Feasibility, safety, and efficacy of a novel external compression vascular closure device: The LockeT® study.

INTRODUCTION: Hemostasis following large-bore femoral vein access remains a challenge. Manual compression has been the standard of care but requires bedside staff, prolonged bed rest, and longer length of stay. The LockeT is an external compression device that attempts to address these issues while achieving venous hemostasis. OBJECTIVES: We evaluate postprocedural hemostasis and vascular closure outcomes after using LockeT following cardiac electrophysiologic procedures. METHODS: We performed a single-center, observational study of patients who underwent vascular closure for electrophysiolo

Late onset iatrogenic limb ischaemia after deployment of an Angio-Seal vascular closure device.

It is common practice to deploy a vascular closure device for access site closure after percutaneous angiography or cardiovascular interventions for immediate haemostasis and to facilitate early discharge. We encountered two octogenarian women who underwent and had subsequent vascular access site closure with Angio-Seal (St Jude) and who later presented with limb ischaemia needing surgical revascularisation. Our patients had undergone uneventful deployment of the Angio-Seal vascular closure device (VCD) at the right common femoral artery (CFA) access site with successful haemostasis. About 3 w

ESR Essentials: arterial vascular access and closure devices-practice recommendations by the Cardiovascular and Interventional Radiological Society of Europe.

Vascular access is the initial, very important, step of endovascular procedures. Various access sites include the common femoral artery, brachial artery, radial artery, popliteal artery, and distal tibial vessels (pedal arteries). Successful arterial access requires advanced knowledge of anatomy, as well as proper training and experience. Today, vascular access should be obtained using real-time, ultrasound guidance to reduce access time, patient discomfort, and puncture-related complications including dissection, arteriovenous communication, and bleeding. Nevertheless, high-level evidence to

Clinical outcomes of MANTA vs suture-based vascular closure devices after transcatheter aortic valve replacement: An updated meta-analysis.

OBJECTIVE: A recently published randomized control trial showed different results with suture-based vascular closure device (VCD) than plug-based VCD in patients undergoing transfemoral transcatheter aortic valve replacement (TAVR). The learning curve for MANTA device is steep, while the learning curve for suture based VCD is shallow as the devices are quite different. In this meta-analysis, we have compared suture-based (ProGlide and Prostar XL) vs plug-based VCDs (MANTA). METHODS: We performed a meta-analysis of all published studies (using PubMed/Medline and Cochrane databases) reporting t

Efficacy and Safety of ProGlide Versus Prostar XL Vascular Closure Devices in Transcatheter Aortic Valve Replacement: The RISPEVA Registry.

Background Transcatheter aortic valve replacement (TAVR) requires large-bore access, which is associated with bleeding and vascular complications. ProGlide and Prostar XL are vascular closure devices widely used in clinical practice, but their comparative efficacy and safety in TAVR is a subject of debate, owing to conflicting results among published studies. We aimed to compare outcomes with Proglide versus Prostar XL vascular closure devices after TAVR. Methods and Results This large-scale analysis was conducted using RISPEVA, a multicenter national prospective database of patients undergoin

Optimising the atrial fibrillation ablation pathway using vascular closure devices: effects on length of stay, procedure time and outcomes.

BACKGROUND: The rising prevalence of atrial fibrillation has led to increasing the numbers of pulmonary vein isolations. Optimising care pathway efficiency is essential for sustainable healthcare delivery. This study aimed to evaluate the impact of closure devices (CD) within the care pathway on hospital stay, patient satisfaction, staff workload, and costs, following catheter ablation for atrial fibrillation. METHODS: This study compared the standard care pathway following catheter ablation using manual compression (MC, December 2023-February 2024) to a modified pathway incorporating suture-

Vascular Closure Devices Versus Standard Care in Patients Who Underwent Atrial Fibrillation Ablation: A Systematic Review and Meta-Analysis.

INTRODUCTION: The adoption of vascular closure devices (VCDs) has gained prominence as an alternative to standard care (SC) for patients undergoing atrial fibrillation (AF) ablation. Although VCDs present potential benefits, there are ongoing concerns related to the duration required for hemostasis, the time until patient discharge, and the incidence of vascular complications. Therefore, we conducted a systematic review and meta-analysis to assess the outcomes associated with VCDs compared to SC. METHODS: We systematically searched the PubMed, Embase, and Cochrane databases for randomized con

Preliminary experience with transcatheter closure of wind-sock-like perimembranous ventricular septal defects using Amplatzer Vascular Plug IV devices.

Amplatzer Vascular Plug IV (Abbott, USA) is usually used for the occlusion of abnormal tortuous vessels and has not been tried for the transcatheter closure of perimembranous ventricular septal defects with wind-sock morphology. Here, we report on three successful cases of perimembranous ventricular septal defect transcatheter closure using Amplatzer Vascular Plug IV. We did not observe residual shunting or new onset of complications during follow up. These preliminary positive results advocate the application and suitability of Amplatzer Vascular Plug IV for closing wind-sock-like perimembran

Network Meta-analysis of Randomized Trials on the Safety of Vascular Closure Devices for Femoral Arterial Puncture Site Haemostasis.

The safety of vascular closure devices (VCDs) is still debated. The emergence of more related randomized controlled trials (RCTs) and newer VCDs makes it necessary to further evaluate the safety of VCDs. Relevant RCTs were identified by searching PubMed, EMBASE, Google Scholar and the Cochrane Central Register of Controlled Trials electronic databases updated in December 2014. Traditional and network meta-analyses were conducted to evaluate the rate of combined adverse vascular events (CAVEs) and haematomas by calculating the risk ratios and 95% confidence intervals. Forty RCTs including 16868

Vascular closure with a suture retention device versus suture-mediated closure system after large bore venous access for electrophysiologic procedures.

BACKGROUND: The LockeT is a suture retention device that may be a postoperative closure option for electrophysiologic procedures requiring large-bore vascular access (LBVA) but warrants further evaluation. OBJECTIVES: We compare the use of LockeT to a suture-mediated vascular closure device for LBVA after left atrial appendage occlusion (LAAO). METHODS: We performed a retrospective study of LBVA after LAAO from March 2023 to August 2024 using LockeT or Perclose ProGlide. Endpoints included access-site-related complications, time to hemostasis (TTH), time to ambulation (TTA), and same-day dis
