Establishment of an animal model of vascular restenosis with bilateral carotid artery grafting.

BACKGROUND: Vascular restenosis occurring after CABG is a major clinical problem that needs to be addressed. Vein grafts are associated with a higher degree of stenosis than artery grafts. However, the mechanism responsible for this effect has not been elucidated. We aimed to establish a rabbit model of vascular restenosis after bilateral carotid artery grafting, and to investigate the associated spatiotemporal changes of intimal hyperplasia in carotid artery and jugular vein grafts after surgery. MATERIAL AND METHODS: Twenty adult New Zealand white rabbits (10 males; 10 females), weighing 2.

Intraoperative imaging techniques in infrainguinal arterial bypass grafting: completion angiography versus vascular endoscopy.

This prospective study was designed to establish whether vascular endoscopy would provide more information on the graft lumen than standard completion angiography during infrainguinal bypass surgery. Ninety-nine patients with 102 infragenicular bypass grafts who underwent both angiography and angioscopy intraoperatively were evaluated. In 99 of the 102 patients the indication was critical limb ischaemia. Of the 102 bypass grafts, 81 were autogenous vein. Distally, 24 grafts were anastomosed to the below-knee popliteal segment, 64 extended to the crural and 14 to the pedal arteries. On completi

Use of Omniflow® II Biosynthetic Graft for the Treatment of Vascular Graft and Endograft Infections.

BACKGROUND: Vascular graft/endograft infection is a rare but life-threatening complication of cardiovascular surgery and remains a surgical challenge. Several different graft materials are available for the treatment of vascular graft/endograft infection, each having its own advantages and disadvantages. Biosynthetic vascular grafts have shown low reinfection rates and could be a potential second best after autologous veins in the treatment of vascular graft/endograft infection. Therefore, the aim of our study was to evaluate the efficacy and morbidity of Omniflow® II for the treatment of vasc

Bone Grafting for Scaphoid Nonunions: Is Free Vascularized Bone Grafting Superior for Scaphoid Nonunion?

BACKGROUND: Nonvascularized bone grafting (NVBG) and vascularized bone grafts (VBGs) have been used to treat scaphoid nonunions. A comparison of a single institution' s experience using structural iliac crest bone graft (ICBG), 1,2-intercompartmental supraretinacular artery (1,2-ICSRA), and medial femoral condyle (MFC) grafts to treat scaphoid nonunions was undertaken. METHODS: Thirty-one patients underwent ICBG, 33 patients underwent 1,2-ICSRA, and 45 patients underwent MFC grafting. Mean time to follow-up was 16 months (range, 2-153). Twenty-three percent used tobacco products. Mean age was

Comparisons between prosthetic vascular graft and saphenous vein graft in femoro-popliteal bypass.

PURPOSE: Infrainguinalfemoropopliteal bypass (IFPB) is recommended to peripheral arterial disease (PAD) with a long occlusion of the superficial femoral artery (SFA). The aims of our study were to determine the patency of graft materials, and identify the risk factors of graft failure. METHODS: From January 1995 to April 2011, we had performed 380 IFPBs in 351 patients, including 302 femoro-above the knee (AK) bypasses and 78 femoro-below the knee (BK) bypasses. We compare age, sex, severity of ischemia between polytetra-uoroethylene (PTFE) graft and saphenous vein (SV) graft, and evaluate pa

Treatment of scaphoid nonunion: pedicled vascularized bone graft vs. traditional bone graft.

The clinical results of the application of pedicled vascularized bone graft (VBG) from Lister's tubercle vs. traditional bone graft (TBG) were evaluated and compared. Thirteen cases of symptomatic scaphoid nonunion were treated between January 2011 and December 2012, including 7 cases subject to VBG and the rest 6 cases to TBG, respectively. Outcomes were assessed by modified Mayo wrist score system. All cases were followed up for an average period of 3.5 months after operation. The results showed that total scores in VBG group were 86.4±9.4 after operation with excellent result in 4 cases, go

Novel Bioresorbable Vascular Graft With Sponge-Type Scaffold as a Small-Diameter Arterial Graft.

BACKGROUND: Current commercialized small-diameter arterial grafts have not shown clinical effectiveness due to their poor patency rates. The present study evaluated the feasibility of an arterial bioresorbable vascular graft, which has a porous sponge-type scaffold, as a small-diameter arterial conduit. METHODS: The grafts were constructed by a 50:50 poly (1-lactic-co-ε-caprolactone) copolymer (PLCL) scaffold reinforced by a poly (1-lactic acid) (PLA) nanofiber. The pore size of the PLCL scaffold was adjusted to a small size (12.8 ± 1.85 μm) or a large size (28.5 ± 5.25 μm). We compared the d

Vascularized Versus Non-vascularized Bone Grafting for Scaphoid Non-union: A Meta-Analysis.

Non-union after scaphoid fracture is a common complication. Controversy exists regarding the efficacy of vascularized bone graft (VBG) versus non-vascularized bone graft (NVBG) for scaphoid non-union. In the current meta-analysis, a comprehensive search of PubMed and the Cochrane Library database was done from inception to June 2024. We included randomized controlled trials (RCTs) and prospective comparative studies that reported outcomes following scaphoid non-union comparing VBG versus NVBG. Two reviewers independently extracted data and assessed the risk of bias. Any discrepancy was resolve

Compliant bilayer vascular grafts with in situ heparin functionalization for small diameter vascular regeneration.

