The paradox of obesity cardiomyopathy and the potential for weight loss as a therapy.

Obesity is an independent risk factor for developing heart failure and the combination of the two disease states will prove to be a significant health burden over the coming years. Obesity is likely to contribute to the development of heart failure through a variety of mechanisms, including structural and functional changes, lipotoxicity and steatosis and altered substrate selection. However, once heart failure has developed, it seems that obesity confers a beneficial influence on prognosis in what has been termed the 'obesity paradox'. This may be a statistical phenomenon, but it should be co

Developing implementation strategies for a community-based digital weight management program: A hybrid deductive-inductive thematic analysis grounded in implementation theory.

OBJECTIVES: Community-based digital weight management programs have shown potential to address the public health concern of obesity, but often face implementation barriers. This study aimed to identify and evaluate current implementation barriers, facilitators, and further develop strategies. METHODS: A prospective qualitative study was conducted via workshops of stakeholders from community-based weight loss management services across Singapore, alongside a separate series of interviews for adopters. A hybrid deductive-inductive approach to thematic analysis was then adopted, informed by the

Feedback and Strategies From People With Intellectual Disability Completing a Personalized Online Weight Loss Intervention: A Qualitative Analysis.

Coaching log notes for 15 participants from a 24-week blended online and telehealth randomized controlled trial were analyzed using thematic analysis and analyst triangulation to determine the factors that facilitated participant adherence to weight loss strategies, use of technology, and motivational interviewing. Several participants reported that restricting processed carbohydrates, limiting portion size, and maintaining healthy substitutions were effective nutritional strategies. Participants were less successful with adherence to their exercise goals, often due to time constraints and a l

Economic evaluation of telephone-based weight loss support for patients with knee osteoarthritis: a randomised controlled trial.

BACKGROUND: The prevalence of knee osteoarthritis is increasing worldwide. Obesity is an important modifiable risk factor for both the incidence and progression of knee osteoarthritis. Consequently, international guidelines recommend all patients with knee osteoarthritis who are overweight receive support to lose weight. However, few overweight patients with this condition receive care to support weight loss. Telephone-based interventions are one potential solution to provide scalable care to the many patients with knee osteoarthritis. The objective of this study is to evaluate, from a societa

Third-wave cognitive behaviour therapies for weight management: systematic review and network meta-analysis protocol.

INTRODUCTION: Behavioural and cognitive behavioural programmes are commonly used to assist with weight management, but there is considerable scope to improve their effectiveness, particularly in the longer term. Third-wave cognitive behaviour therapies (CBTs) have this potential and are increasingly used. This systematic review will assess the effect of third-wave CBTs for weight management on weight, psychological and physical health outcomes in adults with overweight or obesity. METHODS AND ANALYSIS: The systematic review will be reported according to the Preferred Reporting Items for Syste

The recruitment of adolescents with obesity to a randomised controlled trial: A Fast Track to Health sub-study.

AIMS: Behavioural weight management interventions facilitate short to medium-term weight and cardiometabolic improvements in adolescent obesity. However, recruiting adolescents to trials of such interventions is challenging. This study describes strategies used to recruit adolescents with obesity into the Fast Track to Health (Fast Track) trial, conducted at two tertiary paediatric centres in Australia. METHODS: Fast Track (HREC/17/SCHN/164) was a randomised controlled trial that recruited 13-17-year-olds with obesity and ≥ 1 cardiometabolic complication, from December 2017 to March 2022. Fam

CONSORT to community: translation of an RCT to a large-scale community intervention and learnings from evaluation of the upscaled program.

BACKGROUND: Translation encompasses the continuum from clinical efficacy to widespread adoption within the healthcare service and ultimately routine clinical practice. The Parenting, Eating and Activity for Child Health (PEACH™) program has previously demonstrated clinical effectiveness in the management of child obesity, and has been recently implemented as a large-scale community intervention in Queensland, Australia. This paper aims to describe the translation of the evaluation framework from a randomised controlled trial (RCT) to large-scale community intervention (PEACH™ QLD). Tensions be

Effectiveness of a Hospital-Based Multidisciplinary Pediatric Weight Management Program: Two-Year Outcomes of PHIT Kids.

