Intraoperative monitoring of cortically recorded visual response for posterior visual pathway.

OBJECT: Intraoperative monitoring of visual evoked potentials (VEPs) has been regarded as having limited significance for the preservation of visual function during neurosurgical procedures, mainly due to its poor spatial resolution and signal-to-noise ratio. The authors evaluated the usefulness of cortically recorded VEPs, instead of the usual scalp VEPs, as intraoperative monitoring focusing on the posterior visual pathway. METHODS: In 17 consecutive patients who underwent microsurgical procedures for lesions near the posterior visual pathway, cortical responses were recorded using 1-Hz fla

Confrontation visual field techniques in the detection of anterior visual pathway lesions.

The accuracy of a variety of finger and color confrontation tests in identifying chiasmal and optic nerve visual field defects was assessed in patients whose field defects had been established beforehand by a conventional achromatic kinetic technique on the Goldmann perimeter. Kinetic and static finger confrontation methods identified an average of 42% of the 28 chiasmal hemianopic defects. False negatives included eyes with hemianopias complete to the largest (V4e) Goldmann isopter. False positives (average, 15%) occurred in eyes containing nerve fiber bundle defects with borders that fell ne

Visual loss due to optic chiasm and retrochiasmal visual pathway lesions.

PURPOSE OF REVIEW: Building on the anatomic and diagnostic approaches presented elsewhere in this issue of CONTINUUM, this article presents important differential considerations for chiasmal and retrochiasmal vision loss, useful strategies for confirming the underlying etiology, principles of their natural history, and, where appropriate, treatment strategies. RECENT FINDINGS: Although a wide variety of pathologic processes can affect the optic chiasm and retrochiasmal visual pathways, those commonly seen in neurologic practice are comparatively fewer in number. Visual electrophysiology in children with tumours affecting the visual pathway. Case reports.

In 9 children (8-14 years of age) with orbital, suprasellar or postchiasmal tumours, visual loss was studied by visual electrophysiology in relation to ophthalmologic and neuroimaging findings. Pattern electroretinography (PERG) and pattern visual evoked potentials (PVEP) to full and half-field pattern-reversal stimulation were recorded and PERG and PVEP changes were related to the tumour location. PERG wave P50 attenuation was found associated with the central retinal dysfunction in the child with orbital rhabdomyosarcoma; PVEP wave P100 delay was associated with the optic nerve dysfunction i

Pattern disappearance visually evoked cortical potential in the diseases of visual pathway.

Visually evoked cortical potentials in response to pattern-appearance, -disappearance, and -reversal stimuli were studied in normal subjects and in patients suffering from optic neuritis. It was found in normal subjects that pattern disappearance VECPs were as reliable as the pattern reversal ones. On the other hand, the appearance response showed interindividual differences which made its evaluation rather difficult. The same was true in the records of the patients. Both the mean peak latencies of the disappearance responses and those of the pattern reversal ones were prolonged significantly

Delineation of the visual pathway in paediatric optic pathway glioma patients using probabilistic tractography, and correlations with visual acuity.

BACKGROUND: Radiological biomarkers which correlate with visual function are needed to improve the clinical management of optic pathway glioma (OPG) patients. Currently, these are not available using conventional magnetic resonance imaging (MRI) sequences. The aim of this study was to determine whether diffusion MRI could be used to delineate the entire optic pathway in OPG patients, and provide imaging biomarkers within this pathway which correlate with a patient's visual acuity (VA). METHODS: Multi-shell diffusion MRI data were acquired in a cohort of paediatric OPG patients, along with VA

MRI of optic nerve and postchiasmal visual pathways and visual evoked potentials in secondary progressive multiple sclerosis.

We studied the relationship between abnormalities shown by MRI and functional disturbances in the visual pathway as assessed by the visual evoked potential (VEP) in 25 patients with established multiple sclerosis (MS); only 4 of whom had a history of acute optic neuritis. Optic nerve MRI was abnormal in 19 (76%) and is thus useful in detecting subclinical disease. Optic nerve total lesion length and area on the STIR sequence was found to correlate significantly with prolongation of the VEP latency. This may reflect a predominantly demyelinating rather than inflammatory origin for the signal ch

Pre- and Postnatal Damage to the Retro-Geniculate Visual Pathways Cause Retinal Degeneration Predictive for Visual Function.

To increase the understanding of the relationship between structure and function in individuals with damage to the brain from different stages of maturation of the visual system, we examined 16 teenagers and young adults. We used diffusion-weighted magnetic resonance imaging (MRI) and fiber tractography of the optic radiation (OR) and optical coherence tomography (OCT) of the peripapillary retinal nerve fiber layer (pRNFL) and the ganglion cell layer + inner plexiform layer (GC+IPL) in the macula. Visual field (VF) function was assessed with the Humphrey Field Analyzer (HFA). Injuries to the i

Tractometry of Human Visual White Matter Pathways in Health and Disease.

Diffusion-weighted MRI (dMRI) provides a unique non-invasive view of human brain tissue properties. The present review article focuses on tractometry analysis methods that use dMRI to assess the properties of brain tissue within the long-range connections comprising brain networks. We focus specifically on the major white matter tracts that convey visual information. These connections are particularly important because vision provides rich information from the environment that supports a large range of daily life activities. Many of the diseases of the visual system are associated with advance

DPVis: Visual Analytics With Hidden Markov Models for Disease Progression Pathways.

