Syphilis serology and HIV infection in Harare, Zimbabwe.

OBJECTIVE: To determine the reliability of serological tests in detecting syphilis in a factory worker cohort and examine the impact of concurrent HIV infection on serological tests for syphilis. METHOD: Reactions to non-treponemal and treponemal antigens were tested using sera from a cohort of 3401 factory workers in Harare, Zimbabwe. The participants consented to regular testing for syphilis, by VDRL, and HIV using two ELISAs. All sera from men who were VDRL positive, and a random sample of VDRL negative sera, were tested by RPR, TPHA, and where appropriate FTA-Abs. From the results, men we

Zimbabwe's hospital referral system: does it work?

BACKGROUND: Anecdotal evidence has suggested inefficiency in the pyramidal health care referral system established in Zimbabwe in 1980, as part of its primary health care (PHC) model. AIM: To assess the functioning of the pyramidal referral system in two rural districts surrounding Harare, Zimbabwe, with regard to two common indicator conditions: pneumonia in children and malaria in adults. METHODS: For a three-month period, all complete inpatient records with discharge diagnoses of pneumonia or malaria from three hospitals representing different levels of care were analyzed (n = 227). Clinical description of Parabuthus transvaalicus scorpionism in Zimbabwe.

An epidemiological and clinical study of Parabuthus transvaalicus scorpionism was conducted in Zimbabwe. Ten per cent of stings resulted in severe scorpionism. The clinical features of 17 patients with severe envenomation were primarily neuromuscular, with significant parasympathetic nervous system and cardiac involvement. The clinical course was prolonged compared to other scorpion syndromes, and significant therapeutic benefit was demonstrated in terms of hospital stay in response to species specific antivenom. The case fatality rate was 0.3%, with deaths in children below 10 years and adult

Oculocutaneous albinism in an isolated Tonga community in Zimbabwe.

Oculocutaneous albinism (OCA) is a recessively inherited genetic condition prevalent throughout sub-Saharan Africa. We now describe a cluster of tyrosinase positive OCA (OCA2) cases belonging to the Tonga ethnic group living in the Zambezi valley of northern Zimbabwe. The prevalence in this region was 1 in 1000, which is four times higher than that for the country as a whole. The gene frequency for OCA2 in this population was calculated as 0.0316, with a carrier rate of 1 in 16. Molecular analysis showed that all five affected subjects from two independent families examined were found to be ho

[HIV-associated tuberculosis in Africa exemplified by Zimbabwe].

In Africa, a rapid increase of human immunodeficiency virus (HIV)-associated tuberculosis cases has been observed; 80% of a worldwide 6 million dually infected persons live in this part of the world. The annual risk of progression to clinically overt tuberculosis in dually infected persons approaches the lifetime risk in persons with tuberculosis but no HIV infection. Zimbabwe is an example which illustrates the rapid increase in notified tuberculosis cases since 1985, accounted for primarily by HIV-associated tuberculosis cases. In sputum-smear positive HIV-associated tuberculosis, classical

Management of cardiovascular disorder in Zimbabwe: 1989-1992.

The challenging task of establishing a cardiothoracic unit with open-heart surgical capability in a third-world setting emphasised the clinical need for such a facility but underscored the economic and logistic difficulties hampering its successful implementation. The common occurrence of advanced rheumatic valvular disease and the inordinately high incidence of congenital cardiac disorder in the indigenous Zimbabwean population mandates a need for their successful surgical management. The progressive maturation and dynamism of an established cardiothoracic unit necessarily requires it to pay

Snake poisoning in rural Zimbabwe--a prospective study.

Over a period of 2 years (January 1991 to December 1992) 274 cases of snake bite were admitted to hospital in the eight provinces of Zimbabwe. Of these patients, 54% were males and 88% belonged to the 6-40-year age group. Five deaths (1.8% of the total cases) were reported. The majority of snake bites (63%) occurred at night (between 6.30 p.m. and midnight) and over 74% took place during the hot rainy season, i.e. between November and April. In over 58% of the cases the victim accidentally stepped on the snake, the snake being cobra in 37%, puff adder in 20% and the black and green mamba in 18

Child feeding practices in a rural area of Zimbabwe.

This paper reports preliminary results from a study of child feeding practices in a rural area of Zimbabwe. Although Zimbabwe has relatively low infant mortality rates, it still has high child malnutrition rates. Several factors, including sex roles and relations between the sexes, access to resources, food costs and availability, time and energy costs of food production and preparation, and illness beliefs and practices, especially those concerning diarrhea and malnutrition, are examined as possible determinants of feeding practices and nutritional and health status. Results are compared to r

Early repair of frontoethmoidal meningoencephalocele in Bulawayo, Zimbabwe.

Frontoethmoidal meningoencephalocele is distinctive in its demographic distribution and its effect on growth of other facial structures. Early diagnosis and referral are of paramount importance. The aim of treatment is to remove the lesion before the deformity disturbs facial growth or alters stereoscopic vision. One-stage, craniofacial surgical intervention is the treatment of choice.

Caesarean section morbidity at district level in Zimbabwe.

The incidence of complications related to Caesarean section was studied among 643 patients operated in four hospitals in Zimbabwe. Major complications were recorded in 15% of the patients and minor complications in 27%. Among these complications, infection morbidity played the leading role. Only 53% of Caesarean section were devoid of complication or need of blood transfusion. It is concluded that in a Third World environment, Caesarean section carries a substantial risk of operative and post-operative complications.

Isolation and characterization of bovine Theileria parasites in Zimbabwe.

Theilerial parasites of cattle were isolated by a variety of methods from the Harare area of Zimbabwe. Parasite stocks were established in lymphoid cell cultures and as cryopreserved sporozoite stabilates in the laboratory. Fourteen stocks in culture were characterized by testing them with monoclonal antibodies (MAb) raised against T. parva parva and T. parva lawrencei antigen. Two of these stocks had profiles similar to T. taurotragi isolates from East Africa, the other stocks had profiles similar to T. parva parva, however, many of them failed to bind MAb No. 7, and this may be a distinctive

Variations in incidence of schistosomiasis in the highveld region of Zimbabwe.

An incidence study of Schistosoma haematobium and S. mansoni was carried out among 487 six--eleven-year old children attending schools situated in the highveld region of Zimbabwe from July 1982 to March 1984. Incidence of both schistosome species were very high and there were marked variations in incidence by locality. No significant differences in incidence were observed between the sexes or among three age groups considered. Incidence of S. haematobium exceeded by a significant amount (P less than 0.02) that found for S. mansoni and this may be attributed to the relative abundance of the two

Post-dysenteric hemolytic uremic syndrome in Bulawayo, Zimbabwe.

An outbreak of dysentery in Zimbabwe was associated with a high mortality, especially in children who developed hemolytic uremic syndrome (HUS). To examine the causes of high mortality from HUS and to suggest measures that could reduce the case fatality rate, clinical and laboratory features of 91 children with dysentery were reviewed. Of these, 14 developed HUS; their findings were compared with age-matched controls with dysentery only. Persistent alteration of consciousness after rehydration, pallor, and oliguria were early clinical indicators of HUS. Leukocytosis and leukemoid reaction, mic
