Vestibular neuronitis. Pathogenesis in the view of virological study of CSF.

Cerebrospinal fluid (CSF) samples from 15 patients with vestibular neuronitis were virologically examined and analyzed, and compared to those from 16 patients with Hunt's syndrome. The results were as follows: The CSF protein in vestibular neuronitis showed an increase, beginning about two weeks after the onset of vertigo, while the cell count remained normal. The CSF protein in Hunt's syndrome rose, but its progress was different from that in vestibular neuronitis. In both diseases, the increase in CSF protein seemed to be due to protein originating in the blood; the increase was probably cau

[The diagnosis, treatment and aviation medicine identification of vestibular neuronitis].

estibular neuronitis is one of the leading causes of peripheral vertigo. Inflammation confined to the vestibular system, and the cochlear, central nervous system and hearing are normal. The disease may be associated with viral infections, but the virus serological positive rate is not high. Vestibular-evoked myogenic potential and head impulse test can be used for sub-type diagnosis, and improve the detection rate of inferior vestibular neuronitis.Corticosteroids therapy is still controversial, and had not been included in VN conventional treatment. Vestibular rehabilitation is helpful in buil

Late and sudden recovery of sudden deafness or vestibular neuronitis.

INTRODUCTION: Vestibular neuronitis (VN) and sudden deafness (SD) are well-defined entities. The abrupt appearance of symptoms in both disorders represents an important element for the diagnosis. In cases of VN, symptoms regress gradually over time as the peripheral function recovers, or the central nervous system compensates for the balance deficit. In cases of SD, the recovery occurs in about half the cases and is generally progressive. The chances to recover normal levels of hearing are better if the recovery occurs early after the onset of the deficit. The goal of the paper is to present n

[Vestibular neuronitis in a teenager with sickle cell disease. Treatment is urgent].

Vestibular syndrome is not frequently described in patients with sickle cell disease. We report the case of a teenager with sickle cell disease who had a vestibular syndrome with vertigo that successfully responded to exchange transfusion. We discuss guidelines and review the literature in view of this case report. Sensorineural disorders should be considered as stroke syndromes. They require urgent treatment consisting of exchange transfusion or maintaining optimal hydration associated with blood withdrawal. Treatment of vestibular syndrome in sickle cell disease is urgent.

[Diagnostic and therapeutic problems in vestibular neuronitis: clinical implications for sudden vertigo].

We defined sudden vertigo as a sudden, unilateral peripheral vestibular dysfunction. The criterion for its diagnosis is a single episode of vertigo without cochlear and central symptoms. Among 20 patients with sudden vertigo there was no difference in clinical aspects between those with CP (canal paresis) (CP% > or = 25%) and those without CP (CP% < 25%). This suggests that sudden vertigo with CP is due to sudden vestibular dysfunction with predominant involvement of the lateral semicircular canal. Basically, vestibular neuronitis is considered to be due to acute unilateral neuropathy of the v

Interpretation of the combined galvanic test. Findings from cases with vestibular neuronitis.

The combined galvanic test (CGT) was developed in our department for simultaneous recording of galvanic eye movement test (GEMT) and galvanic body sway test (GBST). Twelve cases of vestibular neuronitis were studied first with the caloric test, and then with the CGT. The findings of the caloric test corresponded to those of the GEMT, but did not always correspond to those of the GBST. Thus we suggest that the GBST reflects an abnormality in a system different from that reflected by the caloric test and GEMT. It is likely that the caloric response mainly originates from the semicircular canal s

Steroids for Acute Vestibular Neuronitis-the Earlier the Treatment, the Better the Outcome?

OBJECTIVE: To present findings that suggest steroid treatment within 24 hours of onset of vestibular neuronitis results in better restitution of vestibular function than treatment between 25 and 72 hours. PATIENTS: Thirty-three consecutive patients (17 men, 16 women, mean age 57 yr, range 17-85 yr) with acute vestibular neuronitis and treated with steroids within 72 hours after symptom onset. Patients were divided into two groups depending on if they were treated within the first 24 hours or not. INTERVENTIONS: Oral prednisolone 50 mg/d for 5 days with tapering of doses for the next 5 days,

[Lyme disease--a reason for sudden sensorineural hearing loss and vestibular neuronitis?].

