Recurrent retinal detachment after diabetic vitrectomy.

PURPOSE: To investigate the characteristics of recurrent retinal detachment (RD) after diabetic vitrectomy (DV). METHODS: Consecutive cases underwent vitrectomy for recurrent RD after DV was collected and separated into the following four groups for analysis: rhegmatogenous RD (RRD), pure tractional RD (Proliferative Subgroup 1), combined RD with proliferative tissue within the equator (Proliferative Subgroup 2), and combined RD with both posterior and peripheral proliferations (Proliferative Subgroup 3). RESULTS: Of the 41 cases enrolled, retinal reattachment was achieved in 73.2%. Over all

Long-term survival following diabetic vitrectomy.

AIM: To update long-term survival data on patients with proliferative diabetic retinopathy undergoing vitrectomy and to identify associated risk factors. METHODS: Retrospective clinical record review at a single New Zealand tertiary referral centre. A total of 182 eyes that underwent a vitrectomy for a diabetic vitreous haemorrhage and/or tractional retinal detachment between March 2000 and December 2010 were included. Kaplan-Meier survival curves and Cox-regression analyses were performed for survival rates and associated risk factors. RESULTS: The mean age of patients was 55 years (range 2

Microincision vitrectomy surgery for myopic foveoschisis.

OBJECTIVE: To evaluate the technical feasibility and surgical outcome of microincision vitrectomy surgery (MIVS) in patients with myopic foveoschisis (MF). METHODS: In total, 33 eyes of 29 patients who underwent surgical intervention due to MF were included. The patients were diagnosed with MF by spectral-domain optical coherence tomography (SD-OCT) and followed up for 6 months after MIVS that included internal limiting membrane (ILM) peeling with or without intraocular gas tamponade. To identify factors affecting visual outcome, the best-corrected visual acuity, axial length, and SD-OCT find

Cataract surgery after pars plana vitrectomy.

PURPOSE OF REVIEW: To review recent studies and advances and their possible implications in the care of patients undergoing cataract surgery after pars plana vitrectomy. RECENT FINDINGS: Optical biometry has shown to be superior to ultrasound biometry in vitrectomized eyes but still not achieving as good results as it does in nonvitrectomized eyes. Blue light-filter intraocular lenses, with their possible advantage of macular protection, have shown no operative or functional disadvantages in vitrectomized eyes, and thus their routine use can be justified. However, presbyopia-correcting intrao

Sutureless vitrectomy: evolution and current practices.

This review presents the evolution of sutureless vitrectomy and describes the minimisation of surgical techniques in the posterior segment, as well as their advantages and disadvantages. Shorter operating time, improvement of patient comfort and faster visual recovery are feasible when using small-diameter sutureless vitrectomy technique and equipment. However, a number of associated problems, such as postsurgical hypotony and endophthalmitis, were noticed initially in higher rates compared to 20-gauge vitreoretinal surgery. The purpose of this review is to summarise the available information

[Vitrectomy for endophthalmitis after cataract surgery].

PURPOSE: To identify risk factors of poor visual outcome with vitrectomy for early-onset endophthalmitis after cataract surgery. PATIENTS AND METHODS: Clinical records of 29 consecutive eyes with endophthalmitis developing within 6 weeks after cataract surgery and that underwent therapeutic vitrectomy between June 1996 and April 2001 were retrospectively reviewed. Twenty-two of the eyes received intravitreal injections of vancomycin and ceftazidime at the time of vitrectomy, and all patients received intravenous antibiotics. Eyes were divided into two groups; group A consisted of 22 eyes with

Vitrectomy for ciliary block (malignant) glaucoma.

BACKGROUND: Ciliary block (malignant) glaucoma is a rare surgical complication occurring in patients with pre-existing glaucoma. Misdirected aqueous fluid causes forward movement of the lens/iris diaphragm, shallowing the central and peripheral anterior chamber. Although most patients with ciliary block respond to medical or laser therapy, those with refractory glaucoma often require pars plana vitrectomy to normalize aqueous flow. METHODS: The medical records of 21 consecutive patients with refractory ciliary block glaucoma treated by pars plana vitrectomy were reviewed retrospectively to de

[Vitreous hemorrhage: attitudes in therapy, vitrectomy].

Vitreous haemorrhage is a serious complication of proliferative diabetic retinopathy: it provides evidence of the severity of at proliferative retinopathy at stake and precludes laser photocoagulation. In some cases of moderate vitreous haemorrhage, panretinal photocoagulation remains possible especially using longer wavelengths such as krypton red; cryotherapy under ophthalmoscopic control may be an alternative to panretinal photocoagulation. Development of pars plana vitrectomy, however, has been the main break the rough in the management of severe vitreous haemorrhage. Pars plana vitrectomy

[Vitrectomy in oculocerebral non-Hodgkin's lymphoma].

