Hydroxyurea (hydroxycarbamide) in haemoglobin SC disease: Moving from therapeutic hesitation to evidence-guided care.

Girard et al. report a retrospective, single-centre, real-world study evaluating hydroxyurea (hydroxycarbamide) use in adults with haemoglobin SC disease in Quebec. Hydroxyurea was associated with reduced vaso-occlusive crisis composite events, mostly mild adverse events and no observed hyperviscosity-related complications, while highlighting adherence and prescription-renewal barriers in routine care. Commentary on: Girard et al. Hydroxyurea (hydroxycarbamide) use in adults with haemoglobin SC disease: A real-world study in Quebec. Br J Haematol 2026; 209:1291-1300.

Mean Platelet Volume can Predict Cerebrovascular Events in Patients with Sickle Cell Anemia.

OBJECTIVE: The purpose of this study was to determine the impact of mean platelet volume (MPV) on the frequency and severity of vaso-occlusive and cerebrovascular events in patients with sickle cell anemia (SCA). METHODS: The 238 cases diagnosed with SCA were evaluated retrospectively with respect to the occurrence of painful crisis for the previous year. The incidence, severity and type of the vaso-occlusive crises of the patients with SCA between March 2010 and March 2011 were recorded. The last MPV values in patients who were free of erythrocyte transfusion for the last three months and wh…

Thrombocytosis and the generation of platelet-derived microparticles in the pathophysiology of sickle cell disease.

Sickle cell disease (SCD) is a complex thrombo-inflammatory disorder in which haemolysis, platelet activation, thrombocytosis, inflammation and endothelial activation operate as an intricate network creating a perpetual cycle of cellular injury and vascular dysfunction that drives vaso-occlusive crises, tissue ischaemia and progressive end-organ damage. Thrombocytosis is common in SCD, especially in response to acute chest syndrome, vaso-occlusive crises and chronic haemolysis. Increased platelet activity contributes to the proinflammatory and prothrombotic environment. Activated platelets rel…

BCL11A rs1427407 Genotypes in Sickle Cell Anemia Patients Undergo to Stroke Problems in Sudan.

BACKGROUND: Sickle cell disease is an autosomal recessive condition that results from the presence of a mutated form of hemoglobin. Some genetic variants of BCL11A are amenable to therapeutic manipulation. The present study investigated the relationship of a BCL11A variant (rs1427407) and its plasma levels with vaso-occlusive crises and stroke complications among patients in Sudan with sickle cell disease. METHODS: This cross-sectional study was performed between June 2014 and October 2016. The subjects included 166 patients who were diagnosed with sickle cell disease and 35 healthy control s…

Potential efficacy of crizanlizumab in treating priapism in sickle cell disease: A case report.

This report documents the treatment of a 41-year-old male with sickle cell disease (SCD) and repeated stuttering priapism using crizanlizumab, which alleviated the priapism but induced a significant vaso-occlusive crisis during the second infusion. Encouragingly, no subsequent vaso-occlusive crises occurred. However, the potential for infusion-related adverse events warrants close supervision. Further research is necessary to explore its full benefits on priapism management.

Micronutrients and sickle cell disease, effects on growth, infection and vaso-occlusive crisis: a systematic review.

Patients with Sickle cell disease (SCD) exhibit signs of poor growth, increased susceptibility to infection and recurrent episodes of painful vaso-occlusive crises. Micronutrient deficiencies may increase susceptibility to these outcomes. We conducted a systematic review to assess the strength of evidence for improved outcomes related to micronutrient interventions. Six randomized-controlled trials of moderate quality met the inclusion criteria. Zinc supplementation was associated with improved growth and decreased incidence of infection and is a promising intervention in the management of SCD…

Sickle cell vaso-occlusive pain crisis in adults: alternative strategies for management in the emergency department.

The gene for sickle cell disease is carried by 8% of the African-American population in the United States. The primary care physician is often called upon to recognize and treat one of the major sequelae of sickle cell disease--vaso-occlusive pain crisis. An injectable nonsteroidal anti-inflammatory drug has recently become available and may offer some improvement in outcome of vaso-occlusive pain crises. We present five case reports reviewing various current therapeutic options, including newer pharmacologic agents, and comment on alternatives to impatient management of pain crises. The use o

Ischaemia-induced muscle metabolic abnormalities are poorly alleviated by endurance training in a mouse model of sickle cell disease.

