Two different pathways for the transport of primitive and definitive blood cells from the yolk sac to the embryo in humans.

During the early human embryonic period nutrients and blood cells are temporarily provided by the extraembryonic yolk sac (YS). The YS before week six is involved not only in primitive but also in definitive erythropoiesis. While the destiny of primitive erythroid cells that fill the blood vessels of the YS is well known, the final destination of erythrocytes present in the endodermal vesicular system is unknown. In the present study we have investigated, step by step, the destiny of the erythrocytes present in the endodermal vesicles during the embryonic period. Twelve human YSs and their cor

Volume and vascularity of the yolk sac assessed by three-dimensional and power doppler ultrasound.

The yolk sac is an organ of increasingly recognized importance in the initial mechanisms of pregnancy maintenance and the early growth and welfare of the embryo. The aim of our study was to assess the vascularity of the yolk sac and vitelline duct in 150 patients between the 6th and 10th weeks of normal, and uncomplicated pregnancies who were scheduled for termination of pregnancy for psychosocial reasons and 130 complicated pregnancies. In same patients volume of the yolk sac was assessed using Combison 530 3D Voluson, Medison-Kretz Company. Overall visualization rate for yolk sac vessels was

Imaging of omphalomesenteric duct remnants and related pathologies in children.

The omphalomesenteric duct (OMD) or the vitelline duct (VD) is the embryologic communication between the yolk sac and the primitive midgut. OMD or VD anomalies are a group of defects resulting from failure of involution of the OMD. Meckel diverticulum is the most common anomaly that results from failure of resorption of the OMD. Other less common anomalies seen in children include OMD fibrous band, fistula, sinus tract, cyst, and umbilical polyps. These OMD remnants can have variable clinical manifestations such as umbilical discharge, small bowel obstruction, gastrointestinal tract bleeding,

Intestinal obstruction caused by omphalomesenteric duct remnant: usefulness of laparoscopy.

The anomalies related to omphalomesenteric duct remnant constitute an uncommon cause of intestinal obstruction, of which Meckel"s diverticulum and its variants represent the most important clinical presentation. In most cases they are asymptomatic and usually affect young patients. When symptomatic, they usually present episodes of gastrointestinal bleeding or acute abdomen syndromes caused by strangulation of intestinal loops as a result of fibrous intraabdominal remnants or inflammation produced by the diverticulum. In most cases, the unexpected presence of these alterations makes intraopera

Littre Hernia: A Rare Case of an Incarcerated Meckel's Diverticulum.

A Meckel's diverticulum is a remnant of the vitelline duct, which leads to the formation of a true diverticulum containing all layers of the small intestine. The diverticulum can contain ectopic gastric, duodenal or pancreatic tissue and is the most common congenital anomaly of the gastrointestinal tract with estimates of prevalence ranging from 0.3% to 3%. The condition is usually clinically silent. In children the most common complication is gastrointestinal bleeding caused by ulceration due to the acid secretion by ectopic gastric mucosa.

A rare case presentation of a symptomatic omphalomesenteric cyst in an adult, 24-year-old patient, treated with laparoscopic resection.

A persistent omphalomesenteric duct remnant is a rare finding, which typically presents in the pediatric population. Most commonly, it is encountered in the form of a Meckel's diverticulum. In rare instances, omphalomesenteric cysts have been reported in the pediatric population. We present a case of a symptomatic omphalomesenteric cyst in an adult male and our management of this entity.

Patent omphalomesenteric duct and exomphalos minor: a rare and interesting association.

A completely patent omphalomesenteric duct was seen in association with two cases of exomphalos minor. Both the patients also had limb anomalies, and one patient had an ileal duplication and colonic atresia. Both babies underwent emergency surgery to correct the anomalies, and one survived. There have been few reports of this combination of anomalies, and there is increasing appreciation that these lesions occur together more often than by chance.

Umbilical polyp originating from urachal remnants.

Umbilical polyp is a rare disorder of the umbilical region in childhood. It is said to be the result of incomplete closure of the omphalomesenteric duct and presents as a red, moist umbilical mass after separation of the umbilical cord. Umbilical polyp originating from the urachus has not previously been reported. A 10-year-old boy with an umbilical polyp related to the urachus is presented to stress the possibility that umbilical polyps can originate not only from the omphalomesenteric duct but also from urachal remnants.

