Elimination of neoangiogenesis for plaque stabilization: is there a role for local drug therapy?

Emerging data suggest that intraplaque hemorrhage is critical in promoting atherosclerotic lesion instability. Because red blood cell membranes are a rich source of free cholesterol and accumulated red blood cells within plaques promote inflammation, intraplaque hemorrhage is associated with expansion of the necrotic core. Plaque hemorrhage results from the development of immature neointimal vasa vasorum. Therefore, it is proposed that molecular therapies designed to eliminate pathologic neovascularization within developing lesions will interrupt the process of hemorrhage and decrease the rate

Medial Neovascularization: A Novel Mechanism of Aortic Dissection in a Hypertensive Population?

BACKGROUND: This study aimed to elucidate the impact of vasa vasorum on the natural progression of aortic dissection in patients with hypertension. METHODS: Full-thickness ascending aortic walls from 43 participants (median age, 53 years; 32.6% women) were analyzed using pathological observation and shear stress measurement. Comparisons among hypertensive dissection (AD, n=23), hypertensive control (n=12), and nonhypertensive dissection (non-HT-AD, n=8) groups were performed using Kruskal-Wallis and repeated-measures ANOVA tests. RESULTS: Compared with the control group, a higher density of

Internal carotid artery dissection in a patient with Behçet's syndrome.

The authors report a case of internal carotid artery dissection in a young woman with Behçet's syndrome. The authors postulate that a vasculitis of the vasa vasorum already suspected as the basis of aneurysm formation in course of Behçet's syndrome can account for occurrence of arterial dissection in this inflammatory condition.

In utero indomethacin alters O2 delivery to the fetal ductus arteriosus: implications for postnatal patency.

Indomethacin produces constriction and hypoxia of the fetal ductus arteriosus. This is associated with death of smooth muscle cells in the ductus wall and an increased incidence of patent ductus arteriosus in the newborn period. We used fetal sheep to determine which factors are responsible for indomethacin-induced hypoxic cell death. Cell death in the ductus wall is directly related to the degree of indomethacin-induced ductus constriction and is present at both moderate and marked degrees of constriction. Both moderate and marked degrees of ductus constriction reduce vasa vasorum flow to the

Dilation-Responsive Microshape Programing Prevents Vascular Graft Stenosis.

Shape memory materials have been successfully applied to minimally invasive implantation of medical devices. However, organ-movement-specific shape programing at a microscale level has never been demonstrated despite significant unmet needs. As vein-to-artery grafting induces vein dilation and stenosis, a polymeric self-enclosable external support (SES) is designed to wrap the vascular out-wall. Its micropores are programmed to increase sizes and interconnections upon dilation. Vessel dilation promotes venous maturation, but overdilation induces stenosis by disturbed blood flow. Therefore, the

The guardian of intracranial vessels: Why the pericyte?

Intracranial atherosclerotic stenosis (ICAS) is a pathological condition characterized by progressive narrowing or complete blockage of intracranial blood vessels caused by plaque formation. This condition leads to reduced blood flow to the brain, resulting in cerebral ischemia and hypoxia. Ischemic stroke (IS) resulting from ICAS poses a significant global public health challenge, especially among East Asian populations. However, the underlying causes of the notable variations in prevalence among diverse populations, as well as the most effective strategies for preventing and treating the rup

Expression of vascular adhesion molecules in saphenous vein coronary bypass grafts.

BACKGROUND: Adhesion of blood elements to the endothelium is an important step in the development of vein graft disease. This study examines the expression of vascular adhesion molecules on explanted saphenous vein bypass grafts. METHODS: Immunocytochemical staining was performed using explanted saphenous vein grafts from 28 patients. Antibodies against the endothelial markers CD31, von Willebrand factor, intercellular adhesion molecule-1, vascular adhesion molecule-1, and E-selectin were used. RESULTS: Staining for CD31 and von Willebrand factor demonstrated the presence of endothelial cell

Vasculitis unlimited in rheumatoid arthritis an illustrated review.

Rheumatoid arthritis has a multitude of extra-articular manifestations, of which systemic vasculitis is a clinically significant co-morbidity and co-mortality determinant in the prognosis of the disease. Rheumatoid vasculitis may occur in the early stage of the disease but, more commonly, in patients who have had seropositive rheumatoid arthritis for 10 years or longer. Rheumatoid vasculitis has a wide variety of histopathologic expressions and it may affect blood vessels of all sizes (from vasa nervorum or vasa vasorum to the aorta; and occasionally veins and venules). The diagnosis ideally r

Niacin in the prevention of atherosclerosis: Significance of vasodilatation.

