Gunshot wounds to the chest with arterial bullet embolization.

This report describes two cases involving small-caliber gunshot wounds to the chest with embolization of the bullet (.22 long rifle). In the first patient, hemodynamic status permitted additional imaging before surgery. Findings demonstrated embolization of the bullet into the right external carotid artery after penetration into the left ventricle. In the second patient, emergency thoracotomy was required to perform hemostasis following perforation of the pulmonary artery with massive hemothorax. Bullet embolization into the left hypogastric artery through an acquired aortopulmonary fistula su

Trans-mediastinal gunshot wounds: are "stable" patients really stable?

Gunshot wounds that traverse the mediastinum frequently cause serious injury to the cardiac, vascular, pulmonary, and digestive structures contained within. Most patients present with unstable vital signs signifying the need for emergency operation. An occasional patient will present with stable vital signs. Work-ups for such a patient may range from surgical exploration to radiographic and endoscopic testing to mere observation. We report our experience with diagnostic work-up of the stable patient with a transmediastinal gunshot wound. All stable patients who present to our urban level I tra

Gunshot wounds below the popliteal fossa: a contemporary review.

The management of extremity injuries above the knee has been well described, but the evaluation and treatment guidelines for penetrating injuries below the popliteal crease has received less attention. A 6-year retrospective review of 100 patients who sustained isolated below-knee gunshot wounds. Patients with proximal extremity, torso, or head wounds were excluded from review so that we could focus on principles of managing below-knee wounds. All patients were evaluated with complete physical examination, ankle-brachial index, and plain X-rays. One patient presented with hemodynamic instabili

Gunshot wounds to the extremities in children and adolescents.

Thirty-seven patients from 8 months to 17 years of age were evaluated prospectively and retrospectively after sustaining gunshot wounds to the extremities. This review supports the belief that higher velocity weapons and shotgun-wound injuries cause greater morbidity, with increased length of hospital stay and increased cost of care. Late growth abnormalities were related to recognized initial physeal injury and not to unrecognized physeal injury. Immediate neurologic deficits usually resolved partially or fully, without surgical treatment.

[Pathogenesis and treatment of gunshot wounds of soft tissues].

Based on experimental investigations and generalization of the experience with treatment of wounds the authors pay attention to principal distinctions of the gunshot wound from other kinds of open injuries, as well as to the importance of early primary surgical treatment of the gunshot wound in prophylactics of infectious complications. Data on possible use of microwave radiation and medicamentous agents for stabilization of the wound process and restriction of spread of the secondary necrosis++ are presented.

Gunshot wounds to the gravid uterus. A case report.

A pregnant woman sustained an abdominal gunshot wound during the second trimester; the bullet injured multiple loops of bowel and passed through the uterus, placenta and fetus. Although the stillborn fetus was delivered by cesarean section, a review of the literature indicated that operative delivery is not indicated when the fetus has died already. Labor and delivery are well tolerated, and an unnecessary hysterotomy is thus avoided. If labor does not ensue spontaneously, it can be induced. Approximately 40% of fetuses will survive the initial injury. In past reviews the risk of prematurity o

Cervical osteomyelitis complicating transpharyngeal gunshot wounds to the neck.

Four cases of cervical osteomyelitis complicating transpharyngeal gunshot wounds to the neck are discussed. Thorough debridement of bone and soft tissue, adequate drainage, broad-spectrum antibiotics, and rigid immobilization of the cervical spine are essential for the successful treatment of this complication. Inadequate or delayed treatment may result in prolonged hospitalization, the need for multiple surgical procedures, and possible neurologic deficits.

Diagnosis and management of abdominal gunshot wounds during pregnancy.

The literature on gunshot wounds of the gravid uterus is reviewed, and three new cases reported. Clinical management is discussed, with emphasis on the condition of the abdominal wall and other connective tissue during pregnancy, the contraindications for paracentesis, the changes during pregnancy in blood volume, central venous pressure, white blood count, and GI and GU tracts, and indications for celiotomy or cesarean section.

Pediatric Intracranial Gunshot Wounds: Lessons From the Newark Experience.

