Transcortical sensory aphasia: revisited and revised.

Transcortical sensory aphasia (TSA) is characterized by impaired auditory comprehension with intact repetition and fluent speech. We induced TSA transiently by electrical interference during routine cortical function mapping in six adult seizure patients. For each patient, TSA was associated with multiple posterior cortical sites, including the posterior superior and middle temporal gyri, in classical Wernicke's area. A number of TSA sites were immediately adjacent to sites where Wernicke's aphasia was elicited in the same patients. Phonological decoding of speech sounds was assessed by audito

The disturbance of nonverbal functions in dysphasia.

Various modalities of six neuropsychological functions (graphia, calculia, finger gnosis, right-left orientation, praxia and constructive praxia) referred to as parietal or nonverbal have been investigated in the light of speech disorders. We examined 20 patients with brain lesion of vascular origin, who met the diagnostic criteria of mild and moderate dysphagia, 13 patients with Wernicke's and 7 with Broca's dysphasia. Verbal and nonverbal functions in patients with ischemic focuses of the speech area of the left hemisphere were investigated the Boston Diagnostic Aphasia Examination (BDAE). T

MRI Findings In Acute Wernicke's Encephalopathy, Caused By Hyperemesis Gravidarum.

A 25-year old pregnant female with history of confusion and drowsiness for 02 days was referred by neurophysician for MRI brain. MRI demonstrated T2W/FLAIR hyper intensities in medial thalami, periaqueductal areas with variable diffusion restriction, apparent as hyper intense signal on DWI and no signal change on ADC mapping that was typically consistent with Wernicke's encephalopathy. A high index of suspicion is necessary, as delayed or lack of treatment can lead to high morbidity and mortality.

[A case of pure word deafness after cerebral hemorrhage].

The authors present the findings of an 81-year-old female who demonstrated pure word deafness after suffering a cerebral hemorrhage. Her speech was well articulated, but she could not comprehend spoken words. She responded correctly to written instructions, environmental noises and music. We consider that pure word deafness in this case was caused by the interruption of auditory inputs to Wernicke's area from both hemispheres by the hemorrhage.

Diffusion-weighted MR findings in a reversible case of acute Wernicke encephalopathy.

We report a case of acute Wernicke encephalopathy (WE) in which apparent diffusion coefficient maps showed areas of increased diffusion in the bilateral medial thalami that corresponded to the hyperintense lesions on T2-weighted imaging. The hyperintense lesions on T2-weighted imaging disappeared with full recovery from symptoms. These findings suggest that the hyperintense lesions of the acute changes of WE include reversible vasogenic edema and are not caused by acute ischemia.

Gadolinium-enhanced MR findings in a pediatric case of Wernicke encephalopathy.

This report describes the postcontrast MR findings of Wernicke encephalopathy seen in a malnourished 11-year-old boy. The examination showed increased signal on T2-weighted images in the periaqueductal gray matter and medial thalami. On T1-weighted acquisition, these areas showed decreased signal intensity, but on postcontrast T1-weighted examination, they showed moderately intense enhancement. Also noted on the postcontrast examination was mamillary body enhancement.

Speech production: Wernicke, Broca and beyond.

We investigated the brain systems engaged during propositional speech (PrSp) and two forms of non- propositional speech (NPrSp): counting and reciting overlearned nursery rhymes. Bilateral cerebral and cerebellar regions were involved in the motor act of articulation, irrespective of the type of speech. Three additional, left-lateralized regions, adjacent to the Sylvian sulcus, were activated in common: the most posterior part of the supratemporal plane, the lateral part of the pars opercularis in the posterior inferior frontal gyrus and the anterior insula. Therefore, both NPrSp and PrSp were

Duodenal Lymphocytosis and B1 Deficiency: Unveiling the Overlap Between Gut and Brain.

Thiamine (vitamin B1) deficiency is a reversible, yet potentially fatal condition that affects both the central and peripheral nervous systems. Although commonly linked to chronic alcoholism, non-alcoholic causes, including malabsorption syndromes, are increasingly being recognized. We report the case of a 22-year-old woman who presented with progressive lower limb weakness, gait disturbances, and confusional symptoms, following a history of intermittent diarrhea and vomiting. Neurological examination revealed ascending motor deficits, areflexia, cerebellar syndrome, and internuclear ophthalmo

Wernicke-Korsakoff Syndrome in Hyperemesis Gravidarum: A Case Report and Literature Review.

BACKGROUND: Wernicke-Korsakoff syndrome (WKS) is a triad of ophthalmoplegia, ataxia and memory deficits due to thiamine insufficiency resulting from under- or untreated Wernicke's encephalopathy (WE), which may be associated with hyperemesis gravidarum (HEG). CASE PRESENTATION: We present a case of a 36-year-old Filipino woman in her first trimester with HEG, along with the WKS triad and abnormal hyperintense signals in the bilateral thalami and midbrain, left occipital lobe, periaqueductal gray matter and pontine periventricular areas. Neurologic deficits partially improved but persisted des

Brain lesions in alcoholics: neuropathological observations.

