Diagnostic assessment of traumatic brain injury by vacuum extraction in newborns: overview on forensic perspectives and proposal of operating procedures.

BACKGROUND: Traumatic brain injury (TBI) during birth constitutes one of the most relevant causes of mortality and morbidity in newborns worldwide. Although improvements in obstetrical management and better indications for caesarean section have led to a consistent decrease in the incidence of perinatal mechanical injury, vacuum extraction is still associated with a high complications rate leading to several forensic issues in the evaluation of healthcare professional management. METHODS: Vacuum-associated lesions may be topographically distinguished as extracranial or intracranial injuries.

Nonsurgical removal of a rectal foreign body using a vacuum extractor. Report of a case.

PURPOSE: Rectal foreign bodies can be extracted by nonsurgical methods. However, glass objects require technical considerations to minimize morbidity and may necessitate surgical extraction. We describe a technique that allowed safe transanal extraction of a glass foreign body and avoided laparotomy. METHODS: A patient with a history of a previous rectal foreign body that required laparotomy presented with another incarcerated rectal foreign body. After attempts at manual extraction failed, spinal anesthesia was induced, and an obstetric vacuum extractor was used to transanally withdraw the g

Impact of parity and intrauterine fetal condition during vacuum extraction.

PURPOSE: The main goal of this study was to investigate the impact of parity and intrauterine fetal condition on vacuum extraction. MATERIAL AND METHODS: This study included 10742 newborns who were delivered during 2008 in obstetrics/gynecology clinics in Kosovo, Yugoslavia. One hundred fifty-eight of these were vacuum deliveries. Data were collected for indicators such as parity, maternal age, gestation duration, newborn vitality, and birth weight. RESULTS: Vacuum extraction was used in 158 cases (1.47 %), of whom 116 (73%) were nulliparous and 42 (27%) were multiparous. In 121 cases (76.5%

[Complications of vacuum extractor deliveries].

OBJECTIVES: To describe maternal and neonatal complications following deliveries assisted by vacuum extraction and to compare outcomes with those obtained after spontaneous vaginal delivery. We wanted to know if vacuum extractor was a risk factor by itself. MATERIALS AND METHODS: We conducted a retrospective study of two years activity involving 4524 deliveries of which 845 (18.7%) were vacuum extractor assisted. We precisely defined maternal and neonatal complications to compare their rates in spontaneous vaginal delivery and vacuum extractor groups. RESULTS: There were 1333 maternal compli

[A Case Report of a Pregnant Woman who Had Urethral Avulsion during Vacuum Extraction Delivery and Ipsilateral Kidney].

The incidence of lower urinary tract injuries associated with vaginal delivery is 0.03-0.05%, and reports of urethral injuries related to vacuum delivery are also extremely uncommon. We report a case of urethral injury in a 29-year-old woman during her first pregnancy and delivery due to vacuum extraction. The urethral injury occurred in continuity with a vaginal laceration on the opposite side of the midline episiotomy. A cystoscopy performed after urethral repair did not reveal the left ureteral orifice. Further examination revealed that the left kidney was not identifiable on ultrasound, an

[Vacuum extraction].

OBJECTIVE: To summarize available data concerning vacuumextraction and it's use in the Czech Republic. DESIGN: Review article. METHODS: Compilation of published data from scientific literature. CONCLUSION: The rate of vacuumextraction use in the Czech Republic is one of the lowest in the world. Forceps is usually the preferred technique. The use of vacuumextraction is on the rise, the limiting factor is the fact that in Czech Republic it is not recommended in cases with presumed fetal distress. According to world literature, the preferred technique in cases where operative vaginal delivery

[Peridural anesthesia and method of delivery. Peridural anesthesia performed by the obstetrician].

In deliveries in epidural analgesia (PDA) there are more obstetrical operations than in deliveries without PDA. We have investigated the delivery mode in relation to the indication of epidural analgesia. If the PDA is done because of prolonged course of labour or because of suspicious fetal heart rate, about 40% of these women deliver spontaneously, in about 35% a cesarean is done, and in about 25% a forceps- or vacuum-extraction is done. This high rate of operative interventions is not unexpected due to the indication that led to epidural analgesia. If the PDA is done because of painful labou

Soft cup vacuum extraction: a comparison of outlet delivery.

