Gross anterior segment ischaemia following vitreoretinal surgery for sickle-cell retinopathy.

We report the case of a 32-year-old Afrocaribbean man with known stage 3 proliferative sickle-cell retinopathy who presented with a mixed picture of tractional and rhegmatogenous macula off detachment. He underwent left primary 25 g vitrectomy with silicone oil, delamination and endolaser photocoagulation under a general anaesthetic. He, however, presented 48 hours postoperatively with gross anterior segment ischaemia. His pain and ocular signs settled over the course of a few days following administration of supplemental oxygen, oral steroids, analgesia and intravenous hydration. Examination

Reliability of standardized echography before vitreoretinal surgery for proliferative diabetic retinopathy.

Standardized A- and B-scan echography was performed in 45 patients (68 eyes) affected by proliferative diabetic retinopathy, before vitreoretinal surgery, to evaluate the percentage of agreement between the ultrasonographic data and the surgical findings. The agreement was quite good for tractional retinal detachment (93%) and for posterior vitreous detachment (91%). The main cause of failure was the presence of vitreoschisis, while preretinal and vitreal membranes can easily be detected.

Central retinal artery occlusion on postoperative day one after vitreoretinal surgery.

PURPOSE: To report two cases of central retinal artery occlusion (CRAO) associated with vitreoretinal surgery. OBSERVATIONS: Two patients underwent vitreoretinal surgery and were diagnosed with CRAO on postoperative day one. Both had received retrobulbar anesthetic blocks, followed by pars plana vitrectomy in one patient and scleral buckling in the other patient. Best-corrected visual acuity at last follow-up was 20/40 and 20/400. CONCLUSIONS/IMPORTANCE: CRAO is a rare but serious adverse event after vitreoretinal surgery. The causative mechanism is not known in these patients.

Perfluorocarbon liquid: its application in vitreoretinal surgery and related ocular inflammation.

The application of perfluorocarbon liquids has been well acclaimed in vitreoretinal surgery. Its unique physical properties make it an ideal intraoperative tool to improve the efficiency and safety of surgical procedures in complicated cases. The main functions of perfluorocarbon liquids in vitreoretinal surgery include relocating and fixing the detached retina, displacing the subretinal and subchoroidal to fluid anteriorly, revealing proliferative vitreous retinopathy (PVR) for further maneuvers, protecting the macula from exposure to chemicals with potential toxicity, and assisting the remov

Endoscopy-guided vitreoretinal surgery following penetrating corneal injury: a case report.

INTRODUCTION: Severe ocular trauma requires emergency surgery, and a fresh corneal graft may not always be available. We describe a case of perforating eye injury with corneal opacity, suspected endophthalmitis, and an intraocular foreign body. The patient was successfully treated with a two-step procedure comprising endoscopy-guided vitrectomy followed by corneal transplantation. This surgical technique offers a good option to vitrectomy with simultaneous keratoplasty in emergency cases where no graft is immediately available and there is the possibility of infection due to the presence of a

[Binocular vision after secondary lens implantation following vitreoretinal silicon oil surgery].

In 10 cases, aphakia after silicone oil surgery was corrected as follows: after completion of all retinal interventions, but not less than 3 months after silicone oil removal, an anterior chamber lens (Kelman Multiflex) was implanted. Binocular vision was interrupted for periods ranging from 4 months to 4.6 years (mean 2 years). Tests of the recovered binocular vision, on average 13 months after implantation of the lens, revealed simultaneous vision in 7 cases and exclusion in 2 (primary amblyopia; secondary divergent squint). Stereoscopic vision of 3800" was achieved in 4 cases, and of 200" i

Vitreoretinal and anterior segment surgery through the pars plana: part I.

Instrumentation and clinical experience dealing with intraocular surgery through a pars plana approach have advanced rapidly during the past 5 years. These techniques have been used to successfully treat a number of ocular disorders. Considerable experience with this form of surgery has accumulated, and the results and incidence of complications are similar among centers. Additional refinements in instrumentation and surgical techniques will probably expand the capabilities and further improve the results of pars plana surgery. However, we are rapidly approaching a point at which the primary l

The Impact of Vitreoretinal Surgery in Patients with Uveitis: Current Strategies and Emerging Perspectives.

Uveitis constitutes a heterogeneous group of intraocular inflammatory pathologies, including both infectious and non-infectious aetiologies, often leading to substantial morbidity and permanent loss of vision in up to 20% of the affected cases. Visual impairment is most prominent in intermediate, posterior, or panuveitis and is commonly associated with cystoid macular oedema, epiretinal membranes, macular holes, and retinal detachment. In the context of uveitis, these complications arise as a result of recurrent flare-ups or chronic inflammation, contributing to cumulative ocular damage. Pars
