Transient right ventricular dysfunction after pericardiectomy in patients with constrictive pericarditis.

Pericardiectomy is the standard treatment in patients with chronic constrictive pericarditis who have persistent symptoms. However, myocardial atrophy with prolonged pericardial constriction and abrupt increase in venous return can lead to heart failure with volume overload after pericardial decompression, especially in the right ventricle (RV). We experienced a 44 year old male patient who developed transient RV failure after pericardiectomy for constrictive pericarditis. Echocardiography revealed a markedly dilated RV with decreased peak systolic velocity of the tricuspid annulus, suggesting

Right ventricular dysfunction during intensive pharmacologic unloading persists after mechanical unloading.

BACKGROUND: Right ventricular (RV) dysfunction is associated with adverse outcomes in heart failure (HF). Mechanical unloading should be more effective than pharmacologic therapy to reduce RV afterload and improve RV function. We compared RV size and function after aggressive medical unloading therapy to that achieved in the same patients after 3 months of left ventricular assist device (LVAD) support. METHODS AND RESULTS: We studied 20 patients who underwent isolated LVAD placement (9 pulsatile and 11 axial flow). Echocardiograms were performed after inpatient optimization with diuretic and

Intracardiac shunt with hypoxemia caused by right ventricular dysfunction following pericardiocentesis.

Significant hypoxemia can result from right-to-left intracardiac shunting through a patent foramen ovale, an atrial septal defect or a ventricular septal defect. Pulmonary embolus, congenital heart disease and pericardial tamponade are well-recognized causes of right-to-left shunting. However, right-to-left shunting can also follow pericardiocentesis. A case of profound hypoxemia caused by right ventricular hypokinesis precipitated by pericardial tap is reported. This under-recognized entity can be responsible for significant morbidity in the critical care setting. The clinical presentation, n

Mitigating Post-operative Right Ventricular Dysfunction After Left Ventricular Assist Device: The RV Protection Study.

BACKGROUND: Despite improvements in hemocompatibility-related adverse events (HRAEs) with the HeartMate 3 left ventricular assist device (LVAD), hemodynamic-related events (HDREs), such as right ventricular failure (RVF) and aortic insufficiency, still result in considerable morbidity and mortality. We investigated a comprehensive, upfront RV protection strategy combining hemodynamic, ventilatory and pharmaceutical optimization to mitigate the risk of RVF. METHODS/RESULTS: Participants were prospectively randomized in a 1:1 fashion to either the RV-protection strategy or usual care for post-o

Withaferin A Attenuates Angiotensin II-Induced Right Ventricular Dysfunction and Fibrosis.

Our previous studies have shown that continuous infusion of angiotensin II (Ang II) in C57BL/6J mice causes dysfunction and a cachexia-like pathogenesis in both skeletal muscle and the left ventricle, which is significantly reduced by withaferin A (WFA), a steroidal lactone. However, it remains unknown whether WFA can reverse right ventricular (RV) dysfunction induced by Ang II. To determine the effects of WFA in attenuating Ang II-induced RV dysfunction, we employed a model in which continuous Ang II infusion via an osmotic pump in C57BL/6J mice induced cardiac remodeling. We then focused on

Right ventricular dysfunction in cardiogenic shock on temporary mechanical circulatory support.

AIMS: Right ventricular dysfunction (RVD) is common in cardiogenic shock (CS) due to left ventricular failure and is associated with increased mortality, but its impact in patients requiring temporary mechanical circulatory support (tMCS) remains unclear. We explored RVD and its association with clinical outcomes in CS patients under tMCS. METHODS AND RESULTS: Individuals with CS from the SWISS Circulatory Support Registry (NCT04117230) were included. RVD was defined by ≥1 of the following parameters: tricuspid annular plane systolic excursion (TAPSE) < 14 mm, TAPSE/systolic pulmonary artery

Challenges and Insights: Severe Acute Right Ventricular Dysfunction in Pulmonary Hypertension.