One hundred eighteen nulliparous patients delivered under conduction anesthesia who met the standard criteria for low forceps delivery were randomly assigned to one of three groups to be delivered by low Tucker-McLane forceps, Silastic vacuum extraction system, or Mityvac vacuum extractor. Significant maternal soft tissue trauma was identified in 48.9% of the forceps group, 36.1% of the Silastic group, and 21.6% of the Mityvac group. Superficial fetal scalp changes were found in 71% of the forceps group, 44% of the Silastic group, and 46% of the Mityvac group. Of these, cephalhematomatas were

The Association between the Number of Vacuum Pop-Offs and Adverse Neonatal Outcomes.

OBJECTIVE: Obstetrical vacuum manufacturers have long recommended a maximum of two to three pop-offs be allowed before abandoning the procedure. However, there is a paucity of evidence on the safety of vacuum-assisted vaginal delivery in relation to the number of pop-offs to support this recommendation. Our objective was to examine whether the number of pop-offs in a vacuum-assisted vaginal delivery was associated with adverse neonatal outcomes. STUDY DESIGN: This is a retrospective cohort study of women who underwent a trial of a vacuum-assisted vaginal delivery at a single tertiary care ins

Anaesthetic management of labour in two patients with Klippel-Feil syndrome.

Two patients with Type I Klippel-Feil syndrome presented at the antenatal clinic. The first patient, who suffered from sleep apnoea, was delivered of a healthy infant by vacuum extraction. The second, who was profoundly deaf and had marked kyphoscoliosis, developed pregnancy-induced hypertension and urinary tract infection and was delivered at 38 weeks by vacuum extraction. In both cases epidural analgesia was employed to allow pain relief during labour. Anaesthetic management of Klippel-Feil syndrome is discussed and the benefits of early anaesthetic assessment and continued involvement of se

Patient perception of involvement in medical care during labor and delivery.

BACKGROUND: Patients who feel involved in their treatment have better outcomes than those who do not. OBJECTIVE: To identify determinants of perceived patient involvement in obstetric care. METHODS: A retrospective study was undertaken in 1,452 (83%) of 1,750 women sampled in November 1995 from maternity wards of 14 general hospitals in Israel. A postal and telephone survey using a self-administered questionnaire included the following variables: hospital (identity number), patients' age, self-reported complications, previous deliveries, education, ethnicity, and number of obstetric interven

Skull subcutaneous emphysema--a rare complication of vacuum extraction and scalp electrode.

A full-term newborn infant with severe subcutaneous emphysema of the skull as a complication of vacuum extraction and scalp electrode wound is reported. We hypothesize that detachment of subcutaneous tissue and transmission of the negative pressure associated with the vacuum extraction resulted in entry of air through the scalp electrode wound.

Forceps, vacuum, and breech extraction for difficult delivery of an unengaged fetal head during cesarean birth: a retrospective cohort study.

OBJECTIVES: To compare perinatal outcomes, primarily neonatal transcutaneous bilirubin levels and the need for phototherapy, associated with three fetal extraction techniques used during cesarean birth after difficult or failed manual delivery of an unengaged fetal head: short forceps, vacuum, and breech extraction. METHODS: A retrospective, single-center analytical study was conducted including all cesarean births performed between July 2022 and July 2025 in which fetal extraction was considered difficult and required the use of short forceps, vacuum, or breech extraction. Influence of difficulty of instrumentation with vacuum on the rate of levator ani muscle avulsion identified by 3-4 d transperineal ultrasound.

OBJECTIVES: Evaluation of the influence of difficulty of instrumentation with vacuum on the rate of levator ani muscle (LAM) avulsions. MATERIALS AND METHODS: Prospective observational study with 86 nulliparous women with at term gestation who required instrumentation with vacuum to complete fetal extraction. After every delivery, each explorer reported the number of vacuum tractions needed to complete fetal extraction, as well as the subjective complexity of the instrumentation. LAM avulsion rate was assessed by 3D-4D transperineal ultrasound evaluation 6 months after delivery. RESULTS: Sev

Prospective assessment of vacuum deliveries from midpelvic station in a tertiary care university hospital: Frequency, failure rates, labor characteristics and maternal and neonatal complications.

BACKGROUND: Midpelvic vacuum extractions are controversial due to reports of increased risk of maternal and perinatal morbidity and high failure rates. Prospective studies of attempted midpelvic vacuum outcomes are scarce. Our main aims were to assess frequency, failure rates, labor characteristics, maternal and neonatal complications of attempted midpelvic vacuum deliveries, and to compare labor characteristics and complications between successful and failed midpelvic vacuum deliveries. STUDY DESIGN: Clinical data were obtained prospectively from all attempted vacuum deliveries (n = 891) ove