[Complications caused by remnants of the omphalomesenteric duct].

31 cases of complicated remnants of the omphalomesenteric duct are reviewed. In particular mechanical just as often as inflammatory complications are noted. There was a predominance of male infants with a mean age of 3.5 years. Surgery is recommended even for still non complicated Meckel's diverticula in childhood. There was no mortality.

Umbilical Appendix Masquerading as a Patent Omphalomesenteric Duct in a Neonate.

The umbilicus is the site of a number of well-recognized and unusual abnormalities. Well-known neonatal umbilical abnormalities include umbilical hernias, granulomas/polyps, and congenital remnants of development. In this article, we describe a rare case of an appendix draining through the umbilicus of a neonate. In the literature, there are only 15 cases with possible umbilical appendix. We describe this rare case along with a review of the literature and discuss the underlying pathophysiology.

Omphalomesenteric duct cyst as a content of omphalocele.

A rare occurrence of an omphalomesenteric duct cyst in an exomphalos minor sac is reported herein. The noteworthy points in this case were an unusual presentation of the rarest variety of umbilical cord cyst. The tapering intra-abdominal end of the cyst was found to be attached to the ileal mesentry, thereby simulating a herniating mesenteric cyst till the histolopathological report resolved the issue.

The curious umbilicus: clue to the cause of abdominal pain.

Omphalomesenteric duct derivatives cause an assortment of complications, most notably intestinal hemorrhage and obstruction. Intestinal obstruction is the most lethal complication and usually results from a diverticulum with an attachment to the umbilicus. This cause of intestinal obstruction is not generally recognized preoperatively. We report a teenager with episodic abdominal pain, acute small bowel obstruction, and a curious umbilical deformity with an underlying omphalomesenteric duct remnant. We believe that this physical finding suggests the diagnosis in patients with intestinal obstru

Matrix Gla Protein expression pattern in the early avian embryo.

MGP (Matrix Gla Protein) is an extracellular matrix vitamin K dependent protein previously identified as a physiological inhibitor of calcification and shown to be well conserved among vertebrates during evolution. MGP is involved in other mechanisms such as TGF-β and BMP activity, and a proposed modulator of cell-matrix interactions. MGP is expressed early in vertebrate development although its role has not been clarified. Previous work in the chicken embryo found MGP localization predominantly in the aorta and aortic valve base, but no data is available earlier in development. Here we examin

The umbilical mass: a rare neonatal anomaly.

Umbilical anomalies are a rare presentation in the pediatric patient. The differential diagnosis includes anomalies resulting from urachal and vitelline duct derivatives such as urachal sinus, urachal cyst, urachal diverticulum, patent urachus, herniated Meckel's diverticulum, umbilico-enteric fistula, or umbilical polyp. In this article, a case presentation of an umbilical anomaly along with the differential diagnosis and management options are discussed. Based upon this review of the literature, the authors propose a management algorithm for treating children with umbilical anomalies.

Vitelline vascular remnant causing intestinal obstruction in a patient with TARP syndrome.

BACKGROUND: TARP syndrome, characterized by talipes equinovarus, atrial septal defect, Robin sequence, and persistent left superior vena cava, is an X-linked recessive condition caused by deleterious variants in RBM10. Vitelline vascular remnants (VVR) are a rare vitelline duct anomaly with approximately 26 cases previously reported. There are no previously reported cases of VVRs in patients with TARP syndrome. CASE: We present a male neonate diagnosed with TARP syndrome via trio whole exome sequencing who had classic features of this syndrome, although his course was additionally complicated

Perforation of Meckel's diverticulum with ectopic gastric mucosa.

Meckel's diverticulum is a congenital anomaly of the gastrointestinal tract that occurs in approximately 2% of the population. It is the result of incomplete obliteration of the vitelline duct (ductus omphaloentericus). During the fetal period, this duct connects the yolk sac with the primitive intestine and normally closes around the 5th-7th week of gestation. It is a true diverticulum because it contains all layers of the intestinal wall. Most diverticula are asymptomatic; however, they may lead to serious complications such as inflammation, perforation, obstruction, and bleeding. Our case r
