Psychological factors associated with perception of experimental pain in vulvar vestibulitis syndrome.

This study assessed the association between pain perception and psychological variables in women with vulvar vestibulitis syndrome (VVS) by comparing 28 VVS women with 50 healthy women. We assessed non genital systemic pain perception with quantitative sensory testing by administering experimental pain stimuli to the forearm. The VVS women demonstrated a lower pain threshold and a higher magnitude estimation of pain, combined with a higher trait anxiety, increased somatization, and a lower body image. Among the VVS women, nonvaginal pain catastrophizing was significantly related to reported pa

Use of oral contraceptive pills and vulvar vestibulitis: a case-control study.

Vulvar vestibulitis is characterized by superficial pain during intercourse. Exploratory studies have suggested that oral contraceptives (OCs) could be associated with occurrence of vulvar vestibulitis. This 1995-1998 case-control study in Québec, Canada, sought to reassess this association. Included were 138 women with vulvar vestibulitis whose symptoms had appeared in the previous 2 years and 309 age-matched controls who were consulting their physicians for reasons other than gynecologic problems or contraception. Cases and controls were interviewed to obtain a detailed history of OC use and

Vulvar vestibulitis in the north of Sweden. An epidemiologic case-control study.

OBJECTIVE: To determine the epidemiologic characteristics of patients with vulvar vestibulitis. STUDY DESIGN: During the years 1992-1994, 32 women with vulvar vestibulitis were referred to the Vulva Clinic, University Hospital, Umeå, Sweden. They were asked questions regarding age, age at first intercourse, pregnancies, number of lifetime partners, use of oral contraceptives and past gynecologic history, including infections and previous treatments. The cases were compared to 17 healthy controls. For statistical evaluation, the two-tailed Wilcoxon rank sum test was used. RESULTS: There were

Vulvar vestibulitis subjects undergoing surgical intervention: a descriptive analysis and histopathological correlates.

OBJECTIVE: We describe here a series of selected patients from an established vaginitis research clinic diagnosed with vulvovestibulitis (VV) who underwent surgical intervention for focal disease. Long-term results of surgical correction are reported and characteristic histopathology findings associated with vulvar vestibulitis are emphasized. STUDY DESIGN: A retrospective chart review was carried out to extract relevant clinical, histologic, and outcome data. Tissue blocks of resected specimens were re-examined for specific inflammatory response. RESULTS: Complete data and long-term follow

Vulvar vestibulitis: prevalence and historic features in a general gynecologic practice population.

All gynecologic patients seen by the author during a 6-month period were questioned and examined by means of a swab test to determine the prevalence of vulvar vestibulitis and the normal variation in sensitivity of vestibular skin. Of 210 patients, 78 (37%) had some degree of positive testing. A total of 31 patients (15%) were found to fulfill the definition of vulvar vestibulitis. A questionnaire was administered to these patients as well as to seven patients in whom vestibulitis had been previously diagnosed. A total of 50% had always had pain, most since their teenage years. Their history w

Vaginismus: an important factor in the evaluation and management of vulvar vestibulitis syndrome.

Dyspareunia is a common complaint in general gynecological practice. Many patients with dyspareunia suffer from vulvar vestibulitis syndrome (VVS). This syndrome constitutes severe pain on vestibular contact or attempted vaginal entry, tenderness to pressure within the vulvar vestibule and physical findings of vulvar erythema of various degrees. As a last resort, and only when all conservative treatments have failed, surgery is attempted. It has been our observation that a considerable percentage of patients with VVS present with concomitant vaginismus. Furthermore, surgery is less successful

Long-term well-being after surgical or conservative treatment of severe vulvar vestibulitis.

OBJECTIVE: To compare long-term well-being of women who needed surgery or did not need surgery in the treatment of severe vulvar vestibulitis syndrome. We also attempted to identify factors explaining differences in the treatment response. DESIGN: An observational case-control study. SETTING: University Hospital Vulva clinic. POPULATION: Sixty-six women diagnosed with severe vulvar vestibulitis and treated initially by conservative management during 1994-2005. Thirty-nine women did not respond and underwent posterior vestibulectomy and 27 were managed without surgery. METHODS: Baseline pat

Cromolyn cream for recalcitrant idiopathic vulvar vestibulitis: results of a placebo controlled study.