OBJECTIVE: For children with obesity, long-term sustainability of weight loss after treatment is difficult to achieve. This study examined 2-year anthropometric outcomes of a moderately intensive group behaviorally based weight management program. METHODS: One hundred seventy-three children with obesity ages 8-18 years participated with their parent or adult caregiver in a 24-week multicomponent intervention, which was followed by monthly sessions for a total of 2 years. Children were considered treatment completers if they attended ≥50% of the 24 weekly sessions. A multilevel model (multiple

The Impact of COVID-19 Lockdown on Weight Loss Program in Infertile Polycystic Ovary Syndrome Women with Obesity.

INTRODUCTION: Lockdown due to the COVID-19 pandemic has caused gym closures and adjustments to ongoing weight loss programs were needed in order to keep the programs running. Little is known whether adjustments affected the success of weight reduction. Weight reduction of at least 5% is related to improved fertility and better pregnancy outcomes in obese women. This study compared success of the weight loss program in infertile polycystic ovary syndrome women with obesity who attended the program before and during lockdown due to COVID-19 pandemic. Furthermore, we checked whether there were an

The cost of diabetes in Canada over 10 years: applying attributable health care costs to a diabetes incidence prediction model.

INTRODUCTION: Our objective was to estimate the future direct health care costs due to diabetes for a 10-year period in Canada using national survey data, a validated diabetes risk prediction tool and individual-level attributable cost estimates. METHODS: We used the Diabetes Population Risk Tool to predict the number of new diabetes cases in those aged 20 years and above over a 10-year period (to 2022), using 2011 and 2012 Canadian Community Health Survey data. We derived attributable costs due to diabetes from a propensity-matched case control study using the Ontario Diabetes Database and o

Which behaviour change techniques within interventions to prevent weight gain and/or initiate weight loss improve adiposity outcomes in young adults? A systematic review and meta-analysis of randomized controlled trials.

Young adulthood is associated with the highest rate of weight gain compared with any other adult age group. This review evaluates the effectiveness of interventions with adiposity outcomes among young adults and identifies which behaviour change techniques (BCTs) are most effective. BCT utilization was assessed using Michie's 93-item BCT Taxonomy v1. Six electronic databases were searched for randomized controlled trials assessing change in adiposity in young adults (17-35 years) until December 2019; identifying 21,582 articles. Fifty-one studies were included. Meta-analyses for weight (n=19 s

Digital Engagement Phenotypes and 6-Month Weight Loss in a Tirzepatide-Supported Digital Weight-Loss Program: A Retrospective Cohort Study.

Background and Objectives: Real-world persistence with medication-supported weight management programs is often low. While digital weight loss services (DWLS) provide multi-modal digital supports to improve engagement and counter attrition, the existing literature frequently relies on unidimensional or binary classifications of user engagement. This study used unsupervised machine learning to identify distinct digital engagement phenotypes and evaluated their independent associations with 6-month weight loss outcomes in patients prescribed tirzepatide. Materials and Methods: This retrospective

Weight management interventions for pregnant women from minority ethnic populations living in the United Kingdom: a scoping review.

BACKGROUND: Maternal obesity disproportionately affects women from minority ethnic backgrounds in the United Kingdom, who face substantially higher risks of gestational diabetes mellitus, hypertensive disorders, and maternal mortality than White British women. Despite well-documented disparities and relevant National Institute for Health and Care Excellence guidance, the evidence base for culturally appropriate weight management during pregnancy remains poorly mapped. AIM: To map and synthesize available evidence on weight management interventions during pregnancy among minority ethnic women