Examination of the autopsy material from a city hospital in Oslo gave the following results: 1. The brain weight of male alcoholics was significantly lower than that of age-matched controls in all 10-year age groups from 40 to 69 years. The mean weight reduction was 31 g. 2. Combined morphometric and histologic studies demonstrated atrophy of the anterior vermis in 42 per cent of all male alcoholics under 70 years of age. In cases over 70 years, 33 per cent of the non-alcoholic controls showed atrophy of the anterior vermis. This is assumed to be a consequence of physiologic ageing. The preval

Clinical forms of aphasia.

A survey is given on the history of knowledge of aphasia and on the necessity and possibilities of classification of aphasic disability. Also the association of clinical syndromes of aphasia with particular damage to the brain is outlined. For Wernicke's aphasia (fluent aphasia with comprehension deficit) a superior posterior temporal lesion is obligatory. The persistent jargon aphasia is associated with a lesion of the supramarginal gyrus. Broca's aphasia is seen with posterior inferior frontal lesions, but additional central and subcortical components are involved in persisting deficit. The

Altered functional connectivity in language and non-language brain networks in patients diagnosed with acute post-stroke aphasia.

OBJECTIVE: A resting-state functional magnetic resonance imaging (rs-fMRI) approach was used to explore functional connectivity (FC) in language and non-language brain networks in acute post-stroke aphasia (PSA) patients, with a specific focus on the relationship between these fMRI results and patient clinical presentation. METHODS: In total, 20 acute PSA patients and 30 age-, sex-, and education level-matched healthy control (HC) participants were recruited and subjected to rs-fMRI imaging. In addition, western aphasia battery analyses(WAB) were used to compute aphasia quotient (AQ) values f

Gayet Wernicke's encephalopathy with cortical damage following a subtotal gastrectomy: An uncommon association.

Wernicke's encephalopathy is a pathological entity caused by Vitamin B1 (Thiamine) deficiency in malnourished individuals, especially alcoholics, patients operated for digestive surgery or suffering from gastrointestinal tract's diseases or incoercible vomiting. Classically it manifests by confusion with oculomotor disorders and ataxia. However, other neurological manifestations are possible. Magnetic resonance imaging is the gold standard imaging technique for diagnosis. It shows signal abnormality on periventricular area around the third and fourth ventricles and on mammillary bodies in the

Interhemispheric compensation: a hypothesis of TMS-induced effects on language-related areas.

Repetitive transcranial magnetic stimulation (rTMS) applied over brain regions responsible for language processing is used to curtail potentially auditory hallucinations in schizophrenia patients and to investigate the functional organisation of language-related areas. Variability of effects is, however, marked across studies and between subjects. Furthermore, the mechanisms of action of rTMS are poorly understood. Here, we reviewed different factors related to the structural and functional organisation of the brain that might influence rTMS-induced effects. Then, by analogy with aphasia studi

Metabolic encephalopathies in the critical care unit.

PURPOSE OF REVIEW: This article summarizes the most common etiologies and approaches to management of metabolic encephalopathy. RECENT FINDINGS: Metabolic encephalopathy is a frequent occurrence in the intensive care unit setting. Common etiologies include hepatic failure, renal failure, sepsis, electrolyte disarray, and Wernicke encephalopathy. Current treatment paradigms typically focus on supportive care and management of the underlying etiology. Directed therapies that target neurochemical and neurotransmitter pathways that mediate encephalopathy are not currently available and represent

Phonological alexia with optic and tactile anomia: a neuropsychological and anatomical study.

We describe a patient with phonological alexia caused by a small hemorrhage in the posterior-inferior portion of the left temporal lobe. The lesion induced a highly selective impairment of phonological reading without concomitant oral language deficits other than anomia for objects presented in the visual and tactile modalities. We propose that an intact dorsal pathway from inferior visual association areas to Wernicke's area via the angular gyrus could mediate reading by the lexical route, while damage to a ventral pathway disrupted the patient's ability to read nonwords. We suggest further t

Association of ictal aphasia with hypoperfusion in language areas in temporal lobe epilepsy patients.

Ictal clinical phenomenology, including aphasia, is usually associated with increased regional cerebral perfusion. We present an unusual pattern of ictal cerebral perfusion in three patients with pharmacoresistant, lesional temporal lobe epilepsy and ictal/postictal aphasia studied with prolonged video-EEG, ictal, and interictal SPECT and MRI for pre-surgical evaluation. Subtraction of ictal-interictal SPECT images co-registered with MRI (SISCOM) showed ictal hyperperfusion in the temporal epileptogenic area in all patients. In addition, hypoperfusion of Broca's area in one case, Wernicke's ar

Disappearance of motor tics after Wernicke's encephalopathy in a patient with Tourette's syndrome.

Tourette's syndrome (TS) is a disease characterized by multiple motor and vocal tics as well as behavioral abnormalities. The anatomic substrates of this syndrome are not defined. We report a 48-year-old man with TS in whom motor tics disappeared after the onset of a midbrain syndrome related to thiamine deficiency (Wernicke's encephalopathy). MRI study showed a lesion in the dorsal area of the midbrain. This case suggests that loops located in the midbrain tegmentum may influence the presence of motor tics in patients with TS.