Given the scarcity of autologous graft donors, a substantial and pressing demand exists for small-diameter (<6 mm) vascular grafts (SDVGs) as viable alternatives in bypass and reconstructive surgeries for the treatment of cardiovascular diseases, including coronary artery disease and peripheral artery disease. However, the challenges of compliance mismatch, inadequate endothelialization, and insufficient anticoagulation of SDVGs generally lead to side effects like thrombosis, inflammation and intimal hyperplasia. To address these issues, this study proposes a biomimetic bilayer SDVG with hepar

In situ replacement of infected vascular prostheses with rifampin-soaked vascular grafts: early results.

Based on the excellent results of experimental studies with antibiotic-bonded vascular prostheses for prevention of graft infection, gelatin-sealed grafts soaked with rifampin were implanted in situ in five patients with vascular infection. All patients were at risk for limb loss or death and could not be treated by standard techniques such as graft excision and extra-anatomic bypass. In one patient an infected aortic stump aneurysm with involvement of both renal and visceral arteries was found. He was treated by implantation of a bifurcation rifampin-soaked graft between the subdiaphragmal ao

Development of silk vascular graft coated with silk sponge.

Purpose: Changes in diet and lifestyle have increased the number of patients with vascular diseases. Synthetic grafts yield unacceptable outcomes in very small vessels, such as coronary arteries, causing the exclusive use of native vessels despite their limited availability and sometimes suboptimal quality. Therefore, a small-diameter vascular graft that achieves outcomes comparable to native vessels is clinically needed. Methods: In this paper, porous silk fibroin vascular grafts (PSVG) were prepared using a double-raschel knitting machine to create vascular grafts from silk fibers, followed

Reinforced biotubes as readily available and regenerative vascular grafts.

The increasing prevalence of cardiovascular diseases and haemodialysis access drive high clinical demand for vascular grafts. Bioinert synthetic and decellularized or cryopreserved artery grafts exhibit poor regeneration and associated complications. Here, we present an approach that combines in vivo bioengineering and decellularization to prepare polycaprolactone skeleton-reinforced biotubes (dPB), which demonstrate appropriate mechanical strength, anti-kinking properties, resistance to repeat puncture, and are compatible with long-term storage at 4 °C. The loose and porous microstructure and

Free Vascularized Fibular Grafting for Osteonecrosis of the Femoral Head.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) remains a challenging condition in orthopaedics1. Free vascularized fibular grafting (FVFG) theoretically has the potential to restore blood supply to the femoral head and enhance mechanical stability within the femoral head. This method has shown promising clinical outcomes in hip preservation. Previous studies have utilized the anterolateral approach to the hip for this surgical procedure. Here, we report a minimally invasive technique utilizing the direct anterior approach for debridement and bone grafting, requiring a shorter fibular gra

Hemodynamics-driven magnetoelastic vascular grafts for stenosis diagnosis.

Conventional approaches for vascular graft stenosis diagnostics, including X-ray angiography, magnetic resonance imaging and Doppler ultrasound, although highly accurate, are cumbersome, used intermittently and often do not detect stenosis early enough, leading to diagnosis only after substantial narrowing. Here we report a magnetoelastic vascular graft (MVG) for post-implantation stenosis diagnosis that is hemodynamics-driven, biocompatible and waterproof. It enables wireless, real-time and continuous diagnosis of stenosis by converting arterial hemodynamics into high-fidelity electrical sign

Endovascular (stent-graft)-assisted open vascular anastomosis.

Endovascular and open surgical approaches are an integral part of treating patients with complex vascular disease and are often considered separately. In some situations, traditional open surgical techniques can be used to facilitate an endovascular approach, as example: iliac conduit use for EVAR/TEVAR, subclavian or axillary conduits for complex endovascular aortic repairs (chimney, B-FEVAR), and bypass to great vessels or visceral artery (celiac, superior mesenteric and renal arteries) debranching. As devices and techniques evolve, the open and endovascular approaches can be utilized in mor

Prosthetic vascular graft management in above-knee amputations.

OBJECTIVE: Critical limb ischaemia (CLI) is the most severe state of peripheral arterial disease and is one of the major causes of lower-limb amputations. One of the treatment choices is prosthetic vascular grafts. Despite treatment, CLI may lead to amputation owing to infection or progressive ischaemia. The aim of this study was to show that multidisciplinary planning and surgery for CLI patients with prosthetic grafts decreased the duration of hospital stay, costs, risk of infection and ascending conversion of the amputation level. METHODS: Forty-two above-knee amputation patients with graf

Regeneration of trachea graft with cartilage support, vascularization, and epithelization.

The repair and functional reconstruction of long-segment tracheal defects is always a great challenge in the clinic. Finding an ideal substitute for tracheal transplantation is the only way to solve this problem. The current study proposed a series of novel strategies for constructing a bionic living trachea substitute. For the issue of tubular cartilage support, cartilage sheet technique based on high-density culture of chondrocytes was adopted to avoid the inflammatory reaction triggered by the materials and thus formed mature cartilage-like tissue in autologous goat model. For the issue of
