The urinary catecholamine metabolites, homovanillic acid (HVA) and vanillylmandelic acid (VMA), are used for the adjunctive diagnosis of neuroblastomas. Headache and hypertension were the main clinical symptoms in this slim 77-year-old lady. Because the hypertension presented as paroxysmal and was combined with palpitations, dizziness, and sweating the suspicion of a phaeochromocytoma arouse. The vanillyl mandelic acid in the urine was elevated. As further investigation revealed that this elevation was due to medication with L-dopa because of Parkinson-syndrome. A proper history of the patients' medication or alternative laboratory tests prevent false conclusions.

BACKGROUND: Phaeochromocytomas are rare tumors of chromaffin cells of neural crest that classically present with symptoms of catecholamine excess such as palpitations, headache and sweating. They are diagnosed by measuring plasma or urinary levels of catecholamines or their metabolites. Anatomic localization is done by computed tomographic scan or magnetic resonance imaging, or meta-iodobenzyl guanidine scan in certain cases. OBJECTIVES: To study the early clinical efficacy of combined therapy of stage 4 neuroblastoma.
METHODS: A retrospective analysis was performed on the medical data and follow-up data of 14 children with stage 4 neuroblastoma who were diagnosed in Hong Kong University-Shenzhen Hospital from January 2016 to June 2021.

Three patients with rapidly progressive, disseminated malignant pheochromocytoma were treated with a combination chemotherapeutic regimen consisting of cyclophosphamide, vincristine, and dacarbazine in repeated 21- to 28-day cycles. All three patients had a marked decrease in blood pressure and an improvement in performance status within the first few cycles of treatment.
