Typhoid conjugate vaccines: is a single dose enough for durable protection?

INTRODUCTION: Typhoid fever is widespread in developing countries. Most typhoid vaccines have gone into some disrepute for their substantial side effects and low efficacy. The latest typhoid vaccines use Salmonella's Vi-capsular polysaccharide (Vi-CPS) conjugated to a protein carrier. The WHO recommends a single typhoid conjugate vaccine (TCV) dose at six months in endemic countries. However, this schedule is contested. AREAS COVERED: The molecular structure of Vi-CPS, emerging Vi capsule variants, the impact of de-O acetylation on vaccine immunogenicity, the key features of an effective Vi-P

Polysaccharide and conjugate vaccines to Streptococcus pneumoniae generate distinct humoral responses.

Streptococcus pneumoniae is a major cause of community-acquired pneumonia, bacteremia, and meningitis in older adults worldwide. Two pneumococcal vaccines containing S. pneumoniae capsular polysaccharides are in current use: the polysaccharide vaccine PPSV23 and the glycoconjugate vaccine PCV13. In clinical trials, both vaccines elicit similar opsonophagocytic killing activity. In contrast to polysaccharide vaccines, conjugate vaccines have shown consistent efficacy against nasopharyngeal carriage and noninvasive pneumonia overall and for some prevalent individual serotypes. Given these differ

[Biosynthesis of polysaccharide conjugate vaccines against Klebsiella pneumoniae serotype O2 strains].

The main purpose of this research is to synthesize and evaluate a new glycoconjugate vaccine against Klebsiella pneumonia (Kp). First, the gene (waaL) responsible for the expression of O antigen ligase was deleted to block the synthesis of bacterial LPS. Then the vector that encodes a glycosyltransferase (PglL) was transferred into the mutant. The enzyme PglL could catalyze the transfer of OPS units to recombinant cholera toxin B subunit (rCTB) to form glycoproteins in vivo. The protective effects of the glycoproteins were studied by the mice models with acute bacteremia that were induced by i

Long-term effect of pneumococcal conjugate vaccines on tympanostomy tube placements.

BACKGROUND: We have previously shown that the 7-valent pneumococcal conjugate vaccine (PncCRM) given in infancy is effective in reducing tympanostomy tube placements up to 4 to 5 years of age. This study aimed to assess the effectiveness of pneumococcal conjugate vaccines PncCRM and PncOMPC from 2 up to 13 years of age. METHODS: Altogether 2497 children participated in the Finnish Otitis Media Vaccine trial conducted in 1995 to 1999 and were vaccinated at 2, 4, 6 and 12 months of age with PncCRM or PncOMPC or hepatitis B vaccine as controls. The data for tympanostomy tube placements were coll

The impact of protein-conjugate polysaccharide vaccines: an endgame for meningitis?

The development and implementation of conjugate polysaccharide vaccines against invasive bacterial diseases, specifically those caused by the encapsulated bacteria Neisseria meningitidis, Haemophilus influenzae and Streptococcus pneumoniae, has been one of the most effective public health innovations of the last 25 years. These vaccines have resulted in significant reductions in childhood morbidity and mortality worldwide, with their effectiveness due in large part to their ability to induce long-lasting immunity in a range of age groups. At the population level this immunity reduces carriage

Prevention of meningococcal disease: current use of polysaccharide and conjugate vaccines.

Invasive meningococcal disease (IMD), although uncommon, is difficult to diagnose and can be rapidly fatal, even in healthy young persons. IMD is cyclic, and serogroups responsible for disease vary by age group, although the prevalence of the serogroups changes over time and by geographical location. Two quadrivalent vaccines are licensed in the United States to prevent IMD caused by serogroups A, C, Y, and W-135, and the US Centers for Disease Control and Prevention recommends routine vaccination with quadrivalent meningococcal conjugate vaccine of adolescents 11-18 years of age and vaccinati

Neonatal and infantile immune responses to encapsulated bacteria and conjugate vaccines.

Encapsulated bacteria are responsible for the majority of mortality among neonates and infants. The major components on the surface of these bacteria are polysaccharides which are important virulence factors. Immunity against these components protects against disease. However, most of the polysaccharides are thymus-independent (TI)-2 antigens which induce an inadequate immune response in neonates and infants. The mechanisms that are thought to play a role in the unresponsiveness of this age group to TI-2 stimuli will be discussed. The lack of immune response may be overcome by conjugating the

Meningococcal A, C, Y and W-135 polysaccharide-protein conjugate vaccines.

Serogroup C meningococcal conjugate vaccines, first launched in the UK in 1999, have been used successfully in Australia, Canada and several other European countries. Combination conjugate vaccines, containing more than one meningococcal polysaccharide, have been developed to broaden protection against the disease. A tetravalent meningococcal A, C, Y and W-135 conjugate vaccine was licensed for use in 11-55 year old adolescents and adults in the US in January 2005, and subsequently also in 2-11 year old children in Canada in May 2006. This article discusses the different glycoconjugate meningo

Staphylococcus aureus types 5 and 8 capsular polysaccharide-protein conjugate vaccines.