Pulmonary hypertension (PH) is rarely a cause of syncope. We highlight an unusual presentation of pulmonary hypertension where management was a veritable challenge. We present a case report of a 35-year-old female with a history of stage 2 hypertension, polycystic ovarian syndrome, and obesity who presented to the hospital with a six-month history of progressive shortness of breath, lower extremity swelling, and recurrent syncope. Further evaluation with transthoracic echocardiography showed features consistent with severe pulmonary hypertension. This untreated severe pulmonary hypertension cu

Prognostic Implications of Right Ventricular Dysfunction in Severe Degenerative Mitral Regurgitation.

BACKGROUND: The prevalence and impact of right ventricular dysfunction (RVD) in degenerative mitral regurgitation (DMR) is unknown. We aimed to determine whether RVD assessed by echocardiography in routine clinical practice is independently associated with mortality in patients with DMR. METHODS AND RESULTS: We used data from the MIDA-Q (Mitral Regurgitation International DAtabase-Quantitative) registry, which included patients with isolated DMR due to mitral valve prolapse from January 2003 to January 2020 from 5 tertiary centers across North America, Europe, and the Middle East. Right Ventricular Dysfunction in Patients Undergoing High-Risk PCI with Impella.

BACKGROUND: Right ventricular dysfunction (RVD) is an important prognostic factor in several cardiac conditions, including acute and chronic heart failure. The impact of baseline RVD on clinical outcomes of patients undergoing high-risk percutaneous coronary intervention (HRPCI) supported by Impella is unknown. METHODS: Patients from the single-arm, multicenter PROTECT III study of Impella-supported HRPCI were stratified based on the presence or absence of RVD. RVD was quantitatively assessed by an echocardiography core laboratory and was defined as fractional area change < 35%, tricuspid ann

Right ventricular dysfunction in rheumatic heart valve disease: A clinicopathological evaluation.

BACKGROUND: . Dysfunction of the right ventricle (RV) in rheumatic heart disease (RHD) is a poor prognostic factor. We planned to observe the clinicopathological changes in the RV of patients with RHD. METHODS: . We defined RV dysfunction by a myocardial performance index value of >0.4 on transthoracic echo-cardiography and included patients with isolated severe mitral stenosis in sinus rhythm with normal left ventricular (LV) function from April 2014 to April 2016. The patients were divided into two groups based on the absence (group I, n=21) and presence (group II, n=22) of RV dysfunction.

Right ventricular diastolic dysfunction and patent foramen ovale causing profound cyanosis.

A 73 year old woman presented with profound central cyanosis and a history of a minor stroke. She had normal heart morphology, normal pulmonary artery pressure, and a normal coronary angiography. A patent foramen ovale (PFO) with a massive right to left shunt was demonstrated at the atrial level, with normal pulmonary venous saturations and PO2 values. This rare, age related case of right ventricular diastolic dysfunction in a normotensive patient revealed a generous PFO allowing a pronounced right to left shunt.

Controlled Right Ventricular Pressure Overload Can Rescue Left Ventricular Dysfunction by Promoting Biventricular Adaptive Hypertrophy.

Pulmonary artery banding (PAB) has been investigated as a therapy for left ventricular (LV) dysfunction in pediatric dilated cardiomyopathy; however, the underlying mechanisms of action of PAB-induced LV rehabilitation remain unknown. This study aims to establish a small animal model of PAB-treated LV dysfunction to document the biventricular hemodynamic and tissue-level modifications promoted by PAB. Sprague-Dawley rats underwent left anterior descending (LAD) artery ligation (LV dysfunction model) followed by PAB 1-week post-injury (LAD + PAB, n = 13). Controls consisted of sham (n = 16), PA

Left ventricular hypertrophy, diastolic dysfunction and right ventricular load predict outcome in moderate aortic stenosis.

AIMS: Predictors of progression of moderate aortic valve stenosis (AS) are incompletely understood. The objective of this study was to evaluate the prognostic value of left ventricular hypertrophy (LVH), diastolic dysfunction, and right ventricular (RV) load in moderate AS. METHODS AND RESULTS: Moderate AS was defined by aortic valve area (AVA), peak transvalvular velocity (Vmax) or mean pressure gradient (PGmean). A total of 131 Patients were divided into two groups according to the number of pathophysiological changes (LVH, diastolic dysfunction with increased LV filling pressures and/or RV