Ocular non-Hodgkin lymphoma (NHL) is thought to be a rare intraocular tumor; the clinical diagnosis, however, is difficult, and ocular NHL might occur more frequently than we thought. Vitrectomy is essential for establishing the diagnosis. During the last 6 years, we performed vitrectomies for ocular NHL in seven eyes of four patients. We report on the visual outcome and cytological findings. In three patients, we suspected the oculocerebral form of the disease; one patient suffered from a systemic lymphoma. The mean observation time was 19 months (5-30 months). In one patient, we failed to ex

Combined phacoemulsification and pars plana vitrectomy.

Combined phacoemulsification and pars plana vitrectomy were performed in eight eyes with cataract and vitreoretinal disease. Six eyes had tractional retinal detachment and/or nonclearing vitreous hemorrhage, one eye had a retained metallic intraocular foreign body, and one eye had a traumatic retinal detachment. In six eyes, a posterior chamber lens implant was placed in the capsular bag. Postoperative visual acuity improved in each case and ranged between 20/15 and 6/200. There were no perioperative complications, and the posterior chamber lens implant appeared well tolerated during an averag

Vitrectomy surgery in proliferative sickle retinopathy.

Visual loss secondary to vitreous hemorrhage or retinal detachment can occur in proliferative sickle retinopathy. Severe complications including a high incidence of anterior segment necrosis have been reported in both vitrectomy and scleral buckling operations in patients with this disease. We examined four patients with longstanding vitreous hemorrhages or retinal detachments that were treated successfully with standard vitrectomy surgery techniques or scleral buckling procedures without serious complications.

Pars plana vitrectomy for asteroid hyalosis.

Asteroid hyalosis is a common, usually asymptomatic, benign vitreous disease affecting older individuals. A case report of an aphakic patient who developed anterior segment complications with no visualization of the fundus is presented. A pars plana anterior and posterior vitrectomy was performed using the Ocutome system. An excellent technical result with moderate restoration of vision was achieved. Preoperative evaluation with ultrasound was helpful. Pathology confirmed the diagnosis of calcium-containing lipids. Other indications for vitrectomy for asteroid hyalosis include retinal detachme

Pars plana vitrectomy in ocular trauma.

Visual improvement was achieved in 62% of 100 consecutive patients with ocular trauma treated by pars plana vitrectomy. Anterior segment injuries had a better prognosis than posterior segment injuries, and retinal detachment was a poor prognostic sign. Patients undergoing vitrectomy during the two weeks after injury had a better visual prognosis than those who had delayed vitrectomy. Pars plana vitrectomy has increased the recovery rate in traumatized eyes which previously were deemed inoperable and frequently were enucleated. Most such eyes have intraocular fibrocellular proliferations, resul

[Controlled suction in vitrectomy (author's transl)].

A peristaltic pump (type "Perpex", J. H. Guldener, Zürich, Switzerland) was modified for controlled aspiration in vitrectomy (fig. 1). The surgeon operates start, instantaneous stop and back flush with a single foot pedal. Suction and cutting device are controlled independently. Suction flow can continuously be varied between 0 and 16 ml/min (fig. 2). This less critical function is operated by the assistent. The upper limit of suction flow depends on infusion resistance and preselected differential pressure (fig. 3). The infusion resistance and its reciprocal, the inflow rate, vary considerabl

Anterior segment applications of vitrectomy techniques.

We have used vitrectomy instrumentation and techniques to treat a variety of conditions affecting the anterior segment, including: (1) Various conditions causing an occluded or inadequate pupillary space, (2) Complications of vitreous gel in the anterior chamber, (3) Growth of epithelium into the anterior chamber, (4) Selected indications for lens surgery, (5) Flat anterior chamber, (6) Total hyphaema. The vitrectomy methods provided improved capabilities, including controlled excision of intraocular tissues under optimum visualization using a closed-eye system with normal intraocular pressure

Sympathetic uveitis after trauma and vitrectomy.

A case is describes in which the earliest clinical manifestations of sympathetic uveitis that occurred after trauma and pars plana vitrectomy were confined primarily to the retina, retinal pigment epithelium, and choroid posteriorly, and resembled focal lesions of acute posterior multifocal placoid pigment epitheliopathy. Similar focal lesions were noted on gross examination of the enucleated inciting eye. Histopathological examination revealed that these lesions were caused by focal choroidal granulomas and overlying Dalen-Fuchs nodules. Sympathetic ophthalmia was also noted in four other pat

Vitreous prolapse in cataract surgery or during the extraction of a dislocated lens; corneal dystrophy in aphacic eyes due to corneo-vitreous contact and vitreous invading the anterior chamber following perforating injury proved to be valid indications for partial transpupillary vitrectomy. In malignant glaucoma, in postoperative wound rupture following cataract extraction, in open-sky-surgery of the anterior segment of aphacic eyes and in other particular conditions, transpupillary vitrectomy may also be considered. The transpupillary approach is not indicated in retinal detachment, unless pr
