Active viscosupplements for osteoarthritis treatment.

OBJECTIVE: Osteoarthritis is a chronic, painful and disabling disease which prevalence is increasing in developing countries. Patients with osteoarthritis present a reduced synovial fluid viscoelasticity due to a reduction in concentration and molecular weight of hyaluronic acid. Currently, the main treatment used to restore the compromised rheological properties of synovial fluid is the viscosupplementation by hyaluronic acid injections that can be combined with oral anti-inflammatory drugs for pain relief. Combination of viscosupplements with chemical agents or drugs is emerging as a new str

Single-needle arthrocentesis (Shepard cannula) vs. double-needle arthrocentesis for treating disc displacement without reduction.

OBJECTIVES: To compare single- vs. double-needle arthrocentesis with viscosupplementation for treating disc displacement without reduction of the temporomandibular joint. METHODS: Fifty-six patients with a magnetic resonance imaging diagnosis of non-reducing disc displacement were included. Arthrocentesis with viscosupplementation was performed on patients in group I using the Y-shaped needle (Shepard cannula) technique; the standard double-needle technique was performed on patients in group II. Patients were evaluated on postoperative day 1, and 1, 3, and 6 months later, patient- and procedu

Intra-articular 2.5% polyacrylamide hydrogel, a new concept in the medication of equine osteoarthritis: A review.

Recent clinical and experimental trials have demonstrated that intra-articular 2.5% Polyacrylamide hydrogel (PAAG) is highly effective (82.5% free of lameness horses at 2 year follow-up), lasting and safe for the treatment of equine osteoarthritis (OA). Over the last decade, intra-articular 2.5% PAAG has shown to be a potent and promising drug in the medication of OA in horses, as no other single medical treatment for OA has such prolonged efficacy. Most of these studies were presenting some limitations. Preliminary observations on the mechanisms of action of intra-articular 2.5% PAAG support

Current Concepts in Osteoarthritis of the Ankle: Review.

Ankle osteoarthritis constitutes a large burden to society and is a leading cause of chronic disability in the United States. Most commonly, it is post-traumatic, occurs in younger individuals, and is associated with obesity. This entity presents similarly to osteoarthritis of the other joints, with the typical nonspecific symptoms of stiffness, swelling, and pain. Radiographic investigation includes four weight-bearing standard views: antero-posterior and lateral foot, mortise view of the ankle, and a specialized view of the hindfoot. In this review, we covered epidemiology, anatomy and biome

[Iatrogenic rheumatic diseases].

This article reviews, through illustrative cases, five rheumatic diseases associated with drug treatments: statins and myopathy, quinolones and tendinopathy, viscosupplementation and pseudoseptic arthritis, diuretics and gout, barbiturates and adhesive capsulitis. Due to the rarity of such cases, we lack validated therapeutic recommendations. Announcement to the adverse drug reaction reporting system is the best way to enhance our knowledge and to improve the public health.

Therapeutic Evaluation of Intra-articular Platelet-Rich Plasma, Steroid, and Viscosupplement Injections for Sacroiliitis: A Retrospective Comparative Study.

Introduction Sacroiliitis is a well-recognized cause of axial low back pain and significantly contributes to chronic cases seen in orthopedic and pain clinics. While intra-articular corticosteroid injections provide established short-term symptom relief, their long-term effectiveness remains variable. Platelet-rich plasma (PRP) has emerged as a biological alternative with regenerative potential, and viscosupplementation using hyaluronic acid offers viscoelastic and anti-inflammatory effects that may enhance joint function. This study aimed to compare the therapeutic efficacy of intra-articular

Osteoarthritis of the shoulder in under-50 year-olds: A multicenter retrospective study of 273 shoulders by the French Society for Shoulder and Elbow (SOFEC).

INTRODUCTION: Osteoarthritis (OA) of the shoulder in under-50 year-olds is rare, and treatment is delicate. Shoulder replacement incurs frequent long-term risk of progression and a high revision rate, making it unsuited to young active patients. The aim of the present study was to determine the epidemiology of shoulder OA in under-50 year-olds and to assess the clinical results of the various treatment options. HYPOTHESIS: The main study hypothesis was that well-conducted non-operative treatment can allow shoulder replacement to be postponed. The secondary hypothesis was that anatomic total s

Rheological properties and clinical efficacy of a chemically modified hyaluronic acid for joint health enhancement.

Osteoarthritis (OA) significantly impairs mobility and quality of life, with its prevalence rising due to ageing populations. Intra-articular hyaluronic acid (HA) injections, or viscosupplementation, restore synovial fluid viscosity, enhancing lubrication and joint function. Hymovis®/Hymovis ONE®, an advanced HA-based viscosupplement, incorporates hydrophobic modifications via MO.RE. technology, forming a reversible network that enhances viscoelasticity and lubrication. Hymovis exhibits self-healing properties, retaining elasticity and viscosity under mechanical stress, and demonstrates enhanc

Septic knee arthritis after intra-articular hyaluronate injection. Two case reports.

Intraarticular sodium hyaluronate injection to treat osteoarthritis is associated with minor side effects. Infections seem uncommon. We report two cases of septic knee arthritis. One patient was an 80-year-old woman who was admitted for Staphylococcus aureus knee arthritis after several intraarticular injections of sodium hyaluronate and corticosteroids. In the other patient, a 78-year-old woman, Neisseria mucosa knee arthritis occurred after a single sodium hyaluronate injection. Faultless aseptic technique is essential when administering hyaluronate viscosupplementation. Patients should be i

Nonoperative Options for Management of Articular Cartilage Disease.

Nonoperative options for articular cartilage injury are pervasive but have not shown to be curative. Recommendations for low-impact exercise and weight loss provide benefit and are a foundation for the treatment of osteoarthritis. Judicious use of NSAIDs and acetaminophen can be appropriate for pain management. Topical NSAIDs may be a treatment option with fewer side effects than its oral counterpart. Additionally, viscosupplementation injections are useful for mild to moderate knee osteoarthritis, whereas short-term pain relief is provided by intra-articular corticosteroid injections. Future

Interventional Procedures in Musculoskeletal Regenerative Medicine.

Regenerative techniques are a rapidly expanding frontier in the interventional treatment of musculoskeletal disorders, involving endogenous mechanisms and bioactive molecules to repair tissue and modulate inflammation. Despite extensive basic research, the evidence supporting clinical use is highly heterogeneous and limited by variability in cell sources, preparation protocols and patient selection, high risk of bias, small sample sizes, and short follow-up periods. This article presents a critical overview regarding viscosupplementation, prolotherapy, platelet-rich plasma, collagen, stem cell

Hyaluronan: Sources, Structure, Features and Applications.

Hyaluronan (HA) is a non-sulfated glycosaminoglycan that is present in a variety of body tissues and organs. Hyaluronan has a wide range of biological activities that are frequently influenced by molar mass; however, they also depend greatly on the source, purity, and kind of impurities in hyaluronan. High-molar-mass HA has anti-inflammatory, immunosuppressive, and antiangiogenic properties, while low-molar-mass HA has opposite properties. A number of chemical modifications have been performed to enhance the stability of HA and its applications in medical practice. Hyaluronan is widely applied