OBJECTIVE: Patients with chronic idiopathic vulvar vestibulitis have increased mast cells when biopsied, and cromolyn has been suggested as a treatment. The purpose of this study was to assess the efficacy of 4% cromolyn cream in women with vulvar vestibulitis. METHODS: A prospective, double blind, randomised, placebo controlled study was initiated at two centres. Patients with vulvar vestibulitis were assigned to apply cromolyn or placebo cream to the vestibule. Symptoms (burning, irritation) and signs (erythema, extent of erythema, tenderness) were recorded on a 0-3 scale. In the sexually a

Effectiveness of hypnosis for the treatment of vulvar vestibulitis syndrome: a preliminary investigation.

INTRODUCTION: Vulvar vestibulitis syndrome (VVS) is a common cause of vulvar pain. Therapeutic options target different pain systems believed to be involved in its development and maintenance. Most treatments target the pain component with the assumption that sexual function will increase once the pain has decreased, yet this is not necessarily the case. AIMS: Research has supported the effectiveness of hypnosis for many chronic pain disorders, and a case report demonstrated pain reduction and an increase in intercourse pleasure in a woman with VVS. This preliminary study examined the effecti

Vulvar vestibulitis and risk factors: a population-based case-control study in Oslo.

Vulvar vestibulitis is a major cause of entry dyspareunia in young women. The aim of this study was to evaluate a self-reported history of bacterial vaginosis, candidiasis, use of oral contraception and nulli-pregnancy as risk factors for vestibulitis. A retrospective examination of medical records was performed for 45 patients with vestibulitis from a vulvar clinic in Oslo, median age 24 years, age range 19-49 years. Four controls per case were selected randomly from the same Oslo source population as the cases. Age-matching was not performed, as matching does not control for confounding in t

Surgical treatment of vulvar vestibulitis syndrome: outcome assessment derived from a postoperative questionnaire.

INTRODUCTION: Vulvar vestibulitis syndrome (VVS) is the most common pathology in women with sexual pain. Surgery for VVS was first described in 1981. Despite apparently high surgical success rates, most review articles suggest that surgery should be used only "as a last resort." Risks of complications such as bleeding, scarring, and recurrence of symptoms are often used to justify these cautionary statements. However, there are little data in the peer-reviewed literature to justify this cautionary statement. AIMS: To determine patient satisfaction with vulvar vestibulectomy for VVS and the ra

Itch and burning pain in women with partial vaginismus with or without vulvar vestibulitis.

Fifty-three women with partial vaginismus with or without vulvar vestibulitis and 27 asymptomatic women estimated sensations of burning pain and itch at 20 standardized moments during a standardized penetration situation, including vaginal muscle contractions. Forty-three women with partial vaginismus (81.1%) reported burning pain, 23 (43.4%) itch, and 22 (41.5%) both complaints, compared to 0% of the asymptomatic women. In 17 of 22 cases, burning pain preceded the appearance of itch and in four cases the two complaints coincided. The median time from the moment when burning pain started until

Histologic and biomolecular aspects of papillomatosis of the vulvar vestibule in relation to human papillomavirus.

OBJECTIVE: To determine whether human papillomavirus (HPV) plays a role in the genesis of papillomatosis of the vulvar vestibule. METHODS: We conducted a study based on molecular hybridization and histology of biopsy material obtained from the inner surface of the labia minora of 25 women with papillomatosis of the vulvar vestibule who presented no abnormal clinical, cytologic, or colposcopic changes in the cervix or vagina. These women were compared with 24 women with condyloma acuminatum of the vulvar vestibule and with ten women with normal vulvar epithelium and no cervicovaginal changes.

Increased blood flow and erythema in the posterior vestibular mucosa in vulvar vestibulitis(1).

OBJECTIVE: To evaluate vascular changes as a possible underlying cause of mucosal erythema in women with vulvar vestibulitis. METHODS: Laser Doppler perfusion imaging was used to map the superficial blood flow in the vestibular mucosa in 20 women with vestibulitis and in 21 healthy control subjects. A possible correlation between perfusion values and graded erythema (1-5) around the vaginal introitus was analyzed. Changes in microvascular density in the posterior part of the mucosa were investigated in sections from ten patients and ten controls by a computer-assisted image-processing program

Hyperinnervation and mast cell activation may be used as histopathologic diagnostic criteria for vulvar vestibulitis.

OBJECTIVE: Vestibulitis is currently diagnosed based only on clinical criteria. To achieve histopathological diagnostic criteria, we carried out a computerized image analysis method. METHODS: Vestibular tissues removed from 40 women with severe vestibulitis were immunostained for mast cell count and degranulation by C-kit and mast cell tryptase, respectively. Vestibular nerve cells total area was evaluated after S-100 stain. Controls were 7 women aged 18-48. The images were converted to a digital signal, and analyzed using Image Proplus V4 software. RESULTS: We found a significant increase i