BACKGROUND: Staphylococcus aureus, the first or second most common pathogen isolated from patients, is capsulated; there are at least 12 capsular types, and types 5 and 8 comprise approximately 85% of blood. Types 5 and 8, composed of a trisaccharide repeat unit including a mannose uronic acid and 2 fucoses, are non-immunogenic. As protein conjugates, they induce opsonophagocytic antibodies that confer type-specific active and passive protection in mice. METHODS: A phase II study of patients with end-stage renal disease showed that these conjugates induced approximately one third of the immun

Response and persistence of antibodies to PRP-T and DTwP vaccines with concomitant administration of conjugate vaccines.

We first studied the immunogenicity of PRP-T and DTwP vaccines in Filipino infants given at 6, 10 and 14 weeks concomitantly with either an aluminum adjuvanted eleven-valent pneumococcal conjugate vaccine (11PncTD) or a meningococcal diphtheria-conjugated vaccine as compared to a control group that received only DTwP/PRP-T. The GMCs and proportions of infants achieving protective antibody concentrations to DTwP and PRP-T vaccine antigens were similar among the groups. In the second phase, the control group received 11PncTD at 18 weeks and the antibody concentrations were measured at 9 months i

Multidimensional challenges in Brazil's decision-making process of vaccines adoption: The case of childhood pneumococcal conjugate vaccines.

UNLABELLED: The objective of this paper was to examine the process of adopting new vaccines in a middle-income country. METHODS: We used an intrinsic explanatory case study to analyze the recent decision-making process of switching 10-valent Pneumococcal Conjugate Vaccine (PCV10) to PCV13 in the Brazilian childhood immunization schedule. We analyzed Conitec (Comissão Nacional de Incorporação de Tecnologias no Sistema Único de Saúde) official documents. RESULTS: In November 2022, the Conitec plenary recommended, at first, switching from PCV10 to PCV13, considering PCV13 non-inferiority and si

Development of invasive non-typhoidal Salmonella conjugate vaccines and their evaluation in a trivalent formulation with typhoid conjugate vaccine.

Invasive nontyphoidal Salmonella (iNTS) infections, primarily caused by Salmonella enterica serovars Typhimurium (S. Typhimurium) and Enteritidis (S. Enteritidis), represent a significant public health concern, particularly in sub-Saharan Africa, where multidrug-resistant (MDR) strains are increasingly prevalent. Despite the substantial disease burden, no vaccines are currently licensed for iNTS. This study aimed to develop an iNTS conjugate vaccine by conjugating O-specific polysaccharide (OSP) antigens to carrier proteins using chemical conjugation, a proven method known for its efficiency a

Post-13-Valent Pneumococcal Conjugate Vaccine Dynamics in Young Children of Serotypes Included in Candidate Extended-Spectrum Conjugate Vaccines.

After worldwide implementation of 10-valent and 13-valent pneumococcal conjugate vaccines (PCV10/PCV13), a 20-valent PCV (PCV20) was developed. We assessed dynamics of non-PCV13 additional PCV20 serotypes (VT20-13), compared with all other non-VT20 serotypes, in children <2 years of age in late PCV13 (2015-2017) and early PCV (2009-2011) periods. Our prospective population-based multifaceted surveillance included isolates from carriage in healthy children, children requiring chest radiography for lower respiratory tract infections (LRTIs), and children with non-LRTI illness, as well as isolate

Co-administration of vaccines: a focus on tetravalent Measles-Mumps-Rubella-Varicella (MMRV) and meningococcal C conjugate vaccines.

Simultaneous administration of different vaccines is a strategy to increase the possibility to receive vaccines at appropriate age, safely and effectively, reducing the number of sessions and allowing a more acceptable integration of new vaccines into National Immunization Programs (NIPs). Co-administration can be performed when there are specific indications in the Summary of Product Characteristics (SmPC) of the vaccines; but, in absence of these indications, the practice is possible if there are no specific contraindications nor scientific evidence to discourage simultaneous administration.

[Quadrivalent meningococcal conjugate vaccine].

Meningoccocal meningitis is still a severe disease. In Switzerland the predominant serotypes are serotype B and C, but this could change as other groups predominate elsewhere. The indications for immunizing against meningococcal infections as described in the Swiss recommendations will be reviewed. New quadrivalent vaccines conjugated with a protein and efficient against serogroup ACWY will soon be available in our country. They show a number of advantages in term of type of protection and should simplify the current vaccination schedule while giving a better coverage against various serogroup

Menactra: a meningococcal conjugate vaccine.

Menactra is the first conjugated meningococcal vaccine approved in the US. It protects against meningococcal serogroups A, C, Y and W-135. The CDC's Advisory Committee on Immunization Practices (ACIP) recommends administration of the new vaccine to pre-adolescents aged 11 or 12, teens entering high school and college freshmen living in dormitories. These broad recommendations have led to a shortage of the new vaccine. Menomune, the old unconjugated quadrivalent meningococcal vaccine, is also effective in these age groups and could still be used.
