Urinary Biomarkers and Response to Treatment in Overactive Bladder (OAB): A Systematic Review and Meta-Analysis.

INTRODUCTION: Overactive bladder (OAB) is a prevalent symptom-based syndrome lacking objective diagnostic tests. Urinary biomarkers, particularly nerve growth factor (NGF), have emerged as potential non-invasive tool for diagnosis and treatment monitoring. OBJECTIVES: To systematically review and meta-analyze the association between urinary biomarker with treatment response and symptom score changes in adults with OAB. METHODS: Prospective clinical studies relating urinary biomarkers to established OAB treatments were identified in eight databases up to May 2024, selected with predefined cri

Daily symptom associations for urinary urgency and anxiety, depression and stress in women with overactive bladder.

INTRODUCTION AND HYPOTHESIS: Women with overactive bladder (OAB) report psychological distress, anxiety and depression, but short-term associations between these symptoms are poorly studied. Our objectives were to study daily associations between OAB symptoms and psychological symptoms and test whether these associations were stable when reassessed after 3 months. We hypothesized that OAB symptoms are positively associated with anxiety and depression symptoms over a short-term (daily) basis. METHODS: Female patients with OAB [bothersome urgency and/or urgency urinary incontinence (UUI)] asses

[Cellular regulation of the lower urinary tract as a cause of the bladder overactivity and reduced efficiency of pharmacotherapy].

Overactive bladder (OAB) is a polyetiological nosology. Its symptoms are often characterized not only with detrusor hyperactivity, but also with the increased sensitivity of afferent fibers, which is clinically manifested as urgency. In addition, the disorders at the level of receptors expression and the synthesis of mediators lead to the development of bladder pain syndrome (BPS), which also significantly reduces the quality of life of patients. In recent years, the experimental animal studies achieved significant progress in understanding of the pathogenesis of lower urinary tract dysfunctio

Emerging Urinary Biomarkers for Diagnosis, Phenotyping, and Treatment Monitoring in Female Overactive Bladder: A Systematic Review.

INTRODUCTION AND HYPOTHESIS: Overactive bladder (OAB) syndrome, characterised by urinary urgency, frequency, and nocturia can significantly impacts patients' quality of life. Current diagnosis is clinical, but complex cases often require invasive urodynamic studies (UDS), which are costly, subjective, and carry risks like discomfort and infection. Therefore, we hypothesise that urinary biomarkers could serve as noninvasive diagnostic tools for OAB. Establishing a reliable biomarker profile could ultimately lessen the reliance on invasive US, provided clinical validity is confirmed. METHODS: F

Urinary nerve growth factor levels could be a biomarker for overactive bladder symptom: a meta-analysis.

To examine whether urinary tract nerve growth factor (uNGF) could be a biomarker for overactive bladder (OAB) symptom, we conducted a comprehensive meta-analysis of 8 case-control studies. In all the studies considered, patients with OAB symptom had a higher uNGF level compared to healthy people. In addition, patients had a significantly lower uNGF level after successful treatment. In the subgroup analysis, we found that patients with OAB-wet symptom had a higher uNGF level than patients with OAB-dry symptom. However, no significant difference was found between patients with OAB symptom and pa

Barriers to early and effective overactive bladder management in male patients with lower urinary tract symptoms.

Overactive bladder is underdiagnosed and undertreated in men as symptoms overlap with benign prostatic obstruction. This study identified barriers urologists encountered in effectively managing overactive bladder in men with lower urinary tract symptoms. 60 urologists recruited across Australia, Brazil, Colombia, Russia, South Korea and Thailand via convenience sampling participated in in-depth interviews. They were asked about their understanding and management of overactive bladder in men with lower urinary tract symptoms. Thematic analysis identified six themes: (1) overactive bladder diagn

Induction of Bladder Overactivity by Nerve Growth Factor in Testes in Rats: Possible Neural Crosstalk Between the Testes and Urinary Bladder.

OBJECTIVES: To clarify the pathophysiological factor underlying neural crosstalk among pelvic organs, we investigated the possible role of nerve growth factor (NGF) in the neural crosstalk between the testes and urinary bladder. METHODS: Nerve growth factor (10, 30, and 100 µg/mL) or saline was injected into the testes of male Wistar rats. The change in bladder capacity via cystometry and duration of spontaneous scratching behavior induced by NGF in conscious rats was measured. The effects of pretreatment with capsaicin on NGF-induced changes in bladder capacity and behavior were examined. Fu

Effect of detrusor overactivity on the expression of aquaporins and nitric oxide synthase in rat urinary bladder following bladder outlet obstruction.

BACKGROUND: Aquaporins (AQPs) have recently been reported to be expressed in rat and human urothelium. Nitric oxide (NO) is thought to play a role in the bladder overactivity related to bladder outlet obstruction (BOO). The purpose of this study is to investigate the effect of BOO on the expression of AQP2-3 and nitric oxide synthase (NOS) isoforms in rat urothelium. METHODS: Female Sprague-Dawley rats (230-240 g, n = 60) were divided into 2 groups. The control group (n = 30) and the partial bladder outlet obstruction (BOO) group (n = 30). After 4 weeks, we performed a urodynamic study to mea

Lactobacillus-Polydopamine System for Targeted Drug Delivery in Overactive Bladder: Evidence from Bladder Cell Spheroids, Rat Models, and Urinary Microbiome Profiling.

INTRODUCTION: Overactive bladder (OAB) is a highly prevalent condition with limited treatment options due to poor efficacy, side effects, and patient compliance. Novel drug delivery systems that can target the bladder wall may improve OAB therapy. METHODS: We explored a polydopamine (PDA)-coated lactobacillus platform as a potential carrier for localized OAB treatment. Urinary microbiome profiling was performed to identify the presence of lactobacillus in healthy and OAB groups. Lactobacillus-PDA nanoparticles were synthesized and characterized by electron microscopy and spectrophotometry. A

New participant stratification and combination of urinary biomarkers and confounders could improve diagnostic accuracy for overactive bladder.

Overactive bladder (OAB) is a highly prevalent symptom complex characterised by symptoms of urinary urgency, increased frequency, nocturia, with or without urge incontinence; in the absence of proven infection or other obvious pathology. The underlying pathophysiology of idiopathic OAB is not clearly known and the existence of several phenotypes has been proposed. Current diagnostic approaches are based on discordant measures, suffer from subjectivity and are incapable of detecting the proposed OAB phenotypes. In this study, cluster analysis was used as an objective approach for phenotyping pa