NEW FINDINGS: What is the central question of this study? The aim of this study was to evaluate the potential beneficial effects of endurance training during an ischaemia-reperfusion protocol in a mouse model of sickle cell disease (SCD). What is the main finding and its importance? Endurance training did not reverse the metabolic defects induced by a simulated vaso-occlusive crisis in SCD mice, with regard to intramuscular acidosis, mitochondrial dysfunction or anatomical properties. Our results suggest that endurance training would reduce the number of vaso-occlusive crises rather than the c…

Base Editing of HBG1 and HBG2 Promoters for Sickle Cell Disease.

BACKGROUND: Sickle cell disease is characterized by chronic hemolytic anemia and recurrent severe vaso-occlusive crises. Ristoglogene autogetemcel (risto-cel) includes autologous CD34+ hematopoietic stem and progenitor cells that have been base-edited to target the HBG1 and HBG2 promoters and inhibit BCL11A binding without altering BCL11A expression, yielding a switch in hemoglobin production from sickle hemoglobin (HbS) to antisickling fetal hemoglobin (HbF). METHODS: In this phase 1-2 study, we enrolled patients 12 to 35 years of age with sickle cell disease who had had at least four severe…

Monitoring the acute phase response to vaso-occlusive crisis in sickle cell disease.

AIMS: To identify suitable acute phase proteins as objective markers of tissue ischaemia during painful vaso-occlusive crises in sickle cell disease. METHODS: The prodromal and established phases of 14 vaso-occlusive crises were studied longitudinally in 10 patients with sickle cell anaemia. Automated solid phase enzyme immunoassays were used to measure the fast responding acute phase proteins C-reactive protein and serum amyloid A protein. Slower responding glycoproteins (fibrinogen, orosomucoid, sialic acid and concanavalin-A binding) were measured in parallel. RESULTS: C-reactive protein …

Enhancing the care of patients with sickle cell disease.

When typically round red blood cells intermittently elongate and become lodged in the body's microcirculation, this can result in painful vaso-occlusive crises, often referred to as 'sickle cell crises'. The sickling and unsickling process can result in acute pain, chronic anaemia, ischaemic injury and multiple organ damage. One of the main concerns raised by patients with sickle cell disease is the lack of knowledge and understanding of their condition among healthcare professionals in acute care settings. Therefore, this article aims to enhance nurses' understanding of sickle cell disease an…

[Severe sickle cell anemia revealed in an adult by medullary necrosis].

INTRODUCTION: Drepanocytosis, even severe, may only be discovered in adults further to severe complications. OBSERVATION: A 44 year-old woman was hospitalised for a severe vaso-occlusive crisis, revealing a drepanocyte syndrome with heterozygote S-beta zero thalassemia. Evolution was marked by severe anaemia, the aregenerative nature of which, uncommon during vaso-occlusive crises and the absence of Parvovirus B19 infection, led to the diagnosis of medullar necrosis. Evolution was rapidly improved after transfusion of erythrocyte concentrations and symptomatic treatment of the pain. COMMENTS…

An assessment of the effectiveness of hydroxyurea therapy in children and adolescents with sickle cell anaemia at Kenyatta National Hospital.

Sickle cell anaemia (SCA) is a major public health concern in Kenya, particularly affecting children and adolescents. Despite the inclusion of hydroxyurea in national treatment guidelines, its real-world use and effectiveness in Kenya remain under-documented, especially in tertiary care settings such as Kenyatta National Hospital (KNH). The drug is known to reduce vaso-occlusive crises (VOCs), transfusions, and hospitalizations, but its impact in resource-limited environments requires further investigation. Hence, this study aimed to assess the clinical and hematological effectiveness of hydro…

Use of gabapentin in the treatment of chronic pain in an adolescent with sickle cell disease.

Vaso-occlusive crises are the most common manifestation of sickle cell disease (SCD) and the main cause of hospital admission in these patients. There is emerging evidence that vaso-occlusive pain has both nociceptive and neuropathic components. However, the treatment of SCD-related pain with neuropathic drugs has not yet been systematically studied, particularly in children. We describe a 14-year-old girl with SCD and multiple hospital admissions for pain management for severe acute vaso-occlusive pain episodes. The patient was evaluated by a multidisciplinary team of specialists which consid…

Hydromorphone-induced choreiform dyskinesia in a patient with homozygous sickle cell disease: a reversible adverse effect.

BACKGROUND: Sickle Cell Disease (SCD) is a type of chronic hemoglobinopathy with vaso-occlusive pain crises that requires aggressive pain management with opioids.Hydromorphone is commonly used in the management of pain in patients with SCD, particularly when vaso-occlusive crises require prolonged opioid therapy. While opioid-induced neurotoxicity is a well-recognized complication, usually presenting as delirium, agitation, or myoclonus, choreiform dyskinesias are rare and only infrequently reported. New-onset involuntary movements in patients with SCD may be concerning for cerebrovascular or …