Clinical researchers use disease progression models to understand patient status and characterize progression patterns from longitudinal health records. One approach for disease progression modeling is to describe patient status using a small number of states that represent distinctive distributions over a set of observed measures. Hidden Markov models (HMMs) and its variants are a class of models that both discover these states and make inferences of health states for patients. Despite the advantages of using the algorithms for discovering interesting patterns, it still remains challenging fo

Signal Propagation in the Human Visual Pathways: An Effective Connectivity Analysis.

Although the visual system has been extensively investigated, an integrated account of the spatiotemporal dynamics of long-range signal propagation along the human visual pathways is not completely known or validated. In this work, we used dynamic causal modeling approach to provide insights into the underlying neural circuit dynamics of pattern reversal visual-evoked potentials extracted from concurrent EEG-fMRI data. A recurrent forward-backward connectivity model, consisting of multiple interacting brain regions identified by EEG source localization aided by fMRI spatial priors, best accoun

[Tractography of the visual pathways: routine examination in magnetic resonance imaging].

Water diffusion analysis in magnetic resonance imaging (MRI) provides an elective visualization of fiber tract orientations in cerebral white matter, especially for optic tracts. We explored 25 patients from 18 to 45 years of age, with normal MRI in 20 subjects, and radiological anomalies in five. On a 1.5 Tesla MRI apparatus, diffusion tensor acquisitions were performed in 5 minutes 58 seconds with an EPI Single Shot sequence covering the entire brain. Image displacements were precluded by patient information and adequate fixation, then digitally corrected on workstations. Volume merging and

Age-related changes in the dynamics of human albino visual pathways.

The integrity of the genetic material of bacteria is guaranteed by a set of distinct repair mechanisms. The participation of these repair systems in bacterial pathogenicity has been addressed only recently. Here, we study for the first time the participation in virulence of the MutSL mismatch repair system of Listeria monocytogenes. The mutS and mutL genes, which are contiguous in the L. monocytogenes chromosome, were identified after in silico analysis. The deduced MutS shares 62% identity with MutS of Bacillus subtilis and 50% identity with HexA, its homologue in Streptococcus pneumoniae; Mu

Sensitivity deficits consistent with aberrant crossed visual pathways in human albinos.

There is both anatomical and electrophysiological evidence that human albinos have abnormally crossed visual pathways. The present study examined some perceptual consequences of such pathways on the visual abilities of 12 oculocutaneous albinos and one ocular albino. Measurements were made of visual fields in all subjects. For seven subjects, contrast sensitivity functions were obtained for the central, nasal, and temporal retinas. There was a wide variation in the dimensions of the visual fields, but there was no evidence for a loss corresponding to the origin of the aberrant projections. On

Mutants with abnormal visual pathways: an explanation of anomalous geniculate laminae.

Rats with geniculate laminae that run perpendicular to the lines of projection are described. Earlier reports of laminae parallel to the lines of projection were based on mammalian mutants that may be relatively common. In these mutants, the chiasmatic course of axons arising in a patch of retinal ganglion cells is wrongly specified.

Enabling global processing in simultanagnosia by psychophysical biasing of visual pathways.

A fundamental aspect of visual cognition is our disposition to see the 'forest before the trees'. However, damage to the posterior parietal cortex, a critical brain region along the dorsal visual pathway, can produce a neurological disorder called simultanagnosia, characterized by a debilitating inability to perceive the 'forest' but not the 'trees' (i.e. impaired global processing despite intact local processing). This impairment in perceiving the global shape persists even though the ventral visual pathway, the primary recognition pathway, is intact in these patients. Here, we enabled global

Dendritic changes in visual pathways in glaucoma and other neurodegenerative conditions.

Visual information is sent from the retina to central visual targets through the optic nerve formed of retinal ganglion cells' (RGCs) axons. In rodents, the superior colliculus (SC) is the major site of termination of retinal axons, whilst in primates and felines, it is the lateral geniculate nucleus (LGN). Glaucoma is a progressive optic neuropathy characterized by RGC death. There is increasing evidence that neuronal changes occur both in retina and central visual targets in glaucoma and other neurodegenerative diseases such as Alzheimer's disease (AD) and amyotrophic lateral sclerosis (ALS)

Neural conduction in the visual pathways in ocular hypertension and glaucoma.

BACKGROUND: The aim of our work was to evaluate neural conduction in visual pathways in subjects with ocular hypertension and glaucoma. METHODS: We assessed simultaneous recordings of pattern electroretinograms (PERG) and visual evoked potentials (VEP) in 16 subjects with ocular hypertension (OHT), in 16 subjects with primary open-angle glaucoma (POAG) and in 15 age-matched controls. The visual stimuli were checkerboard patterns (the check edges subtend 15 min of visual arc; contrast 70%) reversed at the rate of 2 reversals/s. RESULTS: In OHT and POAG patients we found PERG and VEP latencies

The pattern-evoked potential in compression of the anterior visual pathways.

Pattern evoked responses have been recorded in 19 patients with compression of the optic nerve, chiasm or tract, verified at operation. These included 4 patients with orbital tumours, 5 with intracranial meningiomas, 2 with craniopharyngiomas and 8 with pituitary tumours. The evoked response was abnormal in all except one of these patients. The pattern of abnormalities in the response, however, differed from that in the earlier series of patients with primary demyelinating disease. The incidence of delayed responses was much lower, and the magnitude of the delays was smaller. Absent responses