There is a rising interest towards the old drug, nicotinic acid (niacin, vitamin B(3)), because at pharmacological concentrations it has a beneficial effect on HDL cholesterol. Its use, however, was limited due to its adverse effect, flushing. When the mechanism of flushing was solved, a combination of niacin and DP1 receptor antagonist or prostaglandin inhibitor is used, there has been a comeback of niacin with extensive clinical trials. This paper argues that the new strategy with niacin for the prevention of atherosclerosis should be re-evaluated, because vasodilatation of the peripheral ve

Syphilitic aortitis. A case report.

A case of syphilitic aortitis with total occlusion of the infrarenal aorta without aneurysmal dilatation is presented. Incapacitating claudication of both legs together with pain at rest necessitated an aortobifemoral bypass operation, which resulted in complete relief of symptoms. Histological examination of the aorta showed atherosclerosis together with characteristic perivascular lymphocytic infiltration of the aortic vasa vasorum.

Hypoxic microenvironment as a crucial factor triggering events leading to rupture of intracranial aneurysm.

Subarachnoid hemorrhage being the rupture of intracranial aneurysm (IA) as a major cause has quite poor prognosis, despite the modern technical advances. Thereby, the mechanisms underlying the rupture of lesions should be clarified. Recently, we and others have clarified the formation of vasa vasorum in IA lesions presumably for inflammatory cells to infiltrate in lesions as the potential histopathological alternation leading to rupture. In the present study, we clarified the origin of vasa vasorum as arteries located at the brain surface using 3D-immunohistochemistry with tissue transparency.

Takayasu's truncoarteritis. Pulseless disease or aortitis syndrome.

The etiology of Takayasu's truncoarteritis is still unknown, although recent studies have suggested the participation of immunogenetic factors in the pathogenesis of this disease. Based on the summary of autopsy findings and taking clinical features into consideration, the current criteria for the pathological diagnosis of this condition might be summarized as follows. 1) Non-syphilitic truncoarteritis frequently occurring in young females. 2) Frequent manifestation of pulseless syndrome, and occasional presence of specific eye-ground findings or hypertension. 3) Common involvement of cranial

The pathology of Kawasaki disease aortitis: a study of 37 cases.

BACKGROUND: Kawasaki disease (KD) is a systemic vasculitis syndrome that occurs most frequently in children. Most clinical and pathological studies have focused on its coronary artery lesions. To date, no detailed studies of the aorta have been conducted. We studied KD autopsy cases with the aims of clarifying the time-course of changes in aortic lesions, the differences in the inflammatory cells and degree of inflammation at various aortic sites, and the progression of the inflammation. MATERIALS AND METHODS: The study materials were aortic specimens taken from 37 KD autopsy cases (acute pha

Fungal aneurysms of intracranial vessels.

Intracranial fungal aneurysms arise from major cerebral arteries. Fungi directly invade vessel walls from the luminal surface (fungal emboli) or from the adventitia (fungal meningitis). The vasa vasorum are free of fungi. Aneurysmal rupture is common with extensive hemorrhagic necrosis of the surrounding brain. Aspergillus is the usual causative agent; its sources are nasal sinusitis or endocarditis.

Thoracic aortic aneurysm associated with aortitis--case reports and histological review.

Ten cases of post-inflammatory aneurysm of the thoracic aorta were studied morphologically using aortic wall and aortic valve tissue resected during the surgical repair of the lesions. Four cases of aortic aneurysm in Behcet's disease showed massive perivascular infiltration of the lymphocytes and plasma cells in adventitia and vasa vasorum. Fibrous obliteration of the vasa vasorum was seen, and neutrophilic infiltration was also associated in two cases with recurrent prosthetic valve failure. Three cases of aortic aneurysm associated with Takayasu's arteritis showed similar histologic feature

Type A intramural hematoma in the setting of acute type B aortic dissection.

Type A intramural hematoma (IMH) is an uncommon entity, the pathophysiology of which is thought to be related to a contained hemorrhage within the medial layer of the aorta as a result of either rupture of the vasa vasorum or an atherosclerotic plaque. We present a case of type A IMH in the setting of acute type B aortic dissection with suspicion for malperfusion syndrome and discuss the treatment algorithm of this uncommon entity.