INTRODUCTION: Pediatric gunshot wounds to the head (GSWH) are among the most devastating injuries, with mortality rates ranging from 20% to 65%. Prognostic tools such as the St. Louis Score (SLS) and Rotterdam computed tomography (CT) Score have been studied, but data specific to pediatric populations are limited. This study aimed to identify factors predictive of mortality and functional outcomes in pediatric GSWH and evaluate the predictive accuracy of existing scoring systems. METHODS: We retrospectively reviewed all 14 pediatric patients (≤18 y old) with GSWH at an urban, level 1 trauma c

Spinal gunshot wounds: A systematic review of the literature.

BACKGROUND: The appropriate management of spinal gunshot wounds (GSWs) remains controversial. This systematic review presents an overview of the available scientific evidence in the literature regarding efficacy and complication rates of surgical management and antibiotic prophylaxis for spinal GSWs. METHODS: A systematic review of PubMed/Medline was conducted, with a screening of articles for low-velocity civilian spinal GSWs. Collected information included patient demographics, injury description, management decision, outcomes of surgical versus conservative management, and the incidence of

Spinal gunshot wounds and infection: a multicenter cohort study.

STUDY DESIGN: A multicenter retrospective cohort study. PURPOSE: To analyze the development of infectious complications from civilian gunshot wounds to the spine and their association with the type of prophylactic antibiotics administered and the duration of administration. OVERVIEW OF LITERATURE: Despite the risk for infections following spinal gunshot wounds, no standardized guidelines exist for their management. The limited and low-quality evidence available underscores the need for further research. METHODS: This retrospective cohort study evaluated adult civilian patients (≥18 years) t

Self-inflicted craniomaxillofacial gunshot wounds: management, reconstruction, and outcomes.

Suicide by firearm remains one of the leading causes of violence-related injury death in the United States each year. The mortality rate from these injuries is high, resulting in a paucity of outcome data in the literature regarding injuries to the maxillofacial region. This has largely been attributed to a lack of funding for research in this area compared to other leading causes of mortality in the United States. The aim of this study was to detail the authors' experience and approach to complex maxillofacial reconstruction using both local reconstructive methods and microvascular free tissu

Spinal Gunshot Wounds: Pattern and Associated Lesions in Civilians.

STUDY DESIGN: Retrospective, descriptive case series study. PURPOSE: To investigate the frequency, bone pattern, and associated lesions to the spine of gunshot wounds. OVERVIEW OF LITERATURE: Gunshot wounds are penetrating in nature and are caused by a bullet or projectile. These are becoming more common and are associated with significant sequelae, requiring long and costly multidisciplinary treatment. Associated spinal cord injuries (SCIs) in the civilian population represent 13%-17% of all spinal traumas. Spinal gunshot wounds are commonly thought to be stable; however, there is potential

Civilian extraperitoneal rectal gunshot wounds: surgical management made simpler.

BACKGROUND: Rectal injuries are associated with significant morbidity and mortality. Controversy persists regarding routine presacral drainage, distal rectal washout (DRW), and primary repair of extraperitoneal rectal injuries. This retrospective review was performed to determine the outcome of rectal injuries in an urban trauma center with a high incidence of penetrating trauma where a non-aggressive surgical approach to these injuries is practiced. METHODS: The records of all patients with a full-thickness penetrating rectal injury admitted to the Trauma Center at Groote Schuur Hospital ove

An anthropological analysis of gunshot wounds to the chest.

This analysis of gunshot trauma to the bony thorax examines 87 handgun and rifle wounds from documented cases in an effort to corroborate an earlier report and to provide the forensic community with additional literature in this area. Specifically, this study tests whether the trauma signatures associated with gunshot wounds in the bony thorax are useful in determining the direction of fire. Because the ribs occupy a significant portion of the bony thorax, they are struck more frequently than other bones and, consequently, they are the focus of this report. This study confirmed that bullets ca

Civilian craniocerebral gunshot wounds: an update in predicting outcomes.

Given the high mortality in patients sustaining intracranial injury secondary to gunshot wounds (GSWs), predictors to identify patients at increased risk of death are needed to assist clinicians early in determining optimal treatment. There have been few recent studies involving penetrating craniocerebral injuries, and most studies have been restricted to small numbers of patients, which do not allow for adequate prediction of mortality. A retrospective chart review of 298 patients who sustained GSWs to the head between 1992 and 2003 was conducted at a level 1 trauma center. Demographics, bull