BACKGROUND: Lyme disease has been described as one possible cause of sudden sensorineural hearing loss and vestibular neuronitis. The necessity of serological diagnosis and its therapeutic consequences have been discussed controversially. PATIENTS AND METHODS: 344 patients with acute sensorineural hearing loss and 66 patients with vestibular neuronitis were examined in retrospect. By means of ELISA (Enzygnost Borreliosis, Dade Behring Marburg) the specific prevalences of IgG- and IgM-antibodies against borrelia in serum were evaluated. The frequency of seroprevalences for both diseases were c

A case with symptoms of vestibular neuronitis caused by an intramedullary lesion.

We report on a patient with an intramedullary lesion who showed only symptoms of so-called vestibular neuronitis. From the neurological aspects and MRI findings, the lesion was thought to be localized to the vestibular nerve in the pons. Cerebrospinal fluid testing and MRI findings suggested a diagnosis of multiple sclerosis (MS), although the clinical features were not compatible with MS. This case may shed some light on the aetiology of vestibular neuronitis.

Neurotologic evidence of central and peripheral involvement in patients with vestibular neuronitis.

Quantitative vestibulo-oculomotor tests and auditory brain stem responses were studied in 22 patients with vestibular neuronitis in a search for evidence of CNS affection. Such evidence was found in 16 of the 22; only in the other six could the examination findings be regarded as the result of damage limited to vestibular neuroepithelium or nerve. It is concluded that vestibular neuronitis can cause CNS signs by itself, probably because of inflammation in the vicinity of the vestibular nerve.

[Dizziness and vertigo associated with vestibular neuronitis: the approaches to the diagnostics and treatment].

This article is focused on the peculiar features of diagnostics and treatment of dizziness and vertigo in the patients presenting with vestibular neuronitis. The authors present the detailed overview of various theories concerning etiology and pathogenesis of this condition, describe the methods for its clinical and instrumental diagnostics (including its differential diagnostics from other diseases associated with acute dizziness and vertigo). Special attention is given to the methods of pharmacological and physical rehabilitation of the patients suffering from vestibular neuronitis.

Compensatory process of vestibular neuronitis. From findings of combined galvanic test and caloric test.

The compensatory process of vestibular neuronitis in 7 patients was followed up and evaluated using the Combined Galvanic Test (CGT) and other neuro-otological data. CGT is the simultaneous recording of both galvanic eye movement (GEM) and galvanic body sway (GBS), and has proved to be reliable in the diagnosis of retrolabyrinthine disorders. The results were as follows: The subjective symptoms remained in 3 cases from 1 month to 1 year. The recovery of the GBS response was seen earlier than that of both GEM and caloric responses. The recovery of GEM and caloric responses was observed at almos

Comorbid intracranial vertebral artery asymmetry as a risk factor for severe vestibular neuronitis.

BACKGROUND: Our previous study had demonstrated that verterbral artery hypoplasia (VAH) contribute to ipsilateral vestibular dysfunction. The aim of this study was to test if VAH contributes to prognosis of vestibular neuronitis (VN) through presumed regional malperfusion. METHODS: We performed a prospective magnetic resonance angiographic registry in patients with acute vestibular neuritis in which were then assigned to VAH (n=29) and control group (n=40). Vestibular function was determined by caloric irrigation, with the use of the vestibular paresis formula (to measure the extent of unilat

A clinical observation of benign paroxysmal positional vertigo (BPPV) after vestibular neuronitis (VN).

We observed 9 cases of BPPV developed after vestibular neuronitis. The interval between the onset of BPPV and vestibular neuronitis ranged from 2 weeks to 20 years. All cases were examined for critical head position which provoked vertigo, non-gaze nystagmus, positional and positioning nystagmus and caloric nystagmus. No characteristic signs and symptoms could be observed. The function of the posterior canal is thought to be necessary to provoke positional vertigo. Thus in BPPV after vestibular neuronitis the function of the posterior canal would presumably have been preserved to some degree.

Vestibular neuronitis: a review of a common cause of vertigo in general practice.

Vestibular neuronitis is an interesting condition characterized by the acute onset of vertigo, nausea and vomiting, in the absence of hearing loss or tinnitus. There is often evidence of a recent or concurrent upper respiratory tract infection. The disease follows a benign course of between two days and six weeks. It often occurs in epidemics. Following the acute attack, mild transitory episodes of dizziness may recur over a period of 12 to 18 months. Clinical and histopathological evidence suggests that it is caused by an isolated lesion of the vestibular nerve, although the exact aetiology r
