitude, and practice of the use of universal safety precautions among health care workers in a tertiary hospital in Osun State in southwestern Nigeria, 1 year after the containment of the EVD epidemic in Nigeria.
METHODS: Descript

The prevalence of multidrug-resistant organisms (MDROs) in intensive care units (ICUs) is increasing worldwide, with very large variations across countries, microorganisms, and settings. Emerging MDR gram-negative bacteria and fungi raise particular concerns that require improved prevention and control strategies. Vertical approaches are mainly based on screening and contact precautions and/or decolonization of MDRO carriers. On the other hand, horizontal strategies are not pathogen-specific and include standard precautions (i.e., hand hygiene), universal decolonization, antimicrobial stewards

The author reviews the worldwide references concerning the problem of HIV infection in health care workers (HCW) after occupational exposure in its different aspects: infection risk rate (0.36-0.4%); common types of exposure (percutaneous, contact, and mucomembranous); main risk factors related to the lesion, the patient (donor) and HCW (receptor); and preventive attitude to be adopted. In what concerns prevention, the author emphasises the universal precautions (nosocomial infection prevention) and drug prophylaxis with anti retroviral drugs-reasons for administration, inclusion and exclusion

Human immunodeficiency virus infection in women and infants is a significant and growing health care problem in the United States. Young adult black or Hispanic women are at particular risk, especially if they use intravenous drugs or are the sexual partner of someone who does. However, acquired immunodeficiency syndrome/HIV infection is a significant cause of mortality in women of reproductive age (i.e., 15 to 44 years). In addition, the incidence of AIDS in infants directly correlates to the incidence of AIDS in childbearing women. HIV-infected pregnant women need on-going comprehensive pren

Chronic Creutzfeldt-Jakob disease, presenting in later life, may be difficult to distinguish from other dementing illnesses. Patients with this disease may lack the characteristic myoclonus and electroencephalographic complexes. We report four patients with a slowly progressive dementia who were misdiagnosed during life and had a spongiform encephalopathy at autopsy. All four patients had early extrapyramidal rigidity, primitive reflexes, and other neurologic signs. The presence of these neurologic findings early in the course of a dementia suggests that clinicians should consider chronic Creu

PURPOSE OF REVIEW: Healthcare-associated infections (HAIs) remain a critical challenge in intensive care units (ICUs) due to the high prevalence of invasive procedures, vulnerable patient populations, and the increasing threat of antimicrobial-resistant organisms (MDROs). This review synthesizes current evidence on infection prevention and control (IPC) strategies in the ICU setting, highlighting recent findings and innovations in this evolving field, particularly in light of the impact of the COVID-19 pandemic.
RECENT FINDINGS: The review outlines ten key IPC strategies for ICUs, categorizin

Trauma-informed care continues to follow the 4R principles (realize the widespread nature of trauma, recognize what unresolved trauma looks like, respond by changing practices and protocols, and resist retraumatization) but has expanded to include 6 core guiding principles that include safety, trust, peer support, mutuality, empowerment, and the understanding of an individual's cultural context. Additionally, it is now recommended that trauma-informed practices be offered as universal precautions to all individuals, regardless of trauma history, focusing on trauma prevention. Obstetric anesthe

The epidemic of Coronavirus disease 2019 (COVID-19) in China caused by the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) has become a global concern and subsequently labeled a pandemic by the World Health Organization on March 11th. As the world mobilizes to contain the COVID-19, scientists and public health experts are increasingly alarmed about the potentially catastrophic effects of an outbreak in Africa. The establishment of Africa Centres for Disease Control and Prevention by the Africa Union in 2017 was an unprecedented move toward strengthening national responses, so far

This article discusses the most commonly recognized bloodborne infectious diseases with which the nurse anesthetist comes into contact during the provision of clinical care: hepatitis B virus, hepatitis C virus, and the human immunodeficiency virus, along with the potential percutaneous and mucocutaneous routes of exposure. The prevention of occupational exposure to these pathogens is discussed by applying standard or universal precaution measures to anesthesia clinical practice. Treatment measures for accidental exposure to bloodborne pathogens is also discussed.

The increase in HIV infection rates around the world has created concern among medical educators for the safety of students and residents taking overseas electives. In some areas, the HIV adult positivity rate is 15%-20% or higher. Medical schools and residency programs need to make policies to protect their trainees serving abroad. This paper provides background for such policies. A key element is communication among the student, his/her advisor, and the overseas faculty. The student needs a knowledge of universal precautions and will need to have protective equipment that is often not availa

BACKGROUND: Needlestick injuries (NSIs) are a major occupational hazard among healthcare workers and expose them to serious blood-borne infections such as hepatitis B virus, hepatitis C virus, and human immunodeficiency virus. Healthcare workers in teaching hospitals, particularly interns and residents, are at increased risk due to limited experience, high workload, and frequent involvement in invasive procedures. Despite the availability of universal precautions, NSIs continue to occur and are often underreported.
OBJECTIVES: The objectives of this study were to assess the knowledge, attitud

OBJECTIVES: This study aimed to explore barriers to accessing care, if any, among people living with HIV/AIDS (PLHA) in two districts of Bihar. We also aimed to assess attitudes towards PLHA among healthcare providers and community members.
DESIGN: This qualitative study used an exploratory study design through thematic analysis of semistructured, in-depth interviews.
SETTING: Two districts were purposively selected for the study, namely the capital Patna and a peripheral district located approximately 100 km from Patna, in order to glean insights from a diverse sample of respondents.
