Acute Ischemia of the Upper Extremity.

Ischemia of the upper extremity is uncommon but can be seen with open or closed trauma. Those dealing with traumatic injury of the upper extremity should be conversant with techniques of vascular surgery and microsurgery to address these injuries when they occur. Closed injury can occur as well, and at times these are best managed nonoperatively. This article discusses the management of both in the acute setting.

Desmoid tumors of the upper extremity.

PURPOSE: To determine the outcomes of surgical excision with or without adjuvant treatment in the management of desmoid tumors of the upper extremity. METHODS: We retrospectively reviewed 52 patients with a histologically confirmed desmoid tumor in the upper extremity that was managed surgically. All patients presented between 1970 and 2011 and had a minimum 2-year follow-up. RESULTS: There were 25 males and 27 females with an average age of 37 ± 17 years. The most common location was the shoulder (n = 27). The most common symptom was a painful mass (n = 30). Average tumor size was 189 ± 371

Upper extremity injuries in Homer's Iliad.

Homer's Iliad remains a fascinating source of medical history. This epic poem, compiled around 800 BCE, describes several weeks of the last year of the 10-year siege of Troy (Ilion) by the Achaeans. Homer composed the epic by combining and formalizing oral poems, legends, customs, and experiences that originated in the later Mycenaean age (1600-1100 bce). The story centers on the rage of the great warrior Achilles. The Iliad remains the oldest record of Greek medicine and a unique source of surgical history. This study examines the upper extremity injuries described in the Iliad and compares t

Upper extremity neuromuscular injuries in athletes.

Upper extremity muscle and nerve injuries in athletes are important causes of lost playing time and suboptimal performance. Although most muscle injuries are self-limited, imaging may be indicated in select situations for diagnostic and prognostic purposes, to investigate potential complications of injury, and for instituting prompt therapy to hasten recovery. MRI is particularly sensitive to soft tissue abnormalities seen in muscle injury, and it can reliably diagnose and stage direct injuries such as contusions and lacerations, and indirect injuries such as strains, delayed-onset muscle sore

Compartment syndrome of the upper extremity.

Compartment syndrome involves the sustained elevation of interstitial tissue pressures within an osteofascial envelope to nonphysiologic levels. Tissue injury involves a spectrum from reversible to irreversible damage and, therefore, early recognition and treatment is critical for optimal outcomes. This article reviews the nature of upper extremity compartment syndrome; considers the general classification scheme and potential causes; and discusses the pertinent anatomy, pathophysiology, treatment recommendations, and outcomes for this challenging condition.

Vascular anomalies of the upper extremity.

The topic of vascular anomalies is uncommon in the hand surgery literature, but hand surgeons do diagnose and treat patients with hemangiomas and vascular malformations. These are separate entities and require different treatment strategies. Proper diagnosis will lead to timely and appropriate treatment.

Microsurgical complications in the upper extremity.

Although many advances have been made in microsurgery, it is not without complications. As microsurgeons continue to make advances in technology, technique, and applications that expand the utility of this field to more and more patients, they must be prepared to deal with the complications related to donor and recipient sites and the medical comorbidity that accompanies these large endeavors in the pre-, post-, and intraoperative periods.

Imaging pediatric sports injuries: upper extremity.

With increasing youth participation in organized sports, more injuries in this age group are being treated by primary care and sports medicine physicians. Overuse injuries are much more common now than in past decades, with Little League shoulder, Little League elbow, and gymnast wrist being particular concerns. Rotator cuff tears and glenoid labral injuries, once thought to be rare in this age group, are also more common now. Osteochondritis dissecans of the elbow is relatively common and typically fares poorly without surgery. Wrist abnormalities that occur nowadays include triangular fibroc

Intraoperative distraction in the upper extremity.

Intraoperative distraction aids in the restoration of length, facilitates reduction, assists in maintaining rotational alignment, and provides a temporary stable platform for definitive fixation of acute fractures, malunions, and nonunions. This technique has been described at length in the lower extremity; however, there is a paucity of literature regarding its use in the upper extremity. Distraction is the application of tension across a fracture site. Proximal and distal fixation may be achieved in several ways, with common instrumentation including the use of an external fixator set. Intra

Work-related upper extremity musculoskeletal disorders.

Upper extremity musculoskeletal disorders such as DeQuervain's tendonitis, carpal tunnel syndrome, and rotator cuff tendonitis have become increasingly common among working people in the United States. Extensive epidemiological investigation indicates that the adverse ergonomic exposures of force, repetition, vibration and certain postures are risk factors for development of many of these disorders. Assessment of patients with possible work-related upper limb disorders requires eliciting information about the illness, performing an examination about the illness, and obtaining information about

Upper extremity replantation: three-year experience.

Microsurgery in Mexico was initiated in 1967, when the first report of the subspecialty was published. At our hospital, we have had a well-organized microsurgery department since 1995. This has improved our management of patients with amputations of the upper extremity. This article presents our experience with upper extremity replantation, including hand and fingers. During the first 3 years, we managed 55 patients, 42 male and 13 female, aged 2-52 years, who had suffered amputations of some part of their upper extremity or even of the complete limb. These patients underwent surgical explorat

Focal upper extremity neuropathy in neonates.

Focal upper extremity neuropathies are common in neonates. The brachial plexus is the most common site involved. Brachial plexus injuries may involve different structures, thus producing different clinical presentations: complete brachial plexus palsy, Duchenne-Erb palsy, upper-middle trunk brachial plexus palsy, Klumpke palsy, fascicular brachial plexus palsy, and bilateral brachial plexus palsy. The causes of brachial plexus palsy are obstetric injury, intrauterine compression, humeral osteomyelitis, hemangioma, exostosis of the first rib, neck compression, and neoplasm. The differential dia

Upper extremity function after wrist arthrodesis.

Several studies have examined the normal range of wrist motion used to accomplish activities of daily living. Little information is present, however, on what functional limitations might be experienced by patients actually undergoing formal wrist arthrodesis. This study undertook comprehensive functional evaluation of 23 patients who underwent wrist arthrodesis for post-traumatic conditions. Follow-up evaluation averaged 54 months and consisted of a clinical questionnaire, the Jebsen Hand Function Test, and a functional rating devised by Buck-Gramcko/Lohmann. Fifteen of the 23 patients returne
